Peripheral Blood Stem Cells versus Bone Marrow for T Cell-Replete Haploidentical Transplantation with Post-Transplant Cyclophosphamide in Hodgkin Lymphoma.

Haploidentical stem cell transplantation (haplo-SCT) with post-transplant cyclophosphamide (PT-Cy) represents a potential curative strategy for patients with Hodgkin lymphoma (HL) when a matched related or unrelated donor is not available. The role of graft source, either bone marrow (BM) or peripheral blood stem cells (PBSCs), in this setting has not been fully elucidated. We performed a retrospective study on 91 patients with HL to compare the outcome after BM (n = 53) or PBSC (n = 38) transplant. Eighty-nine patients engrafted with no difference between BM and PBSCs in terms of median time

CD25-based graft-versus-host disease prophylaxis after haploidentical hematopoietic stem cell transplantation: comparison of two prophylactic strategies.

Haploidentical hematopoietic stem cell transplantation (haplo-HSCT) is widely used for hematologic malignancies, yet balancing graft-versus-host disease (GVHD) prophylaxis and immune reconstitution remains challenging. In this prospective, nonrandomized study, we compared a higher-dose anti-CD25 strategy that omitted methotrexate with a conventional lower-dose anti-CD25 regimen that included methotrexate. Lower cumulative anti-thymocyte globulin exposure was observed in the higher-dose group. From May 2022 to January 2025, 276 patients were enrolled (lower-dose group, n=149; higher-dose group,

Chronic graft-versus-host disease after haploidentical transplantation in aplastic anemia: clinical characteristics, risk factors, and survival outcomes.

To investigate the clinical characteristics, risk factors, and impacts of chronic graft-versus-host disease (cGvHD) on survival outcomes in patients with severe aplastic anemia (SAA) following haploidentical hematopoietic stem cell transplantation (haplo-HSCT), a cohort of 530 patients was analyzed. The findings revealed that 156 patients developed cGvHD, with a 5-year cumulative incidence of 29.6% (72.4% mild, 17.9% moderate, 9.6% severe). Multivariate analysis identified patient age ≥18 years (hazard ratio [HR] = 1.38, P = 0.044) and a history of grade II-IV acute GvHD (HR = 1.69, P = 0.002)

Navigating Haploidentical Stem Cell Transplantation in Congenital Amegakaryocytic Thrombocytopenia With Prior Intracranial Bleeding: Report of Two Pediatric Cases.

Congenital amegakaryocytic thrombocytopenia (CAMT) is a rare inherited bone marrow failure syndrome caused by biallelic myeloproliferative leukemia (MPL) variants, for which hematopoietic stem cell transplantation (HSCT) is the only curative therapy. We report two children with genetically confirmed CAMT and prior intracranial hemorrhage who underwent haploidentical peripheral blood HSCT with reduced intensity conditioning and post-transplant cyclophosphamide. Individualized management included immunomodulation (bortezomib for alloimmune platelet refractoriness in one child; rituximab for B-ce

Poor platelet engraftment combined with persistent pulmonary infection after haploidentical allogeneic hematopoietic stem cell transplantation: a case report.

Poor graft function (PGF) with persistent thrombocytopenia is a serious post-transplant complication linked to increased mortality. Active infections can worsen outcomes. We report a case of secondary poor platelet engraftment with refractory pulmonary infection after haploidentical HSCT. A 52-year-old woman developed Rhizopus microsporus infection on day +1. Neutrophil engraftment occurred by day +13, but platelet engraftment was delayed until day +40. She later developed Pseudomonas aeruginosa septic shock (day +106) and multidrug-resistant Klebsiella pneumoniae superinfection (day +190). Th

Hemorrhagic cystitis in pediatric severe aplastic anemia undergoing haploidentical hematopoietic stem cell transplantation: incidence, risk factors and outcomes.

BACKGROUND: Hemorrhagic cystitis (HC) is a common complication of hematopoietic stem cell transplantation (HSCT) and may adversely affect the prognosis of patients. However, the risk factors associated with HC and its influence on prognosis remain unclear in pediatric Severe aplastic anemia (SAA) patients who underwent haploidentical HSCT (haplo-HSCT). METHODS: Clinical data from 116 SAA patients who received haplo-HSCT based on the 'Beijing Protocol' at the Children's Hospital of Soochow University between 2018 and 2023 were examined retrospectively. Potential risk factors were identified by

Comparison of rabbit ATLG and ATG for GVHD prophylaxis in hematological malignancies with haploidentical hematopoietic stem cell transplantation.

Rabbit anti-human T lymphocyte globulin (ATLG) and anti-thymocyte globulin (ATG) are commonly used for graft-versus-host disease (GVHD) prophylaxis in allogeneic hematopoietic stem cell transplantation (HSCT). Yet, their efficacy and safety have seldom been compared in hematological malignancies with haploidentical HSCT. A retrospective analysis with 28 ATLG (total dosage, 20-30 mg/kg) and 18 ATG (total dosage, 8-10 mg/kg) patients were performed. The cumulative incidences of chronic GVHD and relapse were comparable between both groups. ATLG showed a trend towards a lower acute GVHD incidence

Successful Haploidentical Bone Marrow Transplantation of an Infant With a Novel Mutation in SAMD9L Gene (Ataxia-Pancytopenia Syndrome).

