Cardiac conduction disorders in six infants with "near-miss" sudden infant deaths.

Cardiac conduction disorders caused sudden serious illnesses in six infants that might have been fatal if diagnosis and treatment had been delayed. These cases provide circumstantial evidence to support a link between cardiac conduction disorders and some sudden infant deaths. A further potential long-term effect of these disorders is illustrated in one child in whom psychomotor retardation seemed to develop after an episode of cerebral hypoxia that was probably by an arrhythmia associated with the Wolff-Parkinson-White syndrome. Cardiac conduction disorders may be detected by routine neonatal…

Overrepresentation of the proarrhythmic, sudden death predisposing sodium channel polymorphism S1103Y in a population-based cohort of African-American sudden infant death syndrome.

BACKGROUND: The S1103Y-SCN5A polymorphism has been implicated as a proarrhythmic, sudden death predisposing risk factor in African Americans, including one postmortem investigation of African-American infants with sudden infant death syndrome (SIDS). OBJECTIVE: The purpose of this study was to assess whether the relatively African-American-specific common polymorphism S1103Y in the SCN5A-encoded cardiac sodium channel is overrepresented in SIDS among African Americans. METHODS: Seventy-one cases from a population-based cohort of unexplained infant deaths among African Americans (37 females a…

Risk factors associated with sudden unexplained infant death: a prospective study of infant care practices in Kentucky.

OBJECTIVE: To ascertain the prevalence of infant care practices in a metropolitan community in the United States with attention to feeding r … months compared with white parents (adjusted OR: 0.14; 95% CI: 0.06-0.33). One infant succumbed to sudden infant death syndrome at 3 months of age, and another infant died suddenly and unexpectedly at 9 months of age. Both were bed sharing specifically with 1 adult in the former and with 2 children in the latter. CONCLUSIONS: Bed sharing and prone placement were mo …

A quest for a histaminergic or orexinergic biomarker for sudden infant death syndrome.

BACKGROUND: Sudden infant death syndrome (SIDS) occurs predominantly during sleep between 2 and 6 months of age, suggesting impaired maturation of arousal pathways. The neurobiological mechanisms involved remain unclear. We investigated the role of wake-promoting Orexin (Ox) and Histamine (HA) neurons in SIDS. METHODS: Cerebrospinal fluid (CSF) Ox levels were measured in 61 living controls and 70 Sudden Unexpected Death Infants (SUDI: 38 SIDS, 32 explained deaths (ED)). HA and tele-methylhistamine (t-MeHA) were analyzed in an additional 46 SUDI (34 SIDS, 12 ED) and 42 controls. Immunohistoche…

What is the mechanism of sudden infant deaths associated with co-sleeping?

The risk of Sudden Infant Death Syndrome (SIDS) has fallen dramatically in the "Back to Sleep" era; however, half the cases now occur when the infant has been sleeping in bed with another person. Despite the association of SIDS with co-sleeping, parents are receiving mixed messages. It is often presumed that co-sleeping deaths are due to 'overlaying', when the adult rolls on top of the baby, stopping baby from breathing. We examine research that shows that it is not necessary to cover the face, or squash the body of a baby to restrict or prevent breathing and cause oxygen deprivation. At birth…

Sudden death in an infant caused by rupture of a basilar artery aneurysm.

Ruptured aneurysms of the cerebrovasculature in infancy and early childhood, except for "giant" aneurysms and arteriovenous malformations, are rare. Seizures, loss of consciousness, and apnea are the usual presenting signs in infancy; symptoms such as headache or visual disturbances and signs such as cranial nerve compression or meningeal irritation commonly found in older children or adults are absent in infants. However, the morphologic findings (i.e., subarachnoid and retinal hemorrhage, and occasionally subdural hemorrhage) may be mistaken for inflicted trauma, especially if the aneurysm i

A study of the endogenous opioid system in the sudden infant death syndrome.

To investigate the possible role of the endogenous opioid system in the pathogenesis of the sudden infant death syndrome (SIDS), we measured met-enkephalin immunoreactivity by radioimmunoassay in brain, lung, and adrenal glands of SIDS victims and of infants (controls) dying of nonneurologic causes. Met-enkephalin was stable in brain tissue up to 24 hours after death. On inspection, met-enkephalin levels in the cerebral cortex of SIDS victims were similar to those in controls. Levels in the caudate nucleus were lower in infants than in adults. In the medulla, the levels in SIDS cases and contr…

Sudden infant death with dysgenesis of the testes syndrome in a non-Amish infant: A case report.

