The anatomical value of technetium-thallium subtraction scanning in detection and location of parathyroid adenomas.

99mTechnetium-201Thallium subtraction scanning was performed in 24 patients with primary (N = 5) and secondary (N = 19) hyperparathyroidism. The preoperative scintigraphy (N = 12) detected 21 of 23 enlarged glands surgically removed and was helpful for detecting abnormal location especially in the mediastinum. Postoperative scanning in patients with recurrent hyperparathyroidism confirmed the excessive growth of the remaining half parathyroid after subtotal parathyroidectomy or a missing fifth parathyroid after total parathyroidectomy and autotransplantation. False negative results were due to

Carotid artery assessment with continuous wave Doppler ultrasound, comparison of two methods with angiography.

A comparison of two non-invasive techniques of evaluation of the internal carotid artery in the neck with the results of angiography was carried out in 31 patients. The techniques were, spectral analysis of the doppler signals obtained from the common, internal and external arteries and, velocity waveform analysis of the common carotid analogue signal, in both methods a continuous wave doppler ultrasound velocimeter was used. With the spectral analysis technique sensitivity was 80% and specificity was 78% for a diagnosis of internal carotid stenosis of greater than 50% diameter reduction. For

Aortic Displacement of a Pulmonary Stent: Multidisciplinary Approach to Treat a Rare Complication.

BACKGROUND: Pulmonary vein (PV) stenosis is a rare complication of radiofrequency-based pulmonary vein isolation (PVI). Its therapy consists of PV stenting. CASE SUMMARY: A 67-year-old woman underwent a thoracic computed tomography scan after left PV stenting for stenosis after PVI. The scan revealed the stent missing in the left inferior PV and worsening high-grade stenosis. An abdominal computed tomography scan identified the displaced stent in the aorta, just above the aortoiliac bifurcation. Digital subtraction angiography also revealed moderate stenosis in the right common iliac artery.

Transvenous embolization of a direct carotid-cavernous fistula through the pterygoid plexus approach.

We present a transvenous embolization technique for a direct carotid-cavernous fistula through the pterygoid plexus to the cavernous sinus which only 2 cases have been previously reported in the English literature. This method is appropriate when transarterial techniques or other attempts at transvenous access have failed due to vessel tortuosity, hypoplasia, stenosis, or occlusion. A middle-aged female patient presented with progressive left exophthalmos with conjunctiva chemosis and bruit after sustaining a falling injury. Digital subtraction angiography revealed Barrow type A carotid-cavern

[Recent progress in radionuclide imaging of parathyroid glands].

Scintigraphic imaging of parathyroid glands has been the subject of new developments. Efficiency of the technique has been improved by the use of a new agent Tc-99m sestamibi (MIBI). A meta-analysis study of the literature confirmed the superiority of this agent toward the conventional Tc/T1201. Various protocols have been proposed for parathyroid imaging with MIBI they are described with a comparison of their advantages and drawbacks. The superiority of a two isotopes protocol using Tc-99m sestamibi with Iodine 123 and a subtraction processing seems proven. Moreover, this latter protocol allo

Gradient echo MRI of portal vein thrombosis.

The MR appearance of thrombosis of the portal vein and its branches using gradient echo (GRE) sequences is described. The study consists of two separate parts. In the first part, five normal volunteers were examined to select the optimal section plane for each portal vessel to be studied. Given the "time of flight" effect of GRE sequences, a scan plane perpendicular to the direction of flow was used to obtain maximal signal enhancement of flowing blood. In the second part of the study, 13 patients with thrombosis of the portal system diagnosed by Doppler sonography, CT, and digital subtraction

Three-dimensional rotational digital angiography in a complicated case of patent ductus arteriosus transcatheter closure.

There has been no report so far as to the usefulness of 3D rotational angiography in congenital heart disease interventions. We present a case of a difficult patent ductus arteriosus in which 3D rotational angiography was the key to successful closure. A 64-year-old woman who had been operated on many years previously for a patent ductus arteriosus came to our attention for a heart murmur on the left upper sternal border. Echocardiography revealed a still patent duct with moderate shunt. The patient underwent cardiac magnetic resonance imaging, which confirmed patency of the duct, but its diam

[Angiographic findings of vertebral dissecting aneurysm. Report of two cases and review of literature].

The authors report two cases of vertebral dissecting aneurysm. The first case, a 49-year-old female, developed severe headache and computed tomography scan showed subarachnoid hemorrhage (SAH), but 4-vessel cerebral angiography failed to show an aneurysm. The second angiograms obtained 2 weeks later showed possible aneurysmal dilatation on the right vertebral artery. The third angiograms, 2.5 months after SAH, disclosed a right vertebral fusiform aneurysm on the arterial phase and it was diagnosed as a dissecting aneurysm since the contrast medium remained until the very late venous phase. 3-D High-Quality Power Doppler Using Coherence-Based Null Subtraction Imaging.

