Tuberculous pleurisy, a type of extra-pulmonary tuberculosis, is the pleural inflammation caused by hypersensitive response of the pleural cavity stimulated by mycobacterium tuberculosis and its metabolites. If the patient does not receive timely and effective treatment and intervention, a large amount of fibrin will be deposited which will cause pleural thickening, adhesions, and even calcification. At present, clinical cases of typical pleural thickening and calcification have been rarely reported. In this paper, we will report a case of significant pleural thickening and calcification cause

The Gen-Probe Amplified Mycobacterium Tuberculosis Direct Test (MTD) has been widely used as a rapid test for the identification of Mycobacterium tuberculosis complex in clinical samples, and several research groups have verified its clinical usefulness. However, most of the specimens they tested were sputum, and there have been few reports on other specimens. In particular, there have been no reports on assessments of methods of preparing samples other than sputum for the MTD. We assessed methods of preparing samples other than sputum and the influence of a local anesthetic and an anticoagula

The value of determination of pleural fluid glucose, pH, lactic dehydrogenase, IgG, IgA, IgM, C3, C4, anti-IgG antibody, and hydroxyproline in distinguishing between pleural effusions caused by rheumatoid arthritis (RA) and those resulting from other diseases was studied. The series comprised seven patients with RA and 115 patients with other diseases including systemic lupus erythematosus, tuberculosis, malignant disease, empyema, pneumonia, congestive heart failure, and nonspecific pleural effusion. The low glucose concentration, the low pH and the low C4 level in rheumatoid pleural effusion

The most efficient and cost-effective approach to the diagnosis of pleural exudates remains controversial. Important considerations include the respective diagnostic yields of thoracocentesis, closed pleural biopsy and thoracoscopy; the incremental gain in diagnostic yield when sequentially combining these investigations; and the role of various image modalities. The diagnostic yield of thoracocentesis is in the order of 60% for malignancy and >90% for tuberculosis. A second aspiration may increase the yield for malignancy, but a third is generally superfluous. Many authorities consider thorac

BACKGROUND & OBJECTIVE: CA-125, an ovarian tumor marker is known to increase in non malignant conditions such as tubercular and non tubercular pleuritis and ascites. We undertook this study to evaluate non-specific rise in CA-125 levels in conditions associated with pleural effusion and ascites and also to understand the mechanism of its secretion.
METHODS: CA-125 levels in 38 pleural and 46 ascitic fluid samples from non malignant cases and 10 blood samples from pulmonary tuberculosis cases were estimated by ELISA. The ascitic fluid samples were collected from cases of bacterial peritonitis,

BACKGROUND: Pleural effusion (PE) is a common problem in medical practice. Transudative effusions usually do not present difficulty in diagnosis, in most populations. Exudative PE however, require careful differential diagnosis that include necessarily tuberculosis and metastatic cancer.
METHODS: We have conducted a cross-sectional study of 221 patients with persistent PE, in order to evaluate accuracy of determining adenosine deaminase activity (ADA) for diagnosis of tuberculosis pleuritis. This group of patients, taken mostly from the State Hospital for pulmonary diseases, consecutively, wa

A 55-year-old woman treated with infliximab for rheumatoid arthritis was admitted due to progressive ascites. A CT scan showed massive ascites, bilateral pleural effusion, disseminated granular shadows in the lung, and multiple swollen mediastinal lymph nodes. A FDG-PET/CT scan showed increases of FDG uptake in the mesentery and peritoneum, mimicking peritoneal carcinomatosis. Subsequent pleural and peritoneal fluid analysis showed elevated adenosine deaminase activity levels with no malignant cells. A right pleural biopsy specimen revealed Langhans giant cells and granulomas. Finally, a diagn

Undiagnosed exudative pleural effusion remains a frequent clinical challenge, particularly in high-burden settings where tuberculosis and malignancy predominate. Pleural fluid adenosine deaminase (ADA) is useful for suggesting tuberculous pleural effusion (TPE), but a substantial "gray zone" overlaps with malignant pleural effusion (MPE). This study evaluated the diagnostic yield of medical thoracoscopy with comparative analysis of pleural fluid ADA levels. This prospective, single-center, exploratory observational study was conducted over 1.5 years in a tertiary care hospital of north India.

OBJECTIVES: To assess the diagnostic accuracy of different cut-off values of pleural fluid adenosine deaminase levels as a diagnostic method for tuberculous pleural effusion.
METHODS: The prospective study was conducted from 2014 to 2016 at the Aga Khan University Hospital, Karachi, and comprised pleural fluid samples of adult patients with and without tuberculosis which were tested for adenosine deaminase levels, and divided into tuberculosis group A and non-tuberculosis group B. Sensitivity, specificity, negative predictive value and positive predictive value were calculated using different

Co-infections in pulmonary tuberculosis are rare among immunocompetent children in settings with low tuberculosis prevalence. We present a case of a two-year-old immunocompetent child with necrotic pleuropneumonia caused by Mycobacterium tuberculosis, Streptococcus pneumoniae, and Staphylococcus aureus co-infection in a low tuberculosis prevalence setting. A 24-month-old boy presented with a five-day history of cough, followed by three days of high fever and dyspnea. Initial laboratory inflammatory markers were elevated. Chest ultrasound, radiography, and CT scan revealed necrotic changes in t

Pleural involvement in visceral leishmaniasis (VL) is rare and may pose a diagnostic challenge, particularly when it mimics pleural tuberculosis. We report the case of a 39-year-old immunocompetent man who presented with a dry cough and night sweats. Initial workup revealed a small lymphocytic exudative pleural effusion, and pleural biopsy showed granulomatous inflammation with a tuberculoid appearance, leading to empirical antituberculosis treatment. Because of clinical deterioration, persistent pleural effusion, and negative molecular testing for Mycobacterium tuberculosis, further investiga
