Vancomycin (VAN), a cationic glycopeptide antibiotic, represents a last-resort therapeutic remedy for life-threatening infections caused by multidrug resistant Gram-positive pathogens. However, biliary excretion of intravenously administered VAN in the gastrointestinal (GI) tract results in the VAN-resistant Enterococcus faecium emergence, a major cause of hospital-acquired infections. Instead of developing new antibiotics, we hypothesize that breaking the connection between intravenous antibiotic use and GI antimicrobial resistance may protect current antibiotics. Here, we develop a novel ant

CONTEXT: Vancomycin is an important antibiotic with a glycopeptide structure, effective against gram-positive bacterial infections through the inhibition of bacterial cell wall biosynthesis. It is indicated for the treatment of serious complicated skin, bloodstream, lower respiratory tract, bone, and joint infections. Primarily administered intravascularly, vancomycin shows negligible oral bioavailability. This review discusses the mechanism of action and toxicity of vancomycin, along with resistance issues and the mechanisms involved.

This study evaluated the reliability of different laboratory methods for detecting resistance to glycopeptide antibiotics-vancomycin and teicoplanin-in clinical Staphylococcus aureus and coagulase-negative staphylococci (CoNS) isolates. While automated systems are widely used in clinical microbiology laboratories due to their efficiency and ease of use, they may yield inaccurate results when assessing glycopeptide susceptibility. A total of 87 previously collected clinical isolates (22 S. aureus and 65 CoNS), initially identified as resistant to at least one of the vancomycin or teicoplanin by

Detrimental consequences of antibiotic treatment may include long-lasting disruption of the gut microbiota. Previous studies found no negative effects of antibiotics on metabolic health, although individualized responses were observed. Here, we aimed to investigate the subject-specific response to vancomycin use in tissue-specific insulin sensitivity by stratifying individuals based on the presence of antibiotic resistance genes (ARGs) or opportunistic pathogens (OPs) in the baseline fecal microbiota. Quantitative Polymerase Chain Reaction (qPCR) was used to detect ARGs and OPs in DNA isolated

With the treatment failure by vancomycin and poor clinical outcomes, the emergence and spread of vancomycin intermediate-resistant Staphylococcus aureus (VISA) has raised more concerns in recent years. While most VISA strains are isolated from methicillin-resistant S. aureus (MRSA), the mechanism underlying the generation of VISA from methicillin-susceptible S. aureus (MSSA) is still largely unknown. Here, we identified a total of 10 mutations in 9 genes through comparative genome analysis from laboratory-derived VISA strain. We verified the role of a novel mutation of WalK (I237T) and our res

Invasive methicillin-resistant Staphylococcus aureus (MRSA) treated with vancomycin (VAN) is associated with reduced VAN susceptibility and treatment failure. VAN combination therapy is one strategy to improve response, but comprehensive assessments of combinations to prevent resistance are limited. This study identifies optimal combinations to prevent the emergence of VAN-intermediate Staphylococcus aureus (VISA). Two standard MRSA and two heterogeneous VISA (hVISA) strains were exposed for 28 days in vitro to VAN alone, VAN with cefazolin (CFZ), fosfomycin, gentamicin, meropenem, rifampin, p

There is an urgent and imminent need to develop new antimicrobials to fight against antibiotic-resistant bacterial and fungal strains. In this study, a checkerboard method was used to evaluate the synergistic effects of the antimicrobial peptide P-113 and its bulky non-nature amino acid substituted derivatives with vancomycin against vancomycin-resistant Enterococcus faecium, Staphylococcus aureus, and wild-type Escherichia coli. Boron-dipyrro-methene (BODIPY) labeled vancomycin was used to characterize the interactions between the peptides, vancomycin, and bacterial strains. Moreover, neutral

Methicillin-resistant Staphylococcus aureus (MRSA) is a leading cause of bacteremia and is associated with significant morbidity and mortality. Prior studies evaluating the association of vancomycin MICs with clinical outcomes in patients with MRSA bacteremia have been inconsistent. This study evaluated the association between vancomycin MICs and 30-day in-hospital mortality rates for patients with MRSA bacteremia. This was a retrospective cohort study of patients with MRSA bacteremia treated with vancomycin for ≥72 h from January 2013 to August 2016. Vancomycin MICs were determined by broth m

Vancomycin-resistant Staphylococcus aureus (VRSA) and methicillin-resistant S aureus (MRSA) strains were examined in hospital effluents. Most S aureus strains are resistant to methicillin (MRSA), followed by tetracycline. Approximately 15% of MRSA strains are also resistant to vancomycin (VRSA). All VRSA strains developed a VanR/VanS-regulated 2-component system of VanA-type resistance in their genome. Results indicate that there is a possibility of developing resistance to aminoglycosides by VRSA strains in the near future.

