Cervical root resorption in two patients with unilateral complete cleft of the lip and palate.

Case reports on cervical root resorption in patients with complete cleft of the lip and palate are scarce. Moreover, cervical root resorption is sometimes diagnosed several years after a combined orthodontic and surgical therapy in patients with cleft. In this article a review of the literature is presented, and a description and the management of the cervical lesion is provided. An additional objective of this report is to advocate for alveolar bone grafting before the age of 12 years while the vulnerable cervical region of the teeth adjacent to the cleft defect is still protected by a thick

Histomorphologic changes in the gingiva and pulp of overretained primary teeth.

This study examined the influence of overretention on the tissues of human primary teeth. The range of overretention was two to thirty-two years. Light microscopy and computerized morphometry were utilized for histologic assessment of twenty-five sites of twenty-one teeth. Dentinoclasts were found on the resorbing root surface of nine teeth; polymorphonuclear leucocytes were found in the pulp of fourteen teeth; and monocytes were present in all pulps. The apical and coronal ends of the junctional epithelium were apical to the cemento-enamel junction in eighteen and fourteen teeth, respectively

Invasive cervical resorption following trauma.

Invasive cervical resorption is an insidious and often aggressively destructive form of external root resorption which may occur as a late complication following dental trauma particularly where it involves damage to cementum and supporting tissues. While this resorption may be evident clinically as a pink coronal discolouration, later with cavitation of the enamel, often there are no obvious external signs and the condition is only detected radiographically. It is characterised by the invasion of the cervical region of the root by fibrovascular tissue which progressively resorbs dentine, enam

Biomechanics of the tooth cervical region investigated using 3D digital image correlation, micro CT, and finite element analysis.

The cervical region of the tooth is a mechanically vulnerable transition zone that transmits occlusal loads from the crown to the root and constitutes a common site of clinical conditions, including dentin hypersensitivity and non-carious cervical lesions (NCCLs). However, the spatial distribution of stresses and strains induced by physiological and off-axis occlusal loading in this region, and their relationship to surface and subsurface damage, remain insufficiently characterized. Here, we investigated cervical tooth biomechanics under controlled axial and oblique loading. Ten extracted mand

Impact of partial restoration margin level in combination with connective tissue graft on the treatment of single RT1 gingival recessions associated with non-carious cervical lesions: a 1-year randomized parallel-group trial.

OBJECTIVES: To assess whether the apical margin position of partial restorations (PRs) influences outcomes of coronally advanced flap (CAF) plus connective tissue graft (CTG) in single RT1 gingival recessions with Class B+ non-carious cervical lesions (NCCLs). MATERIALS AND METHODS: Forty patients were randomized to receive PRs completed at the estimated cementoenamel junction (CEJ-0) or 1 mm apical to it (CEJ-1), placed two weeks before CAF + CTG. Clinical parameters-including recession depth (RD), mean root coverage (mRC%), keratinized tissue height, and gingival thickness-and patient-repor

Digital workflow to assess gingival recession coverage independently of the cemento-enamel Junction: a prospective clinical study using the modified coronally advanced tunnel technique with porcine dermal matrix.

OBJECTIVES: The limited number of studies using digital workflows to measure soft tissue changes depend on the cemento-enamel junction (CEJ), which has been reported to be unreliable. Our primary objective was to apply an advanced digital assessment method, measuring independent from the CEJ to evaluate the modified coronally advanced tunnel technique (MCAT) with a porcine dermal matrix (PDM) for gingival recession coverage. MATERIALS AND METHODS: Patients with type RT1 and RT2 gingival recessions were treated with the MCAT and a PDM. Plaster casts (preoperative and 6 months postoperative) we

Minimally Invasive Treatment of Class 4 Invasive Cervical Resorption with Internal Approach: A Case Series.

Invasive cervical resorption (ICR) is a type of external resorption that can involve the coronal, middle, and apical parts of the root in its advanced stages. The diagnosis and treatment of ICR depend on the extent of the resorption into the dentin. The treatment of advanced ICR is challenging, and these teeth have poor prognoses. This article describes 4 cases of class 4 ICR diagnosed by using cone-beam computed tomography and treated with a minimally invasive internal approach with sodium hypochlorite irrigation and calcium hydroxide dressing. All cases were followed for at least 3 years.

48-month comperative evaluation of a novel glass ionomer cement and a resin composite in restoring non-carious cervical lesions of patients with systemic diseases: A randomized clinical trial.

OBJECTIVE: This randomized clinical trial evaluated the 48-month clinical performance of a glass ionomer cement (GIC) and a resin composite (RC) in restoring non-carious cervical lesions (NCCLs) in patients with systemic diseases. METHODS: A single clinician placed 134 restorations in 30 patients with systemic diseases. The NCCLs were divided into two groups based on the restorative system used: a conventional restorative GIC, Fuji Bulk (GC, Tokyo Japan) [FB] and a posterior RC, G-ænial Posterior (GC, Tokyo Japan) [GP] along with a universal adhesive in etch & rinse mode. All procedures follo

Physical properties of root cementum: part 24. Root resorption of the first premolars after 4 weeks of occlusal trauma.

INTRODUCTION: In orthodontics, adding restorative materials on occlusal or lingual surfaces is a common method to create a mini-biteplane to increase patients' vertical dimension temporarily to facilitate several treatment procedures. However, this method transmits excessive occlusal forces through the periodontal ligament and causes trauma. In this prospective randomized clinical trial, we measured and compared quantitatively the volumes of root resorption after 4 weeks of occlusal trauma. METHODS: Forty-eight maxillary and mandibular first premolars of 12 patients (6 girls, 6 boys) comprise

Combined periodontal and restorative approach to the treatment of gingival recessions with noncarious cervical lesions: a case treated with acellular dermal matrix allograft and compomer restorations.

