Dacron vascular grafts: a long-term solution for short renal vein challenges in living donor kidney transplantation in a retrospective case series.

PURPOSE: Short renal veins in living donor kidney transplantation present technical challenges during graft placement and vascular anastomos … equired reoperation due to bleeding unrelated to the graft. No thrombosis or stenosis was detected. Vascular patency was excellent in all cases during follow-up, aside from 1 death (pneumonia) and 1 graft failure from immunoglobulin A nephropathy recurrence. CONCLUSION: Dacron vascular grafts for renal vein extension in living donor kidney transplantation are a r …

Targeted Delivery of Bioactive Molecules for Vascular Intervention and Tissue Engineering.

Cardiovascular diseases are the leading cause of death in the United States. Treatment often requires surgical interventions to re-open occluded vessels, bypass severe occlusions, or stabilize aneurysms. Despite the short-term success of such interventions, many ultimately fail due to thrombosis or restenosis (following stent placement), or incomplete healing (such as after aneurysm coil placement). Bioactive molecules capable of modulating host tissue responses and preventing these complications have been identified, but systemic delivery is often harmful or ineffective. This review discusses

COMPOUNDING PHARMACIES' POTENTIAL TO CREATE Graft Storage Solutions for Bypass Surgeries.

Several studies have addressed the optimal storage conditions for vascular grafts during bypass surgery. These studies have repeatedly shown that placing vascular graft conduits in isotonic saline solutions, and to a lesser extent in heparinized autologous blood, leads to a profound decline in endothelial cell viability. Endothelial damage to vein grafts can occur at multiple points during a coronary artery bypass graft surgery procedure: graft harvesting, handling, flushing, storage, anastomosis, and arterialization (e.g., damage caused by exposure to arterial blood pressure). This damage to

Co-immobilization of ACH11 antithrombotic peptide and CAG cell-adhesive peptide onto vascular grafts for improved hemocompatibility and endothelialization.

Surface modification by conjugating biomolecules has been widely proved to enhance biocompatibility of small-caliber artificial vascular grafts. In this study, we aimed at developing a multifunctional vascular graft that provides not only good hemocompatibility but also in situ rapid endothelialization. Herein, a vascular graft (inner diameter ∼2 mm) was fabricated by electrospinning with poly(lactic acid-co-caprolactone) and gelatin, and then biofunctionalized with antithrombotic peptide with sequence LTFPRIVFVLG (ACH11) and cell adhesion peptide with sequence CAG through adhesive poly(dopami

Modulatory Effects of Clopidogrel on Vascular Response in the Human Internal Mammary Artery: An In Vitro Study.

BACKGROUND: Coronary artery disease (CAD) is one of the leading causes of mortality worldwide, and coronary artery bypass grafting (CABG) is frequently preferred as a surgical approach in advanced cases. The most commonly used graft material in CABG, the left internal mammary artery (LIMA), is considered the gold standard due to its high long-term patency rates. Clopidogrel, an antithrombotic agent widely used in CABG patients that targets P2Y12 receptors on platelets, has also been suggested to influence vascular responses due to the presence of these receptors in vascular smooth muscle cells

External Support for Saphenous Vein Grafts in Coronary Artery Bypass Surgery: A Randomized Clinical Trial.

IMPORTANCE: Intimal hyperplasia and subsequent saphenous vein graft failure may have significant adverse clinical effects in patients undergoing coronary artery bypass surgery. External support of saphenous vein grafts has the potential to prevent vein graft dilation and hence slow the rate of intimal hyperplasia and increase long-term vein patency. OBJECTIVE: To determine efficacy, as measured by intimal hyperplasia, and safety of an external saphenous vein graft support device in patients undergoing a coronary bypass graft procedure. DESIGN, SETTING, AND PARTICIPANTS: This within-patient r

Hemodialysis fistula stenosis caused by extrinsic vascular compression: The May-Thurner of fistulas.

Significant venous outflow stenosis of a forearm arteriovenous fistulae caused by an extrinsic vascular compression is a very rare entity (no reported cases in the literature). We report a case of high venous pressures during dialysis, secondary to an extrinsic compression of the underlying brachial artery that was diagnosed with Doppler ultrasound. The stenosis localization resembles the typical and frequent juxta-anastomotic venous outflow stenosis. This similarity can be a confounding factor and can lead to failure of conventional treatments. Careful surgical technique of arteriovenous graf

[Hepatic artery reconstruction grafting with the right gastroepiploic artery for surgical treatment of upper bile duct cancer].

The right gastroepiploic artery (GEA) was used as hepatic artery graft in 2 patients with advanced upper bile duct cancer. The pedicle, including the right GEA and surrounding tissues, was mobilized along greater curvature of the stomach. The GEA pedicle was raised up beyond the gastric pylorus and was anastomosed to the distal right hepatic artery by interrupted suture technique using 7-0 monofilament-nylon stitches. The patients recovered well without evidences of anastomotic dehiscence of hepatico-jejunostomy and prolonged liver dysfunction beyond three postoperative days. Angiograms at one

Optimal conduit choice for open lower extremity bypass in chronic limb-threatening ischemia.

