The potentialities of ultrasonography in scrotal diseases, with help of color-Doppler and angio-power, as well as their sensibility and specificity are discussed presenting three rather rare cases occurred with acute symptoms. It emphasises the utility of emergency scrotal scanning.

Segmental testicular infarction (STI) is a rare cause of acute scrotum. The spectrum of findings on gray-scale and color Doppler ultrasonography differ depending on the time between the onset of testicular pain and the ultrasonography examination. We are not aware of the usefulness of shear-wave elastography for the diagnosis of STI. We report the shear-wave elastography features in a case of STI and discuss the role of this diagnostic modality in the differential diagnosis.

Clinicians have relied on radionuclide scanning for evaluating acute scrotal pain and on ultrasonography for chronic scrotal pain and anatomic lesions (testicular and paratesticular structures). This review reinforces the utility of these established imaging techniques and also introduces color Doppler ultrasonography as an integral component in the advancement of testicular imaging, particularly in effective and timely diagnosis of spermatic cord torsion.