Data regarding the outcomes of hematopoietic stem cell transplant (HSCT) for the management of SAMD9L -associated ataxia-pancytopenia syndrome remains limited. We depict the case of a 2-month-old male with a novel mutation in the SAMD9L gene, presenting with respiratory failure, pancytopenia and severe developmental delay. He experienced graft failure 2 months after a 4/6 HLA-matched cord HSCT. At 9 months old, an unsuccessful unrelated donor search prompted a haploidentical HSCT with successful engraftment. He sustains excellent donor chimerism and has improved developmentally over 2 years po

Haploidentical Hematopoietic Stem Cell Transplantation in a 3-Year-Old Girl with Congenital Amegakaryocytic Thrombocytopenia: A Case Report.

Congenital amegakaryocytic thrombocytopenia (CAMT) is an autosomal recessive disorder characterized by severe thrombocytopenia that presents soon after birth and is usually not accompanied by specific somatic malformations [Germeshausen M, Ballmaier M. Best Pract Res Clin Haematol 2021; 34: 101286]. CAMT is more prevalent in females than males [Ballmaier M, Germeshausen M. Semin Thromb Hemost 2011; 37: 673-681; Germeshausen M, Ballmaier M. Haematologica 2021; 106: 2439-2448], in contrast to other congenital bone marrow failure syndromes. Patients with CAMT also exhibit cardiac malformations, c

Comparison of Quality of Life and Outcomes between Haploidentical and Matched Related/Unrelated Donor Allogeneic Hematopoietic Cell Transplantation.

Haploidentical (haplo) donor grafts are a well-established alternative donor source for allogeneic hematopoietic cell transplantation (HCT); however, data comparing health-realted quality of life (HRQOL) measures between haplo-HCT and HCT using other donor sources are lacking. We hypothesized that post-transplantation HRQOL might not differ between haplo-HCT and HCT with other graft sources. We conducted a single-institution retrospective analysis comparing HRQOL of haplo-HCT with matched-related donor (MRD) HCT and matched unrelated donor (MUD) HCT for hematologic diseases. We included 90 hap

Haploidentical Stem Cell Transplantation After TCR-αβ+ and CD19+ Cells Depletion In Children With Congenital Non-Malignant Disease.

Haploidentical hematopoietic stem cell transplantation (haplo-HSCT) represents a valuable alternative for children with nonmalignant disease and ex vivo negative selection of TCR-αβ+ cells is an emerging graft manipulation option that carries several potential advantages in terms of reduced risk of graft-versus-host disease (GvHD) and improved immune reconstitution. We report all consecutive patients with a diagnosis of nonmalignant disease who received a TCR-αβ+ and CD19+depleted haplo-HSCT at "IRCCS Istituto Giannina Gaslini" from 2013 to 2019; the conditioning regimen was myeloablative or n

Shortened-Duration Immunosuppressive Therapy after Nonmyeloablative, Related HLA-Haploidentical or Unrelated Peripheral Blood Grafts and Post-Transplantation Cyclophosphamide.

With post-transplantation cyclophosphamide (PTCy) as graft-versus-host disease (GVHD) prophylaxis, nonmyeloablative (NMA) HLA-haploidentical (haplo) and HLA-matched blood or marrow transplantation (BMT) have comparable outcomes. Previous reports have shown that discontinuation of immunosuppression (IS) as early as day 60 after infusion of a bone marrow (BM) haplo allograft with PTCy is feasible. There are certain diseases in which peripheral blood (PB) may be favored over BM, but given the higher rates of GVHD with PB, excessive GVHD is of increased concern. We report a completed, prospective

FLT3 internal tandem duplication does not impact prognosis after haploidentical allogeneic hematopoietic stem cell transplantation in AML patients.

Acute myelogenous leukemia (AML) patients with fetal liver tyrosine kinase 3 (FLT3) internal tandem duplications (ITDs) have poor prognoses if treated with chemotherapy only, primarily as they experience increased relapse rates. To determine whether this alteration also affects outcomes after haploidentical donor (HID) allogeneic hematopoietic stem cell transplantation (allo-HSCT), we compared 334 consecutive FLT3-ITD-positive vs -negative patients with AML (other than acute promyelocytic leukemia) who underwent HID-HSCT. FLT3-ITD was detected in 39 of 334 patients (11.7%). The 2-year relapse

Complete response following toxic epidermal necrolysis in relapsed adult T cell leukemia/lymphoma after haploidentical stem cell transplantation.

Allogeneic hematopoietic cell transplantation (allo-HSCT) is considered the curative treatment option in patients with aggressive adult T cell leukemia/lymphoma (ATLL), but the treatment of relapse after allo-HSCT remains a major challenge. We report a case of ATLL that was treated with sequential mogamulizumab (MOG) and lenalidomide (LEN) for early relapse after allo-HSCT. A 73-year-old Japanese male with acute-type ATLL underwent haploidentical-HSCT with post-transplant cyclophosphamide. He attained a complete response. However, ATLL relapse was diagnosed by biopsy of skin lesions that appea

Haploidentical allogeneic hematopoietic stem cell transplantation increases the risk of cytomegalovirus infection in adult patients with acute leukemia.

BACKGROUND: Owing to the shortage of hematopoietic stem cells from matched sibling donors (MSD) and matched unrelated donors (MUD), the number of patients undergoing haploidentical allogeneic hematopoietic stem cell transplantation (allo-HSCT) has rapidly increased. Despite a comparable overall survival (OS) and leukemia-free survival using this approach, some evidence suggests that haploidentical allo-HSCT recipients have a higher incidence of cytomegalovirus (CMV) infection, though this has not been clearly established. METHODS: This study retrospectively compared the cumulative incidence o
