Cerebral function and muscle afferent activity following intravenous succinylcholine in dogs anesthetized with halothane: the effects of pretreatment with a defasciculating dose of pancuronium.

The effects of iv succinylcholine (SCh) on cerebral blood flow (CBF), muscle afferent activity (MAA), electromyographic activity (EMG), visible fasciculations, and PaCO2 were tested in 12 dogs anesthetized with 0.87% end-expired halothane (1 MAC). Six dogs (group I) received treatments of both SCh 1.0 mg/kg iv and saline placebo 3.0 ml iv. Fasciculations and substantial increases in EMG activity were observed in all six dogs given SCh. At the onset of fasciculations, there were parallel increases in MAA and CBF to peak values of 466% +/- 77% of control (mean +/- SE) and 136% +/- 5% of control,…

Chlorocresol, an additive to commercial succinylcholine, induces contracture of human malignant hyperthermia-susceptible muscles via activation of the ryanodine receptor Ca2+ channel.

BACKGROUND: A defect in the ryanodine (Ry1) receptor Ca2+ channel has been implicated as one of the possible underlying causes of malignant hyperthermia (MH), a pharmacogenetic disorder characterized by sustained muscle contracture. The disease is triggered by common halogenated anesthetics and skeletal muscle relaxants, such as succinylcholine. This study tested whether the functional properties of the Ry1 receptor Ca2+ channel are affected by chlorocresol, a preservative added to a commercial preparation of succinylcholine (Midarine) and other parenteral compounds. METHODS: In vitro contrac…

Effect and safety of mivacurium chloride and succinylcholine for bronchoscopy: study protocol for a single-center, randomized, non-inferiority, and positive-controlled clinical trial.

BACKGROUND: Succinylcholine is favored for its rapid onset and offset, but its use is limited by adverse effects. Mivacurium chloride, a non-depolarizing relaxant, offers a similar shorter duration and fewer cardiovascular effects, especially without worrying about muscle pain and hyperkalemia, making it a potential alternative in some short-time operations. This study aims to assess the non-inferiority of mivacurium chloride compared to succinylcholine in terms of muscle relaxation effect and safety during flexible bronchoscopy. METHODS: This is a prospective, single-center, randomized, asse…

Serial Fetal Heart Rate Monitoring During Electroconvulsive Therapy in Late Pregnancy: A Case Report of Transition From Succinylcholine to Rocuronium Reversed With Neostigmine.

Electroconvulsive therapy (ECT) is considered relatively safe and effective for severe psychiatric disorders during pregnancy, but evidence regarding neuromuscular blockers for ECT in pregnancy remains limited. We report a 30-year-old primigravida admitted at 30 weeks of gestation with severe major depressive disorder and persistent suicidal ideation despite pharmacotherapy. ECT was initiated at 32 weeks and 2 days of gestation. The first five sessions used succinylcholine. Owing to a nationwide shortage in Japan, the muscle relaxant was switched to rocuronium from the sixth session, with neos…

Simultaneous determination of succinylcholine and its metabolite in animal-derived foods by solid-phase extraction combined with ultra-performance liquid chromatography/mass spectrometry.

RATIONALE: Succinylcholine has been increasingly used in the theft of animals. Because of the presence of residual levels of succinylcholine in poisoned animals, it is harmful for people to eat foods derived from these animals. Therefore, a method should be immediately established to determine succinylcholine and its metabolite in animal-derived foods. METHODS: A fast, highly sensitive method, combining solid-phase extraction (SPE) with ultra-performance liquid chromatography/electrospray ionization tandem mass spectrometry (UPLC/ESI-MS/MS), was developed for the determination of succinylchol…

Comparison of emergence agitation between succinylcholine and rocuronium-sugammadex in adults following closed reduction of a nasal bone fracture: a prospective randomized controlled trial.

BACKGROUND: Sugammadex allows rapid recovery from rocuronium-induced neuromuscular blockade. Succinylcholine is often used for brief surgeries but is associated with myalgia, headache, histamine release, and increased lactate levels. Thus, we hypothesized that succinylcholine may affect emergence agitation (EA) and compared the effects of succinylcholine and rocuronium-sugammadex on EA in patients undergoing closed reduction of a nasal bone fracture under general anesthesia. METHODS: Forty-two patients were prospectively enrolled and allocated randomly to the succinylcholine group (group SC) …

Enhancing effect by nicotinic acetylcholine receptor channel blockers, including beta-eudesmol, on succinylcholine-induced inhibition of twitch tension and intracellular Ca++ in mouse diaphragm muscle.

