The association of retropubic colposuspension for the treatment of urinary stress incontinence with genital prolapse has been reported previously. Described here is a case of an 83-year-old patient who had a colponeedle suspension and was readmitted because of genital prolapse 3 weeks after surgery. This case emphasizes the need for proper evaluation of the whole pelvic floor prior to any surgical treatment for urinary incontinence, and addition of appropriate surgical measures aimed to avoid later genital prolapse, if necessary.

Vaginal prolapse may occur following hysterectomy or may evolve with the uterus in place. Current treatment options for vaginal prolapse have a limited success rate or are associated with significant morbidity. In this retrospective review, we present our experience with a new procedure for repair of vaginal prolapse. Few theories of the P3 component have emphasized the distinction between its parietal and frontocentral parts. This study used a new paradigm for testing the predictions that the parietal P3 is evoked by awaited stimuli (suspense) and the frontal P3 by unexpected stimuli (surprise). Ten patients underwent uterine suspension in conjunction with extensive operative laparoscopy or as primary treatment for deep thrust dyspareunia due to fixed uterine retroversion. Of the 10 patients, 9 had satisfactory relief of symptoms. The Pereyra needle allows the use of very small suprapubic incisions with little increase in operative morbidity or discomfort.

The Stamey procedure was performed on 21 women aged 65 to 81 years; 17 were cured (completely dry) or significantly improved (maximum 1 pad/day). Urethral length was significantly increased by the operation but maximum urethral pressure and maximum flow rate were unchanged. Complications were few.

Twenty patients adjudged to require surgical management of genuine stress incontinence underwent the abdomino-perineal urethral suspension procedure between May, 1989 and February, 1990. Short-term subjective follow-up revealed a 94% cure and improved rate in 19 patients in whom the operation was completed. There was a striking absence of postoperative voiding morbidity. We believe this somewhat neglected procedure is worthy of further objective study and comparison with the more popular methods of surgical management of genuine stress incontinence.

A total of 48 patients with genuine stress incontinence underwent endoscopic bladder neck suspension; 36 patients (75%) were completely cured of their incontinence but 12 (25%) suffered recurrent leakage. A review of 76 patients who underwent endoscopic bladder neck suspension (EBNS) for stress incontinence is presented. Pre-operative video-urodynamic studies in 71 patients demonstrated stable stress incontinence in 61 (86%). There was a history of previous pelvic surgery in 52 patients (68%). Surgery was performed by trainee urologists in 35 cases (46%). The overall complication rate was 46%, of which 17% were per-operative and did not influence the outcome of surgery. There have been a number of reports recently on vesical calculi in women with a history of gynecological procedures. Within 1 year we encountered 2 patients with vesical calculi after vesicourethral suspension performed with concomitant hysterectomy. These calculi formed on a retained intravesical suture and a monofilament knitted polypropylene pledget, and they produced interesting pathological and radiographic findings.

Postoperative intussusception in the adult is a rarely reported and poorly understood condition. Presented is the case of a patient developing incomplete small bowel obstruction after an uneventful vaginal vault suspension. Jejunojejunal intussusception, requiring resection, was found at celiotomy. Intussusception must be considered in treating patients developing postoperative intestinal obstruction.

The diagnosis and management of patients having had superior rectus suspension procedures for the correction of blepharoptosis can be quite difficult. Vaginal surgery for the relief of stress incontinence has a failure rate which is unacceptable. Since 1964 the authors have combined in the management of female urinary disorders. Where surgery was indicated the combined approach was used as the primary procedure. The bladder neck was elevated by a technique of suspension using 2 mm Teflon tape, via the cave of Retzius. It was not a sling. The results of 100 consecutive operations are presented. The team approach has lead to a better understanding and management of all varieties of female bladder problems.
