This finding is helpful in counseling patients who had conceived triplets in terms of preterm delivery, mode of delivery, and the need for embryo reduction at earlier stages of pregnancy, as well as performing a prophylactic cervical cerclage.

This report describes an acardiac fetus of the acormus phenotype in a triplet pregnancy. The diagnosis was confirmed at 15 weeks. In the absence of signs of heart failure in the co-fetus the pregnancy was managed conservatively. The pregnancy was complicated by preterm labour and the fetuses were delivered at 26+5 weeks. The prenatal diagnosis of the acormus phenotype with a well-developed cephalic pole is extremely rare and has never been described antenatally in a higher order multiple pregnancy. We report a case in which a patient received IVF treatment due to massive pelvic endometriosis. The tubal patency and sperm parameters were normal. After e-SET (elective single embryo transfer) ultrasonography showed a triplet pregnancy with thick dividing membranes and lambda sign between all sacs. Two monozygotic and one dizygotic girl were delivered by Caesarean section at 28 weeks gestation. The couple had intercourse 2 days prior to the oocyte pick up (OPU) and the triplet pregnancy was attributed to a combination of assisted and spontaneous conception.

OBJECTIVE: To describe the first reported case of a triplet pregnancy subsequent to the use of an aromatase inhibitor for ovulation induction.
DESIGN: Case report.
SETTING: Academic medical center.
PATIENT(S): A 32-year-old female with polycystic ovary syndrome and primary infertility.
INTERVENTION(S): Letrozole, 5 mg/day, on cycle days 2 to 6 for ovulation induction.
MAIN OUTCOME MEASURE(S): Clinical pregnancy.
RESULT(S): Viable triplet gestation.
CONCLUSION(S): Use of an aromatase inhibitor for ovulation induction does not eliminate the risk for higher order multiple pregnancy.

We report two cases of fetofetal transfusion in monochorionic triamniotic triplet pregnancies. Case 1: At 23 weeks' gestation an amnioreduction was carried out. Three days later, the donor triplet died in utero. Immediately after that, a cardiotocogram of the triplet who had been thought to be unaffected by the fetofetal transfusion, showed a non-reassuring fetal status. This is a case study of malformed conjoined twins in a spontaneous triplet pregnancy. A 20-year-old primigravida spontaneously delivered at 22 weeks gestation a normal 425-gram male newborn followed by a delivery of a 525 grams pygopagus conjoined male twins. One twin had omphalocele, missing ears, cleft palate and dysmorphic figures while the other had imperforated anus and dysmorphic figures. The Apgar scores were 2/0. Ectopic implantation with in-vitro fertilization-embryo transfer may occur in the cornu or tubal stump, which is otherwise rare. Our patient with previous left salpingostomy and right salpingo-oophorectomy had 4 embryos transferred through in vitro fertilization out of which 3 were successfully implanted with twin intrauterine gestation and cornual pregnancy. The cornual pregnancy ruptured at 12 weeks of gestation and the twin intrauterine pregnancy had a successful outcome.

A young primigravida presented at 36 weeks of gestation with complete heart block and triplet pregnancy. She underwent a lower segment cesarean section, and was managed successfully. The patient remained asymptomatic and did not require antiarrhythmic drugs or pacing.

The occurrence of a triplet pregnancy discordant for anencephaly is rare and its management presents a clinical dilemma. We report what appears to be the first case of a triplet pregnancy with two anencephalic fetuses complicated by premature contractions and severe polyhydramnios. Its management, which results a healthy newborn weighing 1385 g is discussed.

OBJECTIVE: Our goal was to compare the lengths of hospitalization and the perinatal outcomes of triplet pregnancies managed with either outpatient or inpatient third-trimester bed rest.
STUDY DESIGN: Thirty-two triplet pregnancies in which outpatient bed rest was prescribed (April 1993 to April 1996) were compared with a historic cohort of 34 triplets (January 1985 to March 1993) in which routine hospitalization was undertaken in the third trimester. We report a case of feto-feto-fetal-transfusion-syndrome (FFFTS) in a spontaneous monochorionic triamniotic triplet pregnancy primarily diagnosed at 17 weeks of gestation. During the course of pregnancy, sequentially two triplets appeared as donor. Symptoms of a recipient (polyhydramnios, tricuspid valve insufficiency, and ascites) were present in the third triplet. Hyperreactio luteinalis is a non-neoplastic tumor-like ovarian lesion associated with pregnancy. Most patients are asymptomatic, with the ovarian enlargement being incidentally discovered at the time of cesarean section. It can simulate a neoplasm on clinical, gross and sometimes microscopic examination. We report a case of hyperreactio luteinalis in a patient, who was diagnosed as having polycystic ovary disease before conceiving a triplet pregnancy after three treatment cycles of human menopausal gonadotropin-human chorionic gonadotropin therapy, and discuss its pathogenesis.

OBJECTIVE: To compare maternal and perinatal complications in triplet and twin pregnancies.
STUDY DESIGN: Case-controlled study in the setting of a University Hospital. Each pregnancy of a consecutive series of 40 triplet pregnancies of 20 weeks or more was matched for parity and maternal age with two sets of twins delivered in the same year. Primary end points of the analysis were maternal complications and perinatal outcome.
