Corticosteroids as an adjunct to antibiotics and surgical drainage for the treatment of pyogenic flexor tenosynovitis.

BACKGROUND: Many similarities exist between pyogenic flexor tenosynovitis and other closed-space infections such as septic arthritis. Previous studies have demonstrated that corticosteroids in conjunction with antibiotics considerably improve treatment outcomes in patients with septic arthritis. Using a chicken model, we investigated whether or not corticosteroids in combination with antibiotics and/or surgical drainage could minimize the loss of range of motion typically associated with pyogenic flexor tenosynovitis. METHODS: We inoculated the flexor tendon sheath of the right long toe of br…

The MRI view of synovitis and tenosynovitis in inflammatory arthritis: implications for diagnosis and management.

MRI scanning is the current gold standard modality for imaging synovitis and tenosynovitis in patients with inflammatory arthritis. Inflamed synovial membrane within the joints and investing tendon sheaths appears thickened on T1-weighted sequences and enhances postcontrast. On T2-weighted sequences, synovitis and synovial effusions typically show a high signal. Studies have shown correlations between the degree of inflammation and vascularity of synovium obtained at biopsy and postcontrast enhancement on matching dynamic MRI scans. Scoring systems have been devised that are based on quantifyi…

[Role of US in the management of de Quervain's tenosynovitis: review of 22 cases].

PURPOSE: To assess the role of US in the management of de Quervain's tenosynovitis. MATERIALS AND METHODS: Using a high-frequency transducer (12 MHz), sagittal and axial images were obtained of the first extensor compartment. The US features of 22 symptomatic patients were reviewed (positive Finkelstein's maneuver). RESULTS: US findings included tendon thickening and synovial sheath thickening with peritendinous edematous changes resulting in a peritendinous hypoecoic halo in all patients. An intertendinous septum was identified in two patients. CONCLUSION: US can confirm the suspected diag…

[Severe polyarthritis and tenosynovitis due to Streptococcus agalactiae in a patient with functional hyposplenia].

Cases of arthritis caused by Streptococcus agalactiae are infrequent and in our knowledge there are no case reports of tenosynovitis caused by S. agalactiae. A 46-year-old woman presented with fever, polyarthralgia, myalgia, diarrhea and vomiting. She had a history of papillary thyroid carcinoma and functional hyposplenia. She was febrile, with arthritis in hands, wrists, elbows, right shoulder and left ankle joints, and presented tenosynovitis in both feet and left hand. Blood and right olecranon bursa sample cultures were positive for S. agalactiae. An ultrasound scan made at the musculus ti…

Incidence and causes of tenosynovitis of the wrist extensors in long distance paddle canoeists.

OBJECTIVES: To investigate the incidence and causes of acute tenosynovitis of the forearm of long distance canoeists. METHOD: A systematic sample of canoeists competing in four canoe marathons were interviewed. The interview included questions about the presence and severity of pain in the forearm and average training distances. Features of the paddles and canoes were determined. RESULTS: An average of 23% of the competitors in each race developed this condition. The incidence was significantly higher in the dominant than the nondominant hand but was unrelated to the type of canoe and the an…

Osteoid osteoma of the radial styloid mimicking stenosing tenosynovitis. A case report.

An osteoid osteoma of the styloid process of the radius mimicking de Quervain's tenovaginitis in a 28-year-old man is reported. As the clinical presentation and X-rays of the wrist were not typical of an osteoid osteoma, only the histological examination led to the correct diagnosis.

[Tenosynovitis in chronic inflammatory joint diseases and its sequelae in the area of the hand].

The synovial lining of tendons may participate early in chronic rheumatic diseases. As a result of the inflammatory activity degenerative changes of tendons may occur. Increasing damage to tendons ends in their rupture. At 775 tenosynovectomies, 395 of them in chronic rheumatic diseases, in 68 patients (78 hands) 173 ruptures of tendons (14 flexors, 159 extensors) were found. The value of early tenosynovectomy as a preventive measure is emphasized.

Enhancing Tenosynovitis Diagnosis With XGBoost: A Machine Learning Approach Using Ultrasound Data.

BACKGROUND: Rheumatoid arthritis (RA) is a systemic autoimmune disorder characterized by chronic inflammation and progressive joint destruction. Tenosynovitis is one of the early manifestations of RA. This study aims to develop a machine learning (ML)-based model using ultrasound (US) radiomics to objectively diagnosis tenosynovitis in RA patients, thereby facilitating accurate evaluation of RA. METHODS: This study included a total of 1496 grayscale US images of the wrist extensor tendons, wrist flexor tendons, and finger flexor tendons from 152 patients with RA. Radiomic features were extrac…

Aromatase Inhibitor-Induced Carpal Tunnel Syndrome and Stenosing Tenosynovitis: A Systematic Review.

BACKGROUND: Although aromatase inhibitors are the first-line treatment in postmenopausal women with hormone receptor-positive breast cancer, there is increasing evidence that they can induce carpal tunnel syndrome and stenosing tenosynovitis. This systematic review summarizes the risk factors, incidence, and management for patients with aromatase inhibitor-induced carpal tunnel syndrome and stenosing tenosynovitis compared to tamoxifen or placebo. METHODS: A Preferred Reporting Items for Systematic Reviews and Meta-Analyses-guided systematic review of PubMed/MEDLINE, Ovid Embase, and the Coch…

Rheumatologic Perspective on Persistent Right-Hand Tenosynovitis Secondary to Mycobacterium marinum Infection.

BACKGROUND: Rheumatologic conditions share many symptoms with infectious processes. The immunosuppressive therapies used in rheumatology unfavorably impact any underlying infection. Nontuberculous mycobacteria (NTM) are difficult to grow in culture media and may affect the musculoskeletal system, developing manifestations that may imitate rheumatic inflammatory arthritis. For this reason, surgical debridement and biopsy culture are essential in cases where suspicion remains high. CASE PRESENTATION: We present the case of a patient with progressively worsening right-hand tenosynovitis who was …

Outcome of low level lasers versus ultrasonic therapy in de Quervain's tenosynovitis.

BACKGROUND: de Quervain's tenosynovitis is an inflammation of abductor pollicis longus (APL) and extensor pollicis brevis (EPB) muscle tendon sheaths at the level of radial styloid process. Its conservative management includes nonsteroidal anti-inflammatory drugs, wrist and thumb immobilization, ultrasonic therapy (US Th.) and low level laser therapy (LLLT). Literature is scanty on comparative efficacy of US Th. and LLLT for its management. This prospective study evaluates outcome of US Th. versus LLLT in de Quervain's disease. MATERIALS AND METHODS: Thirty patients clinically diagnosed de Qu…

Extracorporeal shockwaves therapy for finger stenosing tenosynovitis: a systematic review and meta-analysis.

BACKGROUND: Stenosing tenosynovitis is a common chronic tendon disease that seriously affects daily life and working ability. However, its treatment is very challenging and requires more effective treatment methods. A large number of clinical studies have shown that extracorporeal shock wave therapy (ESWT) may relieve the symptoms of stenosing tenosynovitis, but there are no published reviews or meta-analyses specifically and comprehensively evaluating its efficacy for this disease. Therefore, through conducting a meta-analysis, this study systematically evaluated the therapeutic effect of ESW…
