Norepinephrine inhibition of mesenchymal stem cell and chondrogenic progenitor cell chondrogenesis and acceleration of chondrogenic hypertrophy.

OBJECTIVE: Mesenchymal progenitor cell chondrogenesis is the biologic platform for the generation or regeneration of cartilage, but the external influence of the sympathetic nervous system on this process is not yet known. Sympathetic nerve fibers are present in articular tissue, and the sympathetic nervous system influences the musculoskeletal system by, for example, increasing osteoclastogenesis. This study was initiated to explore the role of the sympathetic neurotransmitter norepinephrine (NE) in mesenchymal stem cell (MSC)-dependent and cartilage progenitor cell (CPC)-dependent chondrogen

[Impact of surgery and trauma in developing chronic pain].

Surgery and trauma are recognized as important causes of chronic pain. In this manuscript, we analyzed the patients who visited our clinic with posttraumatic or delayed surgical pain complaints between January 2001 and January 2004. Identification and evaluation of pain were done as defined in the taxonomy of IASP. Duration and severity of pain as well as diagnosis and etiology were evaluated as suggested in the taxonomy. Our results showed that 107 out of 2866 patients (4 %) were diagnosed of having chronic pain related to the history of trauma or surgery. The age range of the patients was fr

Pathobiology of neuropathic pain.

This review deals with physiological and biological mechanisms of neuropathic pain, that is, pain induced by injury or disease of the nervous system. Animal models of neuropathic pain mostly use injury to a peripheral nerve, therefore, our focus is on results from nerve injury models. To make sure that the nerve injury models are related to pain, the behavior was assessed of animals following nerve injury, i.e. partial/total nerve transection/ligation or chronic nerve constriction. The following behaviors observed in such animals are considered to indicate pain: (a) autotomy, i.e. self-attack,

Subclavian artery dissection and triple infarction of the nervous system.

BACKGROUND: Subclavian artery dissection is a rare entity. It is usually associated with anomalous aortic vasculature. Only with trauma or catheterization procedures is subclavian artery dissection with normal aortic vasculature reported. PATIENT: We describe a patient with intrascapular pain, an occipital headache, and 3 distinct infarctions in the nervous system. He had spontaneous subclavian artery dissection with normal aortic vasculature. CONCLUSION: Subclavian artery dissections should be suspected in patients with intrascapular pain, occipital or cervical pain, and symptoms within the

Practical approach to the loss of smell.

Loss of the sense of smell can be easily confirmed in any physician's office by having the patient try to identify various odors. The etiology of anosmia can be extremely varied, including nasopharyngeal disorders such as rhinitis and tumors; neurologic conditions such as head trauma, neoplasms, vascular lesions and infections of the central nervous system; viral infections; familial and congenital disorders; drugs; industrial exposure; endocrine diseases, and several other disorders. The prognosis of anosmia is guarded, and its treatment depends on the etiology.

[Physical and neurological state of the newborn after perinatal asphyxia].

Asphyxia neonatorum is the most prevalent birth trauma. Usually it leads to the birth of weak and nonvigorous child. Perinatal asphyxia is the reason of not only postasphyxic hypoxic-ischemic encephalopathy, but also multiple organ dysfunctions. Neurological sequelae and consequences of multisystem dysfunction affect on newborn status in first life months and later on by the muscle tone disturbances, physical and mental retardations. In this paper it is presented modern conceptions about asphyxia origin, risk factors, neurological and systemic complications for child nervous system and organis

Lifetime prevalence of injuries in incoming division I collegiate football players.

OBJECTIVES: The purpose of this study is to determine the lifetime prevalence of past injuries in incoming first year football players in a Division 1 college football team. METHODS: Pre-participation questionnaires from 605 first-year football players over 20 years (1996-2015) were examined to determine the prevalence of concussions, stingers, fractures, and musculoskeletal surgeries sustained before playing at the collegiate level. Players were grouped by position: wide receiver and defensive back (WR/DB), offensive and defensive linemen (OL/DL), all other positions (OP), and unknown (UKN).

Management of the pulseless pediatric supracondylar humeral fracture.

A pediatric supracondylar humeral fracture with a pulseless, poorly perfused hand requires emergency operative reduction. If the limb remains pulseless and poorly perfused after fracture fixation, vascular exploration and possible reconstruction is necessary. A pediatric supracondylar humeral fracture with a pulseless, well-perfused hand should be treated urgently with operative fixation of the fracture and subsequent reassessment of the vascular status. Controversy exists regarding the optimal management of pediatric supracondylar humeral fractures with a pulseless, well-perfused hand followi

Current Progress on Neuroinflammation-mediated Postoperative Cognitive Dysfunction: An Update.

Postoperative cognitive dysfunction (POCD) is a common complication of the central nervous system (CNS) in elderly patients after surgery, showing cognitive changes such as decreased learning and memory ability, impaired concentration, and even personality changes and decreased social behavior ability in severe cases. POCD may appear days or weeks after surgery and persist or even evolve into Alzheimer's disease (AD), exerting a significant impact on patients' health. There are many risk factors for the occurrence of POCD, including age, surgical trauma, anesthesia, neurological diseases, etc.

Post-traumatic stress disorder and cardiometabolic dysfunction: Molecular mechanisms and therapeutic targets.

