OBJECTIVE: We present long-term results for the Pyrocardan®, a pyrocarbon trapeziometacarpal interposition implant used for the treatment of osteoarthritis of the thumb.
METHODS: Between March 2009 and May 2013, 199 arthroplasties were performed in a single hand center, in 184 patients by 6 hand surgeons. 107 implants were followed up for a mean 137 months (range, 120-168 months). 82 patients (89 implants) were lost to follow-up.
RESULTS: Median age was 57 years, with 15 patients (14.2%) aged ≤50 years. At last follow-up, functional scores were significantly improved compared to preoperative

The Elektra trapeziometacarpal prosthesis is a cementless, unconstrained prosthesis made by Fixano with titanium and chrome-cobalt steel. This paper examines the results of the first 100 prostheses used by the author for one indication only, viz. painful trapeziometacarpal osteoarthritis. The average follow-up period was 54 (range 36-78) months. Studies of pain, mobility and grip and pinch strength were carried out, the results of which were good in 83 cases. Seven dislocations occurred. Five of these were following severe trauma. The commonest complication was a lack of osteointegration of th

The operations most frequently performed for chronic polyarthritis are synovectomy and artificial joint replacement. The radioisotope synovectomy has in general reduced the need for operative synovectomy. Multicenter studies of the late results of operative synovectomy (over 10 years after the operation) show a lasting improvement in pain, although radiographically the destructive changes that are seen 10 years after the operation are similar to the non-operated side. The synovectomy can therefore buy important time since re-operation, e.g. arthroplasty, is performed mainly on clinical grounds

PURPOSE: Pyrolytic carbon implants have been successfully used in the treatment of osteoarthritis of the metacarpophalangeal and proximal interphalangeal joints. Recently, pyrolytic carbon hemiarthroplasties have been proposed for the treatment of osteoarthritis of the trapezial-metacarpal (TM) joint of the thumb. We wished to review our short-term outcomes for this device in the treatment of TM arthritis.
METHODS: Fifty-four arthritic TM joints in 49 patients, with a mean age of 59 years, were treated with use of a pyrolytic carbon hemiarthroplasty procedure. Underlying diagnoses included os

PURPOSE: To investigate whether volar percutaneous screw fixation of scaphoid waist fractures via a transtrapezial approach causes degenerative changes at the scaphotrapezial (ST) joint at short- to medium-term follow-up.
METHODS: A total of 34 patients were available for follow-up at a mean of 6.1 years (minimum follow-up, 3.7 y) after volar percutaneous fixation of acute scaphoid waist fractures via a transtrapezial approach. The clinical follow-up examination included assessment of pain using a visual analog scale, range of motion, grip strength, and key pinch strength. We obtained radiogr

Osteoarthritis of the carpometacarpal joint of the thumb is a common disease, mostly affecting middle aged women. This article presents the results of a trapeziectomy with a ligament recontruction/tendon interposition procedure. We evaluated 60 male patients with 76 operated thumbs. The mean follow- up was 62 months (ranging from 13 to 133 months, with SD of 33 months). The outcome was measured with the disabilities of the arm, shoulder and hand score, a Nelson Hospital score, a Visual analogue scale score and range of motion. The preoperative height of the trapezium was measured and compared

BACKGROUND: The kinematics of the wrist have been widely studied in terms of flexion/extension, radial/ulnar deviation, and the dart-throwing motion (DTM); however, the opposite dart-throwing motion (oDTM)-despite its functional importance in daily activities-remains poorly characterized. This study aimed to quantify in vivo kinematics of the radiocarpal and midcarpal joints and to characterize their motion patterns during oDTM.
METHODS: In vivo kinematics of eleven wrists from healthy subjects were assessed using four-dimensional computed tomography (4D CT). A custom-designed device guided w

INTRODUCTION: Trapezometacarpal instability with trapezial dysplasia is an incapacitating condition long before radiological changes appear. Most of the patients are young and demanding. Treatment is often conservative. Surgical treatment options are relatively classic, starting with a ligamentoplasty of the base of the first metacarpal or a tenotomy of the transarticular accessory slip of the abductor pollicis longus. Other options such as arthrodesis and arthroplasty have not been so successful in this situation.
METHOD: The idea arose to combine two techniques described previously. In 1973

