Constipation and meconium ileus equivalent in patients with cystic fibrosis.

Constipation and its complications, particularly meconium ileus equivalent, may become management problems in patients with cystic fibrosis. … uch higher prevalence of both conditions. Those with prolonged histories of inadequately controlled steatorrhea appeared to be at higher risk for the eventual development of meconium ileus equivalent. Recurrences and complications of constipation may be avoided by instituting early and aggressive therapy.

Familial amyloidosis with polyneuropathy: roentgenological and gastroscopic appearance of gastrointestinal involvement.

Forty-three patients suffering from familial amyloidosis with polyneuropathy were examined by means of barium meal, barium follow through, barium enema, cholecystogram, and gastroscopy. The findings were reviewed with respect to the presence of steatorrhea and impaired d-xylose absorption. Seven patients had gastric stasis (without gross anatomical changes) on barium meal examination and seven others on the gastroscopy. Gastroscopy revealed that another five patients had inactive peristalsis of the antrum-pylorus. Only one patient had slow transit of barium on follow through. The transit time …

The effect of cimetidine and propantheline on the symptoms of a patient with systemic mastocytosis.

A 24 year old white woman with a lifelong history of systemic mastocytosis and symptoms of diarrhea and flushing was demonstrated to have a normal gastric analysis and inconsistent steatorrhea. She responded well to oral cimetidine therapy for 11 months. A symptomatic recurrence was controlled with the addition of propantheline. Gastric secretory studies demonstrated cimetidine suppression of both basal acid and basal pepsin secretion, as well as maximal pentagastrin-stimulated acid secretion; suppression of stimulated pepsin secretion was minimal. The combination of cimetidine and propantheli…

Whipple's disease involving the pericardium: Pathological confirmation during life.

Cardiac involvement in Whipple's disease has been a frequent autopsy finding but is rarely recognized clinically. We report here a patient, a 63-year-old man, in whom Whipple's disease was diagnosed in 1974, based on a seven-year history of arthralgia, one-year history of weight loss and steatorrhea, and diagnostic small bowel biopsy. Despite complete regression of all joint and bowel symptoms following a prolonged course of tetracycline therapy, the patient developed incapacitating congestive heart failure and signs of constrictive pericarditis, for which a thoracotomy and pericardectomy was …

Diabetic microangiopathy in patients with pancreatitic diabetes mellitus.

Clinically evident diabetic microangiopathy (retinopathy and nephropathy) occurred in 18% of diabetic patients with acute pancreatitis and 1 … he diabetes, rather than the insulin being a causative factor of the microangiopathy. The degree of steatorrhea in diabetic patients with chronic pancreatitis did not protect against the development of microangiopathy.

Cholestyramine and medium-chain triglyceride in prolonged management of patients subjected to ileal resection or bypass.

The pathophysiology of cholerrheic enteropathy is described and a series of patients reviewed. Of 11 patients with chronic disabling diarrhea and steatorrhea after ileal resection or bypass, two had recurrent ileitis, three had lactose intolerance and six of those operated on five years or more previously had vitamin B(12) deficiency. Cholestyramine was given alone or with medium-chain triglyceride (MCT) or Portagen (MCT and lactose). The maximal response occurred when cholestyramine was given with Portagen - significantly reduced stool frequency and weight in all patients and stool fat in fiv…

Tracking the Path of Migratory Pain: Unveiling Whipple Disease.

Whipple's disease (WD), a rare systemic infection caused by Tropheryma whipplei, often presents diagnostic challenges due to its nonspecific and varied clinical manifestations. We report the case of a woman initially diagnosed with undifferentiated connective tissue disease, who later developed severe malabsorptive symptoms, including steatorrhea and weight loss. Imaging revealed mesenteric lymphadenopathy, while histopathology and polymerase chain reaction (PCR) confirmed WD. Treatment included intravenous ceftriaxone followed by prolonged oral cotrimoxazole, leading to significant clinical i…

CT features of Wolman disease (lysosomal acid lipase enzyme deficiency) - A case report.

Wolman disease is a lethal rare autosomal recessive disorder defined by the deficiency of acid lipase enzyme. The disease is a lysosomal storage disease. Multiple organs such as adrenal glands, liver, spleen, bone marrow, small bowel loops, and abdominal lymph nodes are infiltrated by the deposition of lipids. Infants generally present with failure to thrive, abdominal distention, vomiting, steatorrhea, and hepatosplenomegaly. Authors' present a 1 month-old male infant with abdominal distention and failure to thrive who was referred for abdomen CT scan. The CT scan revealed stippled calcificat…

Exocrine pancreatic function in children with Alagille syndrome.

