Tightened Sulci in the High Convexities as a Noteworthy Feature of Idiopathic Normal Pressure Hydrocephalus.

OBJECTIVE: The presence of tightened sulci in the high-convexities (THC) is a key morphological feature for the diagnosis of idiopathic normal pressure hydrocephalus (iNPH), but the exact localization of THC has yet to be defined. The purpose of this study was to define THC and compare its volume, percentage, and index between iNPH patients and healthy controls. METHODS: According to the THC definition, the high-convexity part of the subarachnoid space was segmented and measured the volume and percentage from the 3D T1-weighted and T2-weighted magnetic resonance images in 43 patients with iNP…

Surgical treatment of post-lumbar puncture dural CSF leak causing chronic headache. Case report.

A 58-year-old woman experienced incapacitating headache and occipital paresthesiae for 5 years after lumbar myelography. Conservative methods of treatment failed. Successive investigations for a suspected cerebrospinal fluid (CSF) leak were unrevealing. Leakage of CSF from the subarachnoid space into the epidural space in the lumbar region was finally confirmed when oil-soluble contrast material (Pantopaque), injected into the cervical subarachnoid space, was revealed by a specific technical modification to be escaping from the lumbar sac. Repair of the dural defect with dorsolumbar fascia res…

Symptomatic tarlov cyst following spontaneous subarachnoid hemorrhage.

Most of Tarlov or perineurial cysts remain asymptomatic throughout the patient's life. The pathogenesis is still unclear. Hemorrhage has been suggested as one of the possible causes and trauma with resultant hemorrhage into subarachnoid space has been suggested as an origin of these cysts. However, Tarlov cysts related to spontaneous subarachnoid hemorrhage has not been reported. The authors report a case of Tarlov cyst which was symptomatic following spontaneous subarachnoid hemorrhage.

Spinal subarachnoid tumor seeding from intracranial metastasis: MR findings.

We describe the magnetic resonance findings in two cases of spinal subarachnoid seeding from an intracranial metastatic lesion. Magnetic resonance imaging techniques using T1 and intermediate T1-T2 pulse sequences improve anatomical definition of the spinal cord-thecal sac and allow for optimal contrast to define metastatic deposits in the subarachnoid space. Heavily T2 weighted pulse sequences do not appear to be useful in the diagnosis of subarachnoid metastases.

Cerebrospinal fluid leakage after lumbar puncture in neonates: incidence and sonographic appearance.

OBJECTIVE: The purpose of this article is to review the incidence and sonographic appearance of cerebrospinal fluid leakage after lumbar puncture in the neonatal period. Thirty-three neonates underwent spinal sonography after diagnostic lumbar puncture. A total of 21 of these patients showed cerebrospinal fluid leakage into the epidural space extending from the level of the cauda equina to the lumbar (n = 9), the thoracic (n = 8), or the cervical (n = 4) region. In eight patients, the subarachnoid space was markedly compressed by the epidural fluid collection. CONCLUSION: Cerebrospinal fluid …

Computed tomography in the hydrocephalic patient after shunting.

The interpretation of postshunt computed tomographic (CT) scans of hydrocephalic patients presents numerous difficulties unless the interpreter has a thorough knowledge of the clinical symptoms, type and cause of hydrocephalus, preshunt morphology of ventricles and subarachnoid spaces, possible complications of shunt placement, and signs of shunt failure. A review of 368 scans from 108 hydrocephalic patients included subgroup statistical analysis of the development of ventricular asymmetry and of subarachnoid space changes after shunting. The situation and appearance of ventricular catheters w…

[Modern concepts of anti-inflammatory therapy in bacterial meningitis].

Pathophysiologic disorders in bacterial meningitis and their influence on the course and prognosis of the disease are analysed. Deterioration of the CNS is due to inflammatory response of the host combined with other pathophysiological disorders. Inflammatory response in the subarachnoid space is the most intensive, but the most deleterious, as well, in the first several hours of antibiotic therapy when bacterial disintegration ensues. Fragments of the cell wall and endotoxin are very active components, independent of the presence of living bacteria in the subarachnoid space. Each microorganis…

External hydrocephalus in infants.

External hydrocephalus means abnormal fluid accumulation in the subarachnoid space under increased pressure with no or slight widening of the ventricles. 9 children were investigated because of pathologically increasing head circumference and abnormal transillumination. PEG and/or CT showed widened sulci frontally, parietally and interhemispherically but no widening of the ventricles. 7 patients were subjected to exploratory craniotomy which disclosed a deep arachnoid space. 2 patients were shunted. All follow-up CT examinations showed normal conditions. We suggest that infants with clinical s…

[CT-cisternography of the basal cisterns. A roentgen anatomic study].

Air cisternograms at post mortem and positive contrast cisternograms on patients were performed in order to study intracisternal structures, particularly cranial nerves, as seen on CT. Air and contrast CT cisternograms showed excellent demonstration of the second, third, fifth, sixth, seventh and eighth cranial nerves. The ninth and tenth cranial nerves could not always be separated from each other and demonstration of the first, fourth, eleventh and twelfth cranial nerves was often not possible or was unsatisfactory. With a knowledge of the normal anatomy and of important surrounding structur

Racemose neurocysticercosis presenting with thalamic stroke: A case report and literature review.