CONTEXT: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy. When mild to moderate in severity, nonoperative treatments including osteopathic manipulative treatment (OMT) have been found to be effective. Studies have been carried out to quantify the mechanism of such treatments with cadaver studies, magnetic resonance imaging (MRI), and ultrasound.
OBJECTIVES: This pilot project investigated the mechanism of a previously undescribed technique of nonoperative carpal tunnel treatment, dorsal carpal arch muscle energy (DCA-ME), which focuses on the dorsal arch (trapezium, trap

PURPOSE: Trapeziometacarpal osteoarthritis (TMC-OA) is a prevalent hand disorder affecting a growing number of people worldwide. While a multidisciplinary approach might provide additional advantages, the analgesic and anti-inflammatory role of intra-articular oxygen-ozone (O2O3) injections combined with physical therapy is still unknown. To assess the impact of a multimodal therapeutic approach combining O2O3 injections with physical therapy in patients with TMC-OA.

Various systematic reviews have recently shown that intra-articular platelet-rich plasma (IA-PRP) can lead to symptomatic relief of knee osteoarthritis for up to 12 months. There exist limited data on its use in small joints, such as the trapeziometacarpal joint (TMJ) or carpometacarpal joint (CMCJ) of the thumb. A prospective, randomized, blind, controlled, clinical trial of 33 patients with clinical and radiographic osteoarthritis of the TMJ (grades: I-III according to the Eaton and Littler classification) was conducted. Group A patients (16 patients) received 2 ultrasound-guided IA-PRP inje

BACKGROUND: Electrophysiologic studies have been considered the "gold standard" in diagnosing carpal tunnel syndrome (CTS); however, reports of false-negative results, as well as discomfort for the patient during the procedure has paved the use of ultrasound, being a painless and cost-efficient tool, as an alternative means for its diagnosis. Various ultrasound parameters assessing the median nerve and wrist dimensions have been described, but description of landmarks to assess these in a reliable manner has been lacking.
METHODOLOGY: A systematic search of different databases yielded data re

OBJECTIVE: Trapeziometacarpal osteoarthritis is the second most common degenerative articular disease. Although initial therapy should be conservative, surgical treatment is often required. Several surgical techniques have been described, but none has proved to be a gold-standard. The objective of this study was to evaluate the long-term clinical and radiological results of trapeziometacarpal interposition arthroplasty with the PyroDisk implant (Integra LifeSciences).
METHODS: A retrospective long-term study of all patients who underwent trapeziometacarpal interposition arthroplasty with a py

PURPOSE: The goal of this study is to describe a new radial portal for improving the view of the carpometacarpal (CMC) joint of the thumb without increasing the risk of neurovascular or ligament injuries. A further goal is to define the anatomic relationships of the portals to extra- and intra-articular structures.
TYPE OF STUDY: Cadaver study.
METHODS: Six fresh-frozen adult cadaver hands were studied. The ulnar portal (UP) was marked just ulnar to the extensor pollicis brevis (EPB) tendon at the level of the joint. The new radial portal (RP) was marked just distal to the oblique ridge of t

We retrospectively compared the short-term outcomes of 18 thumbs that had a trapeziectomy and 18 that had a pyrocarbon interposition implant (Pi2) arthroplasty in 33 patients. We measured the Disability of the Arm, Shoulder, and Hand (DASH) and Short Form 36 (SF-36) scores at a mean of 20 months. Pain severity was assessed using a visual analogue scale (VAS), and level of patient satisfaction was assessed using a 5-point scale. The mean DASH scores at follow up were 27 for those that had a trapeziectomy and 35 for those that had a Pi2 arthroplasty (p = 0.001). There was no difference in the VA

INTRODUCTION: Trapeziometacarpal osteoarthritis (TMO) is among the most prevalent forms of upper extremity osteoarthritis. It is frequently associated with significant levels of pain and disability, particularly among ageing women. The typical care pathway for TMO relies on non-surgical approaches for up to 2 years, before surgery is considered. One of the most common non-surgical approaches is an intra-articular cortisone injection. However, these are not universally recommended due to their safety profile and unclear efficacy compared with saline injections. Recent evidence suggests that sal