Alagille syndrome (AGS) is often associated with symptoms of maldigestion, such as steatorrhea, hypotrophy and growth retardation. Exocrine pancreatic insufficiency was proposed as the underlying cause. We aimed to assess the exocrine pancreatic function with the use of different methods in AGS patients. Concentrations of fecal elastase-1 (FE1) and fecal lipase (FL) activities were measured in 33 children with AGS. The C-mixed triglyceride breath test (MTBT) in a subgroup comprising 15 patients. In all patients studied, FE1 concentrations and FL activities were normal. Abnormal MTBT results we…

A preterm infant with secondary carnitine deficiency due to MCT formula--effective treatment of L-carnitine.

We report a preterm infant who was prescribed an MCT formula and subsequently developed carnitine deficiency with liver dysfunction and an elevation of serum CK level. A male infant who had been born at 24 weeks' gestation with birth weight 799 g, was fed with an MCT formula containing 76.8% of all kinds of lipids, because of his steatorrhea after the 30th day. On the 100th day, he was noted hepatomegaly and elevation of serum levels of AST, ALT and CK. The needle biopsy of the liver indicated the existence of the liver damage. He showed low serum carnitine with high urinary loss of acylcarnit…

Cystic fibrosis. As they grow older.

Twenty-eight patients with cystic fibrosis, who survived to over 18 years of age are reported. The ages at the time of diagnosis ranged from and two and half months to over 21 years. Manifestations of cystic fibrosis in adults and youngsters are compared and contrasted. A review of the patients surviving shows that steatorrhea is under control. There is no evidence of biliary cirrhosis in any of these patients. Rectal prolapse has not been an important factor. Pseudomonas and Staphylococci were the major infecting organisms causing lung disease. Two-thirds of the patients do not use prophylact…

[Digestive intolerance to cow's milk proteins in infants. Biological and histological study].

Biological and histological studies were performed in 42 cases of gastro-intestinal intolerance to cow's milk proteins. Hypoproteinemia was present in 2/3 of the cases, anemia in 3/4. Malabsorption syndrome (steatorrhea, xylose) was overt in less than 1 patient out of 2. Serum level of vitamin A was more frequently depressed than that of vitamin E and folinic acid. Intestinal biopsy, performed in 32 children, showed consistent abnormalities, a partial atrophy of the villi being most often seen (26 cases). Eosinophilia was present in 50% of the cases. A titer of anti-milk agglutinins of 1/64 or…

Stagnant loop syndrome resulting from small-bowel irradiation injury and intestinal by-pass.

Stagnant or blind-loop syndrome includes vitamin B12 malabsorption, steatorrhea, and bacterial overgrowth of the small intestine. A case is presented to demonstrate this syndrome occurring after small-bowel irradiation injury with exaggeration postenterocolic by-pass. Alteration of normal small-bowel flora is basic to development of the stagnant-loop syndrome. Certain strains of bacteria as Bacteriodes and E. coli are capable of producing a malabsorption state. Definitive therapy for this syndrome developing after severe irradiation injury and intestinal by-pass includes antibiotics. Rapid sym…

Anderson's disease/chylomicron retention disease in a Japanese patient with uniparental disomy 7 and a normal SAR1B gene protein coding sequence.

BACKGROUND: Anderson's Disease (AD)/Chylomicron Retention Disease (CMRD) is a rare hereditary hypocholesterolemic disorder characterized by a malabsorption syndrome with steatorrhea, failure to thrive and the absence of chylomicrons and apolipoprotein B48 post-prandially. All patients studied to date exhibit a mutation in the SAR1B gene, which codes for an essential component of the vesicular coat protein complex II (COPII) necessary for endoplasmic reticulum to Golgi transport. We describe here a patient with AD/CMRD, a normal SAR1B gene protein coding sequence and maternal uniparental disomy…

Malnutrition, steatorrhoea and pancreatic head tumour.

A 61-year-old female patient is described who presented with weight loss, steatorrhoea and enlargement of the pancreatic head. Surgical exploration for suspected pancreatic cancer revealed multiple peritoneal white spots, initially suggestive for peritoneal metastases or tuberculosis but finally identified as peritoneal sarcoidosis. Pancreatic insufficiency could not be proven in further studies. We found pancreas divisum as an additional cause for the pancreatic head mass, and steatorrhoea was due to late-onset oligosymptomatic coeliac disease. This case demonstrates diagnostic pitfalls when
