Primary repairing in penetrating colon injuries.

BACKGROUND/AIMS: The large bowel is injured in about 10% of penetrated abdominal injuries. Primary repair with colostomy has been discussed recently. MATERIALS AND METHODS: Between 1987 and 1995, sixty-five patients were operated on with penetrated colon injuries were treated with primary repair without colostomy. Any patient with one or more of the following criteria was excluded: Age > 60 years, blood pressure < 80/60 mmHg, time from injury to treatment > 8 hours, Penetrating abdominal trauma index > 50. RESULTS: The mean age of the patients was 31 and female/male ratio was 1/7. Fifty-one

Blowout fractures of the orbital floor.

Six patients with blowout fractures of the orbital floor, not associated with infraorbital rim fractures, are presented. Diplopia, infraorbital anesthesia and enophthalmos following blunt trauma to the eye should alert the examiner to the possibility of a blowout fracture with incarceration of extraocular tissues. Early roentgenographic examinations are often difficult to evaluate because of periorbital edema, mucosal swelling and antral hematoma. In cases in which stereoscopic Waters projections are equivocal, laminograms of the orbit, after the edema subsides, will aid in confirming the diag

A comparative evaluation of the prognostic performance of MGAP and GAP trauma scores in predicting clinical outcomes in multiple trauma patients: protocol for a prospective cohort study from Baghdad, Iraq.

INTRODUCTION: Trauma remains a significant contributor to global morbidity and mortality, with low- and middle-income countries being particularly affected due to limited resources and delayed medical care. Although trauma scoring systems such as MGAP (Mechanism, Glasgow Coma Scale, Age, Arterial Pressure) and GAP (Glasgow Coma Scale, Age, Systolic Blood Pressure) can be used for rapid risk assessment, they have never been validated for use in conflict-affected regions like Iraq. This study assesses the prognostic accuracy of MGAP and GAP in predicting in-hospital mortality, intensive care uni

Short term outcomes of children with abusive head trauma two years post injury: A retrospective study.

PURPOSE: Abusive head trauma (AHT) can have debilitating sequelae for children who survive. A retrospective medical record review was used to describe short-term developmental outcomes of children with AHT and identify predictors of poorer outcomes. METHOD: Children with AHT who received follow up by the hospital's rehabilitation department for 12 to 24 months post-injury were included in this review. Data for 85 children were collected on hearing, vision, gross motor, fine motor, speech and language, cognition, play, adaptive functioning, behaviour and personal-social skills. RESULTS: Globa

Road accidents involving bicycles: configurations and injuries.

This study analyzed the most common types of accident involving bicycles and compared the frequency of injuries. The data source was the database of German In-Depth Accident Study (GIDAS). Cases consist of bicycles and their riders involved in accidents between 2000 and 2010. In most collisions, the bicycle impacted with a car. The percentage of injured bicyclists was higher in collisions with a heavy vehicle and decreased when the bicycle impacted with lighter vehicles. A high percentage of injured bicyclists in single accidents was observed; the most severe injury was more frequently to head

Impact of Cushing's sign in the prehospital setting on predicting the need for immediate neurosurgical intervention in trauma patients: a nationwide retrospective observational study.

BACKGROUND: Cushing's reflex usually results from intracranial hypertension. Although Cushing's sign can implicate severe traumatic brain injury (TBI) in injured patients, no major investigations have been made. The purpose of this study was to assess the predictability of life-threatening brain injury requiring immediate neurosurgical intervention (LT-BI) among trauma patients with Cushing's sign in the prehospital setting. METHODS: This was a retrospective study using data from the Japan Trauma Data Bank from the period of 2010 to 2014. Patients 16 years old or older with blunt mechanisms o

Injury in professional Irish dancers.

Despite the evolution of Irish dance to professional status in recent decades, only scant investigation of musculoskeletal pain and injury among professional Irish dancers (PIDs) has been undertaken. This study investigated the rate of injury and associated factors among 178 PIDs, using an online questionnaire. One hundred and thirty-seven PIDs (76.7%) reported a previous injury, with the foot (67.9%) and ankle (60.6%) most commonly affected. A mean career rate of 2.25 injuries per dancer was computed. The majority of injuries were minor in nature, and almost half occurred midway through a tou

Association of interpersonal violence with self-reported history of head injury.

OBJECTIVE: The purpose of this study was to examine differences in interpersonal violence among individuals who reported a head injury compared with those who did not report a head injury. METHODS: We used data from an 8-year longitudinal study of youth selected by their grade point average to study those at-risk for high school dropout in 4 public high schools in a Midwestern city (N = 850). Participants were followed up from mid-adolescence to the transition into young adulthood. One-way analyses of variance were used to test for differences in levels of interpersonal violence, and repeated

Effects of additional intra-abdominal organ injuries in patients with penetrating small bowel trauma on morbidity and mortality.

BACKGROUND: We aimed to evaluate the effects on morbidity and mortality of additional organ injuries obtained concurrently with penetrating small bowel injuries. METHODS: Between January 2000 and March 2005, patients in whom penetrating small bowel injuries occurred as a result of abdominal injuries were allocated into two groups and assessed. The first group included patients with isolated small bowel injuries, while those in the second group had small bowel injuries together with other intra-abdominal organ injuries. RESULTS: Small bowel injury was identified in 38 patients (34 M, 4 F) wit

[Early fibrobronchoscopy and the microbial flora composition of bronchoalveolar lavage fluid in patients with concomitant injury during treatment in intensive care units].

Resuscitative bronchofibroscopy in victims with severe injury and aspiration has been shown to be both a method for diagnosing damages to the bronchus and lung and that for recovering airway patency in them. The most common involvement (47% of cases) in chest injury has been noted to be the lower lung with the development of pyoinflammatory processes caused by etiologically significant microorganisms, such as Staphylococcus aureus and types of Enterobacteriacea, in most cases (more than 50%).

The epidemiology of posttraumatic adult respiratory distress syndrome.

BACKGROUND: Although adult respiratory distress syndrome is an important early complication of blunt trauma, the epidemiology and risk factors for its development remain poorly defined. The aims of this study were to determine the prevalence and demographics of this complication in a prospective cohort series of patients admitted to the hospital following injury. We also assessed the contribution of the severity and pattern of the injury to the risk of this complication developing. By identifying factors associated with the highest risk of the development of adult respiratory distress syndrome

Dynamic autoregulatory response after severe head injury.

OBJECT: The purpose of this study was to evaluate the extent and timing of impairment of cerebral pressure autoregulation after severe head injury. METHODS: In a prospective study of 122 patients with severe head trauma (median Glasgow Coma Scale Score 6), dynamic tests of pressure autoregulation were performed every 12 hours during the first 5 days postinjury and daily during the next 5 days. The autoregulatory index ([ARI] normal value 5 +/- 1.1) was calculated for each test. The changes in the ARI over time were examined and compared with other physiological variables. The ARI averaged 2.8

[Management of combined thoracoabdominal wounds].

Last years in the country gace rise to the problem of treatment of fire-arm's wounds in civil population. The results of treatment of victims with firearm thoracoabdominal wounds have evidenced unprepareness of surgeons for rendering specialized aid in conditions of "military-urban" surgery. The study performed by the authors has shown that there are substantial and principal differences between many postulates of war-field surgery and "military-urban" surgery. It consists in early hospitalization of victims with combined thoraco-abdominal injuries, prevalence of bullet wounds over fire-mine e