Treatment of gingival recessions has become one of the most challenging procedures in periodontal plastic surgery. Various surgical options with predictable outcomes are available, but in cases with cervical lesions or restorations, optimal functional and esthetic results may require the combination of periodontal and restorative procedures. In this case report, one patient treated with acellular dermal matrix allograft and a coronally positioned flap in combination with compomer cervical restorations is presented. Clinical parameters were recorded immediately prior to surgery and after 12 mon

An alternative hypothesis from veterinary science for the pathogenesis of noncarious cervical lesions.

An alternative hypothesis to abrasion and erosion for the pathogenesis of noncarious cervical lesions was put forward in 1984; the so-called occlusal theory suggested that tensile stresses from occlusal overload could be involved in the pathogenesis of noncarious cervical lesions and that bending stresses applied to teeth could cause disruption of the surface enamel, resulting in increased susceptibility to dissolution and abrasion at the affected sites and in the development of wedge-shaped lesions. This theory has gained increased acceptance in recent years, although absolute scientific evid

Perception and Knowledge of Dental Students and Dentists on the Management of Non-Carious Cervical Lesions: A Cross-Sectional Study.

INTRODUCTION: The combination of mechanical and chemical stress may lead to the development of non-carious cervical lesions (NCCLs), which can be challenging to manage. Professional scientific and technical mastery is essential, requiring close attention to knowledge development from the undergraduate learning process. OBJECTIVES: This study aimed to (1) assess the knowledge of undergraduate dental students and dentists about the aetiology, diagnosis, and treatment of NCCLs; (2) compare self-reported and actual knowledge scores between the groups; and (3) identify the factors influencing thes

Does operatory field isolation influence the performance of direct adhesive restorations?

PURPOSE: To evaluate the influence of different adhesive strategies (etch-and-rinse and self-etching adhesives) and type of field isolation (absolute or relative) on the clinical performance of restorations of noncervical carious lesions (NCCLs). MATERIALS AND METHODS: One hundred forty NCCLs were selected from 38 patients, according to previously established inclusion/exclusion criteria, and assigned to one of four groups (n = 35): etch-and-rinse/rubber-dam (ERR), etch-and-rinse/cotton roll (ERC), self-etching/rubber-dam (SER) and self-etching/cotton roll (SEC). The adhesive systems used wer

Mandibular molar rehabilitation using orthodontic extrusion associated with odontoplasty.

The aim of this clinical report is to describe the successful treatment of a mandibular first molar presenting an extensive fracture at the buccal aspect in a young patient. The extension of the fracture was a negative prognostic factor for tooth maintenance. An alternative clinical treatment was proposed since the patient was young and presented with good oral hygiene and periodontal health. The treatment was based on orthodontic forced eruption associated with odontoplasty. A 3-year follow-up after the surgical procedure demonstrated the maintenance of periodontal health and good plaque cont

Coronally advanced flap procedure: is the interdental papilla a prognostic factor for root coverage?

BACKGROUND: This study was designed to verify if the dimension of the interdental papilla may be a prognostic factor for the clinical outcome of the coronally advanced flap (CAF) in the treatment of gingival recessions. METHODS: Thirty-three Miller Class I recessions were treated in 33 patients using the CAF procedure. Two types of measurements were performed: 1) clinical measurements (probing depth, recession depth, width of keratinized tissue, clinical attachment level) were recorded at baseline and 3 months after surgery and 2) all recessions were photographed and transformed into computer

A histopathologic investigation on the effects of electrical stimulation on periodontal tissue regeneration in experimental bony defects in dogs.

BACKGROUND: One endpoint of periodontal therapy is to regenerate the structure lost due to periodontal disease. In the periodontium, gingival epithelium is regenerated by oral epithelium. Underlying connective tissue, periodontal ligament, bone, and cementum are derived from connective tissue. Primitive connective tissue cells may develop into osteoblasts and cementoblasts, which form bone and cementum. Several procedural advances may support these regenerations; however, the regeneration of alveolar bone does not always occur. Therefore, bone stimulating factors are a main topic for periodont

Coronally advanced flap procedure for root coverage. Treatment of root surface: root planning versus polishing.

This clinical study was designed to determine if mechanical instrumentation (root planing) of the exposed root is useful in treating gingival recession caused by traumatic toothbrushing following a coronally advanced flap (CAF). Ten patients with high levels of oral hygiene (full-mouth plaque score <20%), from 25 to 57 years of age, were selected for the study. Each patient showed 2 bilateral Class I or II maxillary recessions. A total of 20 recessions were treated. The difference in the recessions was < or =1 mm. In each patient, one recession was randomly assigned to the test group and the c

Effect of a crown ferrule on the fracture strength of endodontically treated canines restored with fiber posts and metal-ceramic or all-ceramic crowns.

The aim of this study was to comparatively evaluate the fracture strength of endodontically treated canines restored with glass-fiber posts (GFPs) and either metal-ceramic (MC) or all-ceramic (AC) crowns in the presence or absence of 2 mm of ferrule height. Fifty human maxillary canines were endodontically treated and randomly divided into five groups of 10 specimens each. The first group remained intact (control), while the remainder were restored with GFPs and composite cores with either MC or AC crowns. Each of the AC and MC groups was equally divided between teeth with or without ferrule.
