The role of vagotomy in healing of gastric ulcers induced in rats.

The role of vagotomy for healing of gastric ulcer is obscure. To investigate the effect of vagotomy on healing of experimentally-induced gastric ulcer, a mucosal defect of a constant size was made in the gastric fundus of control rats and other rats subjected to abdominal bilateral vagotomy. Gastric acid secretory response to electrical stimulation of the cervical vagus nerve and changes in body weight and in ulcer index (mm2) were estimated postoperatively, in these groups. Significant acid secretory responses to the vagal stimulation and constant weight gain were observed in the control rats…

Recovery of gastric acid after vagotomy with and without a drainage.

This study tests the possibility that the difference in acid reduction after parietal cell vagotomy (PCV) and selective gastric vagotomy with a drainage (SGV + D) could be explained by differences in the secretion recovered rather than by real secretory differences. A method for estimating the completeness of aspiration during gastric function tests using the isotope Na251CrO4 as an inert marker is described. The method was used in patients with the two types of vagotomy and the recovery of gastric secretion was more complete after PCV (89.7%) than after SGV + D (75.1%). This difference may su…

A prospective randomized trial of vagotomy in chronic duodenal ulceration.

In a prospective, randomized trial, 76 patients with duodenal ulceration treated by truncal vagotomy and pyloroplasty were compared with 77 patients who underwent highly selective vagotomy. A total of 149 patients was followed up for from 1 to 4 years, the average follow-up period being 2.6 years. There was no operative mortality and no significant difference in postoperative morbidity between the two groups. The incidence of recurrent ulceration was greater after highly selective vagotomy, but this difference was not statistically significant. The clinical results were comparable in each grou…

Effect of thoracic vagotomy and vagal stimulation on esophageal function.

The purpose of this study was to determine the effect of thoracic vagotomy and thoracic vagal stimulation upon esophageal peristalsis and lower esophageal sphincter (LES) function in the opossum. The thoracic portion of the vagus nerve was sectioned in the upper or lower thorax. Bilateral, but not unilateral, thoracic vagotomy above the level of the heart abolished peristalsis and LES relaxation in response to swallowing or cervical vagal electrical stimulation. Thoracic vagotomy at the level of the ventricle or below did not alter either peristalsis or LES relaxation during swallowing or cerv…

The practicality of the Congo Red test, or is your vagotomy complete?

The use of Congo Red testing for adequacy of vagotomy in a variety of clinical situations is described. Its intraoperative use is presented for the first time, thus allowing the surgeon immediate documentation of the completeness of his vagotomy and hopefully averting incomplete vagotomy, the single most common cause of ulcer recurrence after vagotomy-pyloroplasty.

Clinical evaluation of truncal vagotomy performed in a community hospital.

A consecutive personal experience in a community hospital involving 871 operations during a 30 year period indicates that truncal vagotomy provides excellent long term results with minimal mortality. In the surgical management of peptic ulcer disease associated with increased risk not only are highly acceptable results obtained, but also truncal vagotomy and pyloroplasty constitute an effective conservative operation with a decreased morbidity rate and lowest mortality. An over-all mortality of 0.9 per cent and a mortality of 3 per cent in the patients presenting with gross bleeding emphasizes…

[Results of vagotomy in duodenal ulcer not responsive to conservative treatment].

In the article, an analysis of the results of vagotomy with pyloroplasty in 114 patients showing duodenal ulcer is presented. It is concluded that patients with uncomplicated duodenal ulcer and minimum changes in the duodenum should not be subjected to vagotomy, though, as a rule, they have a lot of complaints. In patients with marked ulcerous changes in the duodenum vagotomy yields quite satisfactory late results.
