[A surgical approach to tuberculosis of the thoracic spine and kyphotic deformity and compression of the spinal cord].

In patients with tuberculosis of thoracic vertebrae, kyphotic deformity and compression of the spinal cord involves resection of the involved portion and revision of the vertebral column. Resection was performed with a chisel that has an edge stop. Thus, 2 anterior and 2 posterior incisions (grooves) and a cross bed between them are prepared after resection on adjacent surfaces. The anterior epidural space is revised, by setting the spinal cord free from compressing agents. Three-four posterior rib ends are preliminary crossed above or below the apex of the hump at the level of the ends of tra

Aggressive lumbar vertebral hemangioma presenting with foot drop mimicking spinal tuberculosis: A case report.

BACKGROUND: Aggressive vertebral hemangiomas are uncommon vascular lesions that may present with epidural extension and neurological deficit. In tuberculosis-endemic regions, atypical vertebral hemangiomas may mimic spinal tuberculosis, resulting in diagnostic confusion and delay in definitive management. CASE DESCRIPTION: A 20-year-old male presented with a 1-year history of left lower-limb radiculopathy followed by progressive left foot drop for 1 month. The patient had previously received empirical antitubercular therapy without clinical improvement. Magnetic resonance imaging demonstrated

Differences analysis between spinal tuberculosis and brucella spondylitis with preoperative non-invasive differential diagnosis.

OBJECTIVE: Spinal tuberculosis (STB) and Brucella spondylitis (BS) represent two types of granulomatous infections affecting the spine, which exhibit significant similarities that often result in misdiagnosis and overlooked diagnoses during preoperative evaluations. This study aims to identify non-invasive evaluative indicators that facilitate the differentiation between STB and BS. METHODS: This study conducted a retrospective collection of clinical features, laboratory tests, and imaging characteristics from 89 patients diagnosed with STB and 66 patients diagnosed with BS. Differences were

Selection of surgical treatment approaches for cervicothoracic spinal tuberculosis: A 10-year case review.

BACKGROUND: Cervicothoracic spinal tuberculosis is a rare disease. Due to its difficult and challenging surgical exposure, its surgical treatment approach remains inconclusive. Long-term follow-up studies to address this puzzling issue are rarely seen in the literature. The purpose of this study was to explore the selection of surgical treatment approaches for cervicothoracic spinal tuberculosis through a 10-year case review. METHODS: From January 2003 to January 2013, 45 patients suffering from cervicothoracic spinal tuberculosis were treated surgically. According to the relation between the

Magnetic resonance imaging findings in spinal tuberculosis: Comparison of HIV positive and negative patients.

BACKGROUND: There is an increasing incidence of Human immunodeficiency virus (HIV) and tuberculosis (TB) co-infection. This has led to an increasing number of atypical features on magnetic resonance imaging (MRI). We postulated that the type 4 hypersensitivity response causing granulomatous inflammation may be disrupted by the HIV resulting in less vertebral body destruction. This study compares the MRI features of spinal tuberculosis in HIV positive and negative patients. MATERIALS AND METHODS: Fifty patients with confirmed spinal tuberculosis, HIV status and available MRI scans at a single

Could an increase in vigilance for spinal tuberculosis at primary health care level, enable earlier diagnosis at district level in a tuberculosis endemic country?

BACKGROUND: Expert clinicians and researchers in the field of spinal tuberculosis (STB) advocate for early identification and diagnosis as a key to reducing disability, severity of disease, expensive surgery and death, especially in tuberculosis (TB) endemic countries like South Africa. South Africa has the highest incidence per capita of tuberculosis in the world, and a conservative estimate of the incidence of STB in South Africa is 8-16:100 000. People living with STB may initially present to primary health care (PHC) centres, where the opportunity exists for early identification. Spinal pa

All spinal infections are not always tuberculosis: A rare case of brucella spondylodiscitis in South India.

A 49-year-old male presented with persistent fever for 20 days, accompanied by chills, headache, and vomiting. Medical history included congenital hearing impairment and speech difficulty, limiting communication; hence, the history was obtained from his wife. On examination, he was irritable, alert, and had neck stiffness without motor or sensory deficits. Cervical spine tenderness prompted a magnetic resonance imaging of the cervical spine, which demonstrated infective spondylodiscitis involving C6-C7. Given the endemic setting, multiple contiguous vertebral involvement, and occupational expo