Post-traumatic stress disorder (PTSD) is a complex psychiatric condition affecting approximately 6.1% of the US population and is characterized by intrusive memories, trauma avoidance, emotional dysregulation, and hyperarousal. The disorder is associated with significant neurobiological changes, including structural and functional alterations in key brain regions such as the amygdala, hippocampus, corpus callosum, prefrontal cortex, and premotor cortex. Neurologically, PTSD is marked by heightened amygdala activity, reduced ventromedial prefrontal cortex activation, and significant cognitive i

Neuroinflammation in the rat--CNS cells and their role in the regulation of immune reactions.

Recent discoveries suggest that the resident cells of the central nervous system (CNS) the nerve cells and glia, play a more immunologically active role than was previously assumed. Neuroglial communication is of central interest in virtually all types of pathological conditions that affect the brain and several features of the activation that results from nerve cell damage resemble the type of innate immune reactions that occur in other parts of the body In particular, the characteristics of the activation of these CNS cells will affect both the interaction with cells of the immune system as

Heterotopic Ossification Leading to Ureteral Obstruction Resulting in Nephrectomy.

Background: Heterotopic ossification (HO) is the presence of bone in soft tissue where bone normally does not exist. This can be acquired or inherited with the acquired form most often seen with either trauma, spinal cord injury, or central nervous system injury. HO most commonly affects the flexors and abductors of the hip, medial knees, and the shoulders and rarely affects the genitourinary (GU) system. Case Presentation: We discuss a 67-year-old Caucasian male patient who presented with left-sided ureteral obstruction. He was involved in an airplane accident in 2001 resulting in a spinal co

Neglected spinal cord, brain stem and musculoskeletal injuries stemming from birth trauma.

OBJECTIVE: A review of the medical literature was undertaken to determine cause, diagnosis, prognosis, treatment and prevention of injuries resulting from birth trauma. The primary focus was the neonate, though infant, child and adult were also considered because the effects of birth trauma can be life-long. DATA SOURCE: A compilation of case studies and review articles were extracted from numerous "MEDLINE" literature searches. Key Terms included: Birth Trauma, Central Nervous System Injuries, Musculoskeletal Injuries, Stillbirth, Sudden Infant Death Syndrome (SIDS), Cerebral Palsy, Brachial

Penetrating neck injury: Collaterals for another life after ligation of common carotid artery and subclavian artery.

Neck, being not protected by skeleton, is vulnerable to external trauma and injury which involves blood vessels, trachea, esophagus and other endocrine and nervous system organs. Vascular injuries can not only cause potentially life-threatening hemorrhage but also need profound surgical expertise in management. Development of collateral circulation in neck is well known; however, there is scarcity of literature on the role of collateral formation in neck trauma. Here, we present a unique case of penetrating gunshot injury to neck with right common carotid and right subclavian artery injury wit

The Effects of Early Life Pain and Juvenile Fear Conditioning on CRF-Receptor Expression in the Amygdala and Hypothalamus.

Early life pain and stress have lasting consequences on nervous system development that can interact with later stress or trauma to create a susceptibility to fear, anxiety, depression and chronic pain among other psychological disorders. Recent work has identified changes in corticotropin releasing factor signaling in limbic system structures, such as the amygdala and hypothalamus, as a key mechanism behind these changes - albeit in a sex-dependent manner. CRF has two major receptors, CRHR1 and CRHR2 which have also been shown to play key roles in fear and pain expression. The current work ex

American Spinal Injury Association (ASIA) Impairment Scale Motor Grade Outcomes Among Patients With Ballistic Neurologic Injuries Managed Expectantly: A 13-Year Single Institution Retrospective Review and Narrative Review of the Literature.

Background  Gunshot wounds (GSW) are increasingly prevalent in the United States. There is a paucity of data and guidelines governing surgical versus non-surgical management of neurologic GSW. One challenge lies in characterizing and stratifying the natural history of GSW to the nervous system (i.e., neurologic injuries) with respect to outcomes. Our group hypothesized that nonsurgical management would show promise and that rates of recovery would differ based on the injured area. Methods  We reviewed ballistic nervous system injuries involving the spine and extremities over a 13-year period a

Vertebral artery dissection after a chiropractor neck manipulation.

The differential diagnosis for ischemic central nervous system infarcts in young patients includes paradoxic emboli through cardiac shunts, vasculitis, and vascular trauma. We report a young woman who developed headache, vomiting, diplopia, dizziness, and ataxia following neck manipulation by her chiropractor. A computed tomography scan of the head revealed an infarct in the inferior half of the left cerebellar hemisphere and compression of the fourth ventricle causing moderate acute obstructive hydrocephalus. Magnetic resonance angiography revealed severe narrowing and low flow in the intracr

Ethanol and cognition: indirect effects, neurotoxicity and neuroprotection: a review.

Ethanol affects cognition in a number of ways. Indirect effects include intoxication, withdrawal, brain trauma, central nervous system infection, hypoglycemia, hepatic failure, and Marchiafava-Bignami disease. Nutritional deficiency can cause pellagra and Wernicke-Korsakoff disorder. Additionally, ethanol is a direct neurotoxin and in sufficient dosage can cause lasting dementia. However, ethanol also has neuroprotectant properties and in low-to-moderate dosage reduces the risk of dementia, including Alzheimer type. In fetuses ethanol is teratogenic, and whether there exists a safe dose during