Racemose neurocysticercosis is an uncommon type of neurocysticercosis that represents a particularly aggressive infection. It is characterized by the presence of multiple confluent cysts within the subarachnoid space and it carries unique diagnostic challenges. Clinical manifestations include headache, cerebrovascular events, and life-threatening hydrocephalus. A 56-year-old female presented with sudden onset headache and right-sided hemisensory loss. Brain MRI revealed multiple cystic lesions in the subarachnoid space consistent with racemose neurocysticercosis and left thalamus acute lacunar…

Intracranial transthecal subarachnoid fat emboli and subarachnoid haemorrhage arising from a sacral fracture and dural tear.

We present the case of a 28-year-old man with an unusual aetiology of lipid-dense material in the subarachnoid space. CT of the head at presentation was normal. MRI of the spine revealed a defect in the dura at L5/S1, with avulsed left L5 and S1 nerve roots. Haematoma and marrow fat were observed in close relation to the dural tear adjacent to the sacral fracture. Head CT and MRI subsequently demonstrated new lipid-dense material and haemorrhage in the subarachnoid space after sacral instrumentation, presumably owing to transthecal displacement of fatty marrow.

Intraoperative spinal sonography in the cervical anterior approach.

Intraoperative spinal sonography was used during cervical anterior approach procedures for cervical discectomy and osteophytectomy to demonstrate spinal pulsation, the protruded disc or osteophyte, the anterior subarachnoid space, and the spinal cord. Spinal pulsation was recognized in some cases before removal of the disc but the anterior subarachnoid space and spinal cord could not be observed. However, the latter were more clearly observed during removal of the disc and could be seen after total removal of the disc and osteophyte. This method allows confirmation of decompression and pulsati…

Recurrent meningitis due to an intracranial epidermoid.

The association of intracranial dermoids and epidermoids and recurrent meningitis is usually related to the presence of dermal sinuses that allow bacterial entry. Epidermoid cysts rarely discharge their contents into subarachnoid space to cause meningitis, except after surgical removal. We describe a child who had 10 separate episodes of chemical meningitis between age 13 months and her death at age 2 years 9 months. Death was caused by an intra-axial epidermoid in the ventral pons which had ruptured into the subarachnoid space, as shown at autopsy. There was no evidence of sinus tracts, and r…

Prospective measurement of the width of cerebrospinal fluid spaces by cranial ultrasound in neurologically healthy children aged 0-19 months.

BACKGROUND: Ultrasound (US) is often the first method used to look for brain or cerebrospinal fluid (CSF) space pathologies. Knowledge of no … ues. No subdural fluid collection was detected. CONCLUSION: Normative values for the widths of the subarachnoid space and the internal CSF spaces are useful for evaluating intracranial pathology, especially when determining whether an increase in the subarachnoid space width is abnormal.

The effects of raised ICP on lymph flow in the cervical lymphatic trunks in cats.

Lymph was collected from cervical lymphatic trunks of anesthetized cats under conditions of normal cerebrospinal fluid (CSF) pressure and again when the CSF pressure was elevated by infusing artificial CSF into the subarachnoid space at the cisterna magna. There was an immediate increase in lymph flow on initiation of the CSF infusion, but this increase was not maintained although the CSF infusion continued. Lymph protein concentrations fell when the CSF infusion started and remained depressed while the infusion of CSF continued. It is postulated that under steady-state conditions much of the …

Complications associated with epidural steroid injections.

BACKGROUND AND OBJECTIVES: Warnings about the hazards of epidural steroid injections occasionally appear in both medical and lay literature despite a lack of objective data to support such concerns. This literature review was undertaken to survey reports of adverse reactions associated with that procedure. METHODS: The following types of publications from peer-reviewed medical literature was surveyed: reports on series of epidural and subarachnoid steroid injections for sciatica; reports of adverse effects of epidural and subarachnoid steroid injections for sciatica; review articles on epidur

[Use of acetazolamide in external hydrocephalus in infants].

External hydrocephalus is believed to be benign but published cases do not clearly establish this prognosis. Five children with typical external hydrocephalus were given acetazolamide. Head circumference measurements or other investigations demonstrated therapeutic effectiveness in four of the five patients. Although this series is too small to establish that acetazolamide avoided external hydrocephalus-related adverse events, the drug's outstanding tolerance and anatomic effectiveness justify widespread use in all patients with CSF resorption disorders.

Cystic papillary meningioma with subarachnoid dissemination: a case report and review of the literature.

Meningiomas usually present as benign tumors corresponding to WHO grade I. The development of the papillary variant of meningiomas with cyst formation in the central nervous system is extremely rare. We report a case of cystic papillary meningioma in a young female occurring in the lateral ventricle with invasion of brain parenchyma and dissemination of subarachnoid space. The tumor exhibits a marked peritumoral cyst, with contrast enhancement on magnetic resonance imaging (MRI) in accordance with type 2 of Zee's classification of cystic meningioma. Histologically, the tumor displays a classic…