Sudden Infant Death with Dysgenesis of the Testes syndrome (SIDDT) is a very rare condition associated with biallelic pathogenic variants in the TSPYL1 gene first reported in 2004. It is characterized by sudden cardiac or respiratory arrest, disordered testicular development, neurologic dysfunction, and is uniformly fatal before the age of 12 months. There were previously 21 reported cases of SIDDT in the literature, all from nine Old Order Amish families published in a single paper. In this report, we describe a non-Amish, phenotypically female infant with poor feeding and abnormal motor move…

Infant ventricular fibrillation after ST-segment changes and QRS widening: a new cause of sudden infant death?

BACKGROUND: Ventricular arrhythmia-related sudden cardiac arrest in infants with structurally normal hearts is rare. There have been no prev … eotypical transient ECG changes. These findings may represent a new, potentially treatable cause of sudden infant death. Recognition of these prodromal changes may be important in future management and survival of these infants.

Is there a link between infant botulism and sudden infant death? Bacteriological results obtained in central Germany.

UNLABELLED: Despite the fact that botulism was described in Germany for the first time by Kerner in 1820, the disease is almost forgotten in this country. Only about 10-20 cases of classical botulism (intoxication) are recorded every year, including 1-2 cases of clinical infant botulism. As we assumed a high incidence of botulism to be connected with cases of sudden infant death (SID), we undertook the research work presented here. From every case of unexpected infant death up to 12 months of age, standardised specimens (blood, liver and intestine) were taken at autopsy. They were tested for t…

Gasping and other cardiorespiratory patterns during sudden infant deaths.

To gain information on the cardiorespiratory changes occurring immediately before sudden infant death (SID), recordings of heart rate and chest wall impedance were analyzed in nine infants who had died at a median age of 4.8 mo (range 1-6 mo) while attached to a memory monitor. Postmortem diagnoses were sudden infant death syndrome in seven infants and mild bronchopulmonary dysplasia in two infants. Primary cause of the monitor alarm was bradycardia in all but two infants. Heart rate fell to < or = 15 bpm 7.5 min (range 1.4-25.2 min) after the first alarm; there was no indication of heart bloc…

Diagnostic difficulties in cases of sudden death in infants with mandibular hypoplasia.

Infants with mandibular hypoplasia are at risk of sudden death from cardiorespiratory arrest secondary to upper airway obstruction. To evalu … inical details for evidence of airway obstruction is necessary to help distinguish these cases from sudden infant death syndrome (SIDS). Sudden death may, however, occur in infants with mandibular hypoplasia in spite of apparent clinical stability before death with no significant recent episodes of oxygen desaturation.

Myocardial inflammation, cellular death, and viral detection in sudden infant death caused by SIDS, suffocation, or myocarditis.

The significance of minor myocardial inflammatory infiltrates and viral detection in SIDS is controversial. We retrospectively compared the demographic profiles, myocardial inflammation, cardiomyocyte necrosis, and myocardial virus detection in infants who died of SIDS in a safe sleep environment, accidental suffocation, or myocarditis. Formalin-fixed, paraffin-embedded myocardial sections were semiquantitatively assessed for CD3 lymphocytes and CD68 macrophages using immunohistochemistry and for cardiomyocyte cell death in H&E-stained sections. Enteroviruses and adenoviruses were searched for

[Apnea characteristics of children who later died: comparison of sudden infant death with other causes of death].

Apnea characteristics of infants, who were destined to die, were compared with control infants, matched for gestational age, sex, birth weight and age at sleep study. From 1989 to 1994 polygraphic recordings were performed in 4268 infants. In this population 7 cases of sudden infant death (SID) and 13 deaths from other causes (non-SID) occurred. Concerning all forms of apneas a higher frequency and maximal duration was found in the SID group. The number of infants with obstructive apneas and the number of obstructive apneas per infant were higher in future SID victims, as was the frequency, me…

Antenatal and intrapartum factors associated with sudden infant death syndrome in the New Zealand Cot Death Study.

OBJECTIVE: To describe the relationship between antenatal and intrapartum factors and sudden infant death syndrome (SIDS). METHODOLOGY: The New Zealand Cot Death Study was a 3 year case-control study, with 485 infants who died from SIDS in the postneonatal period and 1800 randomly selected control infants. Data were obtained from obstetric records, parental interview and community nursing records. RESULTS: This study confirms many of the antenatal and intrapartum risk factors for SIDS noted in studies from both the southern and northern hemispheres. After controlling for potential confounder…