Vancomycin resistance in bacteria has been classified under high priority category by World Health Organization (WHO) and its presence in hospital effluent is reported to be increasing owing to excess antibiotics use. Among various strategies, bacteriophage has been recently considered as a promising biological agent for combating such antimicrobial resistant bacteria (ARB). However, the influence of effluent's properties on phage-ARB interaction in actual hospital effluent is not completely understood. The present works intends to study this influence of hospital effluent and its parameters o

A new type of vancomycin (Van)-modified carbon nanodots (CNDs@Van) with pH-responsive surface charge switchable activity was successfully developed by covalently cross-linking Van on the surface of carbon nanodots (CNDs). Polymeric Van was formed on the surface of CNDs by covalent modification, which enhanced the targeted binding of CNDs@Van to vancomycin-resistant enterococci (VRE) biofilms and effectively reduced the carboxyl groups on the surface of CNDs to achieve pH-responsive surface charge switching. Most importantly, CNDs@Van was free at pH 7.4, but assembled at pH 5.5 owing to surface

Purpose: Vancomycin is recommended as first-line treatment of methicillin-resistant Staphylococcus aureus (MRSA) bacteremia, dosed by area-under-the-curve (AUC) with an assumed minimum inhibitory concentration (MIC) of 1 mcg/mL via broth microdilution. The purpose of this study was to compare effectiveness of AUC-based and trough-based dosing in MRSA bacteremia with an MIC > 1 mcg/mL via Etest. Methods: This was a retrospective, observational cohort that compared vancomycin dosed by AUC or trough between January 1, 2017 and September 1, 2022. The primary outcome was a composite of treatment fa

Antimicrobial resistance (AMR) exacerbates bacterial infections in different livestock species, thereby posing major challenges to animal welfare and livestock productivity. Small-molecule compounds have emerged as a promising strategy for antibacterial therapy, owing to their chemical diversity and unique mechanisms of action. In this study, we evaluated the antibacterial activity of the small-molecule compound 4-(7-(4-(diphenylamino) phenyl) benzo [c] [1,2,5] thiadiazol-4-yl)-N, N, N-trimethylbenzenaminium (TBP-TA) against vancomycin-resistant Enterococcus (VRE) through in vitro antibacteria

BACKGROUND: Single room isolation with contact precautions is widely regarded as a fundamental strategy to prevent the transmission of multidrug-resistant organisms (MDROs). However, its implementation demands substantial resources, limiting its universal application to all MDROs. In this study, we assessed the effect of discontinuing single room isolation for vancomycin-resistant Enterococcus (VRE).
METHODS: This is a retrospective, observational study conducted at a single 750-bed tertiary center. We conducted an interrupted time series analysis to compare incidence rates and trends of new-

Enterococcus faecalis is an opportunistic pathogen that causes various clinically significant infections, including infective endocarditis and catheter-associated urinary tract infections. E. faecalis assembles hair-like appendages termed endocarditis and biofilm-associated pili (Ebp), crucial for adherence, colonization, biofilm formation, and virulence. The Ebp pilus comprises three pilin subunits (EbpA, EbpB, and EbpC) encoded by the ebpABC pilus operon. EbpC forms the Ebp pilus backbone decorated with EbpA and EbpB at the tip and base for adhesion and anchoring, respectively. Experimental

BACKGROUND: Vancomycin-resistant enterococci (VRE) are multidrug-resistant microorganisms (MDRO) commonly isolated in liver transplant recipients and potentially associated with worse outcomes. We aimed to identify risk factors associated with VRE colonization in liver transplantation (LT) and its impact on posttransplant survival.
METHODS: This is a retrospective cohort that included all adults who underwent LT between 2010 and 2022 at a tertiary-level hospital in São Paulo, Brazil. Multivariate analyses were performed using logistic regression for VRE colonization risk and Cox regression fo