To elucidate the mechanism of neuromuscular block by succinylcholine, nerve-evoked changes in intracellular Ca(++)-aequorin luminescence and twitch tension were measured simultaneously in the presence of several different types of blockers for the nicotinic acetylcholine receptor channel. Mouse diaphragm muscles were pretreated for 30 to 60 min with 3 to 40 microM bupivacaine, chlorpromazine, phencyclidine and beta-eudesmol. The effects of these noncompetitive blockers on the succinylcholine-induced response were also compared with those for pancuronium. These channel blockers potentiated (2- …

Effects of succinylcholine and related substances administered into the medial preoptic area on the local EEG, body temperature, heart rate, galvanic skin resistance and biogenic amines.

Succinylcholine (Sch) which is a cholinergic neuromuscular blocker has been known to occasionally lead to episodes of malignant hyperthermia in swine and humans. In order to find whether it produces any hyperthermic effects through action on medial preoptic area, experiments were carried on by administering intracerebrally the chemical into the medial preoptic area through an in-dwelling cannula-cum-electrode in the free moving rat. The changes in body temperature and the local EEG were studied. For comparison purpose, the effects of carbachol, atropine and phenylephrine were also studied. Fur…

Pretreatment with magnesium sulphate is associated with less succinylcholine-induced fasciculation and subsequent tracheal intubation-induced hemodynamic changes than precurarization with vecuronium during rapid sequence induction.

Although it has side effects, succinylcholine is still widely used in rapid sequence induction. The aim of the present study is to evaluate the effects of pretreat ment with magnesium and precurarization of vecuroni um on succinylcholine-induced fasciculation and subse quent tracheal intubation-induced hemodynamic changes during rapid sequence induction. Fifty-five patients were allocated to three groups by a blinded randomization: Group M received saline 100 ml with magnesium 40 mg x kg(-1) for 5 min at 6.5 min before induction and sub sequently administered saline 1-2 ml at 1.5 min before in…

[Plasma cholinesterase and abnormal reaction to suxamethonium injection. 20-year experience with the Danish Cholinesterase Registry].

For more than 20 years, the Danish Cholinesterase Research Unit (DCRU) has collected information about patients showing an abnormal response to succinylcholine. The purpose of this study was to evaluate our clinical findings in patients referred because of prolonged response following succinylcholine. Also, we wanted to evaluate the results of our prospective controlled studies of the effect of succinylcholine in patients with normal and abnormal plasma cholinesterase genotypes. An explanation for the apparent abnormal response to succinylcholine was only found in roughly 60% of the 1247 patie…

[Anaphylactic shock caused by suxamethonium (Curacit)].

We describe five cases of anaphylactic shock following the short-acting muscle relaxant succinylcholine (suxamethonium). All the patients recovered uneventfully. Skin prick testing identified succinylcholine as the probable causative agent in four cases. The incidence of anaphylaxis to succinylcholine in our material appears to be 1:1,000. Treatment consisted of volume substitution, adequate oxygenation and adrenergic agents. We discuss the routine use of succinylcholine for intubation in elective surgery, and conclude that the anaesthesiologist must always be prepared for a possible anaphylac…

The length-tension diagrams of human oblique muscles in trochlear palsy and strabismus sursoadductorius.

We determined the relation between length and tension in detached oblique muscles of 16 strabismus patients that underwent surgery, before and during contraction evoked by intravenous administration of succinylcholine. We frequently found a nonlinear relation between length and tension, unlike our previous findings in recti. In superior oblique palsies, the superior oblique was found, before injection of succinylcholine, to be stiff after elongation, and did not contract after injection of succinylcholine, while the ipsilateral inferior oblique contract after injection of succinylcholine, but …

Malignant hyperthermia and ECT.

The malignant hyperthermia syndrome is a rare hypermetabolic state that occurs in patients after the administration of certain drugs, most frequently anesthetics and/or succinylcholine. In view of the widespread use of succinylcholine to induce muscle relaxation prior to ECT, it is important that the individual who administers succinylcholine in this setting be familiar with the syndrome and its treatment. This report reviews what is known about malignant hyperthermia and proposes a course of action when the syndrome occurs in a patient undergoing ECT or a patient with known susceptibility to …