Three-dimensional power Doppler (PD) imaging enables visualization of volumetric microvasculature networks. However, 3-D ultrafast PD imaging with matrix arrays suffers from limited resolution due to the restricted aperture size and the use of unfocused plane waves, while delay-and-sum (DAS) exhibits high sidelobe levels (SLLs) and broad main lobes, hindering high-quality PD imaging. In this study, we aim to improve the 3-D null subtraction imaging (NSI) and propose coherence-based NSI beamformers, including NSI based on coherence factor (NSI-CF) and NSI based on spatial and angular CF (NSI-SA

Assessment of scatter components in high-resolution PET: correction by nonstationary convolution subtraction.

UNLABELLED: This paper describes a new approach to determine individual scatter kernels and to use them for scatter correction by integral transformation of the projections. METHODS: Individual scatter components are fitted on the projections of a line source by monoexponentials. The position-dependent scatter parameters of each scatter components are then used to design non-stationary scatter correction kernels for each point in the projection. These kernels are used in a convolution-subtraction method which consecutively removes object, collimator and detector scatter from projections. This

Internal carotid artery hypoplasia.

Hypoplasia of the carotid arteries is a rare congenital anomaly which when clinically symptomatic presents as cerebral ischemia or hemorrhage. Patients with this anomaly commonly have associated variations of the circle of Willis, an increased incidence of intracranial aneurysms and extensive networks of collateral vessels to accommodate the diminished carotid flow. Carotid artery hypoplasia should not be confused with other conditions which may have a similar appearance such as arterial dissection, vasculitis, and tubular fibromuscular hyperplasia.

Interobserver variability in assessing renal artery stenosis by digital subtraction angiography.

Four experienced observers independently assessed digital subtraction angiograms (DSA) of 134 renal arteries in 67 patients. Only the main renal arteries were evaluated. All patients were hypertensive and were screened for the existence of renal artery stenosis. The findings were classified in four categories: 0-49% stenosis; 50-99% stenosis; total occlusion, and fibromuscular dysplasia (FMD). As a measure of interobserver agreement, the kappa-statistic, which corrects for agreement by chance alone, was used. A kappa-value of 0.65 +/- 0.06 (kappa +/- SE) resulted for all observers and all cate

Angiography of the testicular artery. III. Testis and epididymis analysed with a magnification technique.

The resolving capacity of magnification testicular angiography, as related to the known vascular anatomy of the testis and epididymis, was analysed on the basis of 9 normal angiographies and 8 cases of hydrocele with proven normal status of the testis and epididymis. The intratesticular vascular arrangement was demonstrated remarkably well, but of the epididymal vascularity only that in the head of the epididymis. The vascular anatomy of the testis as resolved by angiography may be utilized in the diagnosis of the location and nature of intra- and extratesticular mass lesions.

Scintigraphic detection of gastro-pulmonary aspiration in patients with respiratory disorders.

The scintigraphic detection of small nocturnal aspirations of radio-labelled gastric contents is difficult in the presence of high remaining activity in the abdomen, causing a non-uniform background activity. This problem was examined in phantom experiments and a technique for interpolative background correction was further developed. The accuracy of this technique was found to be influenced by the distance between the lung and the abdominal source of activity, and the minimum detectable 'aspirated' activity was determined as 0.1 MBq at a distance of 15 cm and 1 MBq at 5 cm. The interpolative

Novel neuroimaging modalities in the evaluation of spontaneous cerebrospinal fluid leaks.

Although for the vast majority of patients with spontaneous intracranial hypotension knowledge of the exact site of the underlying spinal CSF leak is not necessary, it is for patients with recalcitrant symptoms. Such patients may require directed treatments such as percutaneous fibrin glue injections or surgery. A variety of MRI techniques have been shown to be able to detect CSF leaks as well and sometimes better than the "gold standard" - CT-myelography. For unusually rapid CSF leaks - particularly those ventral to the spinal cord - digital subtraction myelography or dynamic CT-myelography a

Neuroradiology of the skull base.

Newer imaging modalities, especially high resolution computed tomographic scanning, have radically altered and expanded the role of radiology in the evaluation and management of lesions affecting the skull base. The location, extent, and tissue characteristics of these lesions can be determined with greater precision than ever before. When necessary, diagnostic and therapeutic angiography can be performed more rapidly and more safely with digital subtraction techniques. In the near future magnetic resonance imaging may prove to be an even more precise imaging tool than computed tomography. Mag