Open bypass surgery remains a major tool for limb salvage in chronic limb-threatening ischemia (CLTI). Although rest pain and tissue loss both fall into the category of CLTI, goals of revascularization are markedly different for each context. Rest pain mandates long-term patency considerations. Tissue loss, however, requires consideration of infection risks and patency enough to heal the wound. Of the major conduit options, autologous saphenous vein graft continues to be the conduit of choice, given both superior patency and low risk of infection. When saphenous vein graft is not available or

Multivariate analysis of long-term results after an axillobifemoral and aortobifemoral bypass in patients with aortoiliac occlusive disease.

BACKGROUND: Controversy still remains regarding the long-term results and indications for axillofemoral bypass (AxFB). A comparison of axillobifemoral bypass (AxBFB) and aortobifemoral bypass (ABFB) was thus conducted to determine whether AxFB is an acceptable alternative vascular procedure to anatomic bypass for high-risk patients. METHODS: Sixty-three patients who underwent a total of 25 AxBFBs and 38 ABFBs for aortoiliac occlusive disease were reviewed retrospectively, and both univariate and multivarate analyses were perfomed. RESULTS: The overall survival was 82.8% at five years. A univ

Radio-cephalic arteriovenous fistula recovered with drainage through the brachial vein.

Dysfunction of arteriovenous access for hemodialysis is a challenge for the vascular surgeon. Some patients have complex vascular access with problems that are difficult to solve. Careful analysis of the vascular network with ultrasound and dissection of the veins during surgery can help to identify the best option for each access. We introduce and discuss the case of creation of a radio-cephalic fistula with outflow into the brachial vein in 64-year-old hemodialysis male patient. This technique enables extending fistula patency, arterializing the brachial vein, and improves cost efficiency.

Systemic intraoperative anticoagulation during arterial injury repair: Implications for patency and bleeding.

BACKGROUND: The role of systemic intraoperative anticoagulation (SIAC) during surgical repair of major arterial injuries is controversial. Any potential improvement in arterial patency must be weighed against the risk of hemorrhage in these critically injured patients. We hypothesized that SIAC would increase arterial patency without increasing bleeding complications. METHODS: We conducted a multi-institution, retrospective cohort study of trauma patients with major vascular injury from 2005 to 2013 in three Level I centers. Arterial injuries of the neck, torso, and proximal extremities requi

An exploratory evidence-mapping review of factors influencing the patency of autogenous arteriovenous fistula in hemodialysis patients.

This exploratory evidence-mapping review assessed factors associated with long-term patency of autogenous arteriovenous fistulae (AVFs) in patients undergoing maintenance hemodialysis. Following the PRISMA 2020 guidelines, seven databases were searched from January 2014 to March 2024. We identified 14 eligible sources (5 randomized trials, 1 cohort study, 3 retrospective studies, 3 guidelines, and 2 consensus articles), which were summarized across three main areas, namely patient-related factors, clinical and vascular factors, and care and compliance factors. The results showed that low serum

18 Month patency of acellular bioengineered regenerative conduit as coronary artery bypass.

Coronary artery bypass grafting remains the standard of care for complex coronary artery disease; however, outcomes are limited by failure or lack of suitable native vessels. Tissue-engineered vascular grafts have shown promise in peripheral applications, yet durable performance in coronary circulation remains unmet. In this study, tissue-engineered vessels are implanted with and without external support as coronary artery bypass grafts in an ovine model and anticoagulants are administered for 180 days. Unsupported vessels fail early due to kink-related thrombus, whereas external supported ves

Temporary vascular shunts.

Temporary vascular shunts have been used for nearly 100 years in patients. Originally, they were used as vascular grafts that were likely to thrombose as collaterals would hopefully develop. More recently, they have been used as a device to be replaced by a permanent vascular graft during the same operation or at a reoperation. Indications for the use of shunts are a "damage control" procedure for a peripheral or truncal vascular injury, Gustilo IIIC fracture of an extremity, need for perfusion as a complex revascularization is performed, and planned replantation of a hand, forearm, or arm. The femorofemoral bypass graft. Report of a 11-year experience.

A series of 62 femorofemoral bypass grafts (FFBG) is reported. The indication for this operation was unilateral iliofemoral occlusion with severe ischemia of the lower limb in poor-risk patients. Severe ischemia presented as rest pain and/or minor necrotic lesions to the foot fingers, while patients were defined poor-risk for aging and concomitant diseases advising major surgical procedures and general anaesthesia. Claudication was not considered as an indication for this operation, and this statement is discussed in detail reviewing the literature. Operative death rate was approximately 6.4%
