The treatment of children with urinary incontinence caused by a neurogenic bladder presents a challenge to the paediatric urologist. Between 1965 and 1989, 527 patients with neurogenic bladder were treated and followed-up in the Department of Paediatric Urology of Debrousse Hospital. Of these, 46 were submitted to continent cystostomy, 39 to urinary artificial sphincter and 177 to clean intermittent catheterization (CIC). The diagnosis and treatment of urinary incontinence in the elderly out-clinic patients were reviewed. Sixty-three patients (24 males and 39 females) over 60 years old, who consulted our clinic complaining urinary incontinence, were subjected to the present study. The patients' ages ranged from 60 to 91, with the mean age of 72.9 years old. Today's treatment of female urinary incontinence is not merely surgical. Provided the indications are clearly established (stress incontinence due to sphincter hypotonia, bladder instability and overflow urination in some cases, sensitive frequency) and the therapist properly trained, vesicosphincteral physiotherapy represents an alternative or a complement to the other therapies. The various aspects of this rehabilitation (manual techniques, electrostimulation, biofeedback) are reported here.

The paper reports examination, treatment and follow-up data on 8 children aged 4-13 suffering from total enuresis, who failed previous conservative or operative treatment. Urethral closure with suprapubic catheterization has been used in 50 patients with urethral destruction due to an indwelling catheter. Thirty-seven of the patients were dry after the initial operation. Long-term complications were seen in 33 patients. Those that did occur were not life threatening. This relatively minor procedure offers a good alternative to formal diversion in this group of severely disabled patients.

Between June 1, 1983 and June 30, 1988, 174 patients with stress incontinence were treated with a colposuspension operation; 144 patients were cured, 30 remained wet. Sixteen patients who remained wet had detrusor instability; 9 of these were cured by anticholinergic medications. Of 25 patients who had dual pathology 19 were cured. Seventy nine patients had urinary tract infections. Two patients had ureteric obstruction due to kinking; it is postulated that this can be an infrequent and sometimes unavoidable complication.

A total of 1,012 patients referred to 4 urodynamic centres were questioned, and subsequently investigated. Coital incontinence was present in 192 of the 839 (23%), who were sexually active. Ninety-five patients (49%) had the urodynamic diagnosis of genuine stress incontinence, alone or in association with cystocoele or interstitial cystitis. Fifty-one (26.6%) had urodynamically proven detrusor instability with or without interstitial cystitis. Forty-five (23.4%) had dual pathology as the final urodynamic diagnosis.

The author used Imodium in a clinical trial for the treatment of urinary incontinence following radical prostatectomies, urinary and faecal incontinence due to carcinoma or cerebral accident in history. The drug significantly improved incontinence and did not cause adverse effects. On the basis of the similarity of innervation the author opines that loperamide also influences the function of detrusor and/or bladder orifice besides exerting an antiperistaltic and anal sphincter influencing action.

Five hundred and twenty-eight female patients with urinary incontinence were studied and treated over a 10-year period. Details of management are given, including a description of a vaginal method of reconstruction of the anatomically incompetent bladder neck. The results of treatment of each group are discussed, and the importance of initial clinical recognition of the urinary outflow obstruction syndrome is stressed in order to institute correct treatment.
