Medical and psychosocial outcome of children with congenital central hypoventilation syndrome.

We report the long-term medical and psychosocial outcome of 13 children with congenital central hypoventilation syndrome. One child (8%) died before initial hospital discharge. Of the remaining 12 children, 11 (92%) have been successfully cared for in their natural or foster parents' homes. Home ventilatory support was provided with positive-pressure ventilation, negative-pressure ventilation, or diaphragm pacers. After an initial lengthy hospitalization, children spent little time in the hospital. Severe medical complications were uncommon but included cor pulmonale (one child), poor growth (

Comparing neonatal outcomes of positive- versus negative-pressure extubation: A randomized trial.

BackgroundNeonatal respiratory distress is a major issue for many infants. When non-invasive ventilation fails, endotracheal intubation is often used to secure the airway. However, extubation moving from mechanical ventilation to spontaneous breathing also has its challenges and risks.ObjectiveThis randomized controlled trial aimed to compare neonatal outcomes between positive- and negative-pressure extubation methods in a cohort of neonates ready for extubation.MethodsThis randomized controlled trial was conducted at Ayatollah Taleghani Hospital in Ilam, Iran, from May 2021 to March 2023; the

Intra-abdominal Pressure Monitoring During Negative Pressure Wound Therapy in the Open Abdomen.

INTRODUCTION: Negative pressure wound therapy (NPWT) is commonly used in open abdomen management, where there may be a simultaneous need for prevention of abdominal hypertension, tamponade of hemorrhage, and continuous fascial tension. The regional pressure dynamics of vacuum dressings are poorly understood. METHODS: Three duroc swine underwent mid-line laparotomy and application of vacuum open abdomen dressing, with and without sponge packing. Twenty-five catheters were placed throughout the abdomen to capture and record pressures in each quadrant as the vacuum system was ranged between (-75

Net Atrioventricular Compliance: A Determinant of Pulmonary Artery Pressure in Patients Undergoing Mitral Valve Replacement for Severe Mitral Stenosis.

. This single-center prospective observational study aimed to assess the correlation of net atrioventricular compliance (Cn) with pulmonary artery pressure in 26 adult patients with severe mitral stenosis (MS) undergoing surgical mitral valve replacement (MVR). Cn was estimated by doppler echocardiography, and pulmonary artery pressures measured by pulmonary artery catheter. Early outcomes including duration of intensive care unit (ICU) stay, mechanical ventilation duration, and vasoactive inotropic scores (VIS) were recorded. Cn showed moderate to strong negative correlation with systolic, di

Setting the optimal positive end-expiratory pressure: a narrative review.

The primary goals of positive end-expiratory pressure (PEEP) are to restore functional residual capacity through recruitment and prevention of alveolar collapse. Through these mechanisms, PEEP improves arterial oxygenation and may reduce the risk of ventilator-induced lung injury (VILI). Because of the many potential negative effects associated with the use of PEEP, much research has concentrated on determining the optimal PEEP setting. Arterial oxygenation targets and pressure-volume loops have been utilized to set the optimal PEEP for decades. Several other techniques have been suggested, in

Tachycardia and hypertension enhance tracer efflux from the spinal cord.

BACKGROUND: Disruption of cerebrospinal fluid (CSF)/interstitial fluid (ISF) exchange in the spinal cord is likely to contribute to central nervous system (CNS) diseases that involve abnormal fluid accumulation, including spinal cord oedema and syringomyelia. However, the physiological factors that govern fluid transport in the spinal cord are poorly understood. The aims of this study were to determine the effects of cardiac pulsations and respiration on tracer signal increase, indicative of molecular movement following infusion into the spinal cord grey or white matter. METHODS: In Sprague D

Dyspnea in a patient years after severe poliomyelitis. The role of cardiopulmonary exercise testing.

Dyspnea after polio can occur for a variety of reasons, including neuromuscular disease and upper airway abnormalities resulting from prolonged intubation, including tracheal stenosis, tracheomalacia, and vocal cord paralysis. Routine studies such as spirometry and maximum voluntary ventilation (MVV) measurements can give similar results in these conditions. We present a 50-year-old woman who as a child developed poliomyelitis that required tracheostomy and negative pressure ventilation. Thirty-nine years later, she developed breathlessness with normal spirometry but decreased MVV. The flow vo

Predictive value of lung ultrasound score in weaning from mechanical ventilation in neonatal respiratory distress syndrome.

BACKGROUND: To explore the predictive value of lung ultrasound score (LUS) in weaning from mechanical ventilation in neonatal respiratory distress syndrome (RDS). METHODS: A total of 111 neonates with RDS who received mechanical ventilation in the neonatal intensive care unit (NICU) of Sichuan Provincial People's Hospital were selected as the subjects. Before weaning, the LUS was performed by the 12-region ultrasound score of the lungs. Those neonates were divided into the weaning success group (n = 95) and weaning failure group (n = 16) according to whether they received mechanical ventilati

Infection with Ureaplasma urealyticum: is there a specific clinical and radiological course in the preterm infant?

BACKGROUND: Despite having mild early respiratory disease, many preterm babies develop chronic lung disease (CLD). Intrauterine infection with Ureaplasma urealyticum has been associated with preterm labour and CLD. OBJECTIVE: To test the hypothesis that infection with U urealyticum results in a specific clinical and radiological picture in the first 10 days of life. METHODS: Retrospective study of 60 ventilated babies < 30 weeks gestation, who had tracheal secretions tested for U urealyticum. Placental histology was reviewed by a paediatric pathologist for signs of chorioamnionitis. Chest ra

CLOSED-LOOP VENTILATION AND OXYGENATION WITH DECISION SUPPORT FLUID RESUSCITATION TO TREAT MAJOR BURN INJURY WITH SMOKE-INDUCED ACUTE RESPIRATORY DISTRESS SYNDROME.

Introduction: The understanding of the interaction of closed-loop control of ventilation and oxygenation, specifically fraction of inspired oxygen (FiO 2 ) and positive end-expiratory pressure (PEEP), and fluid resuscitation after burn injury and acute lung injury from smoke inhalation is limited. We compared the effectiveness of FiO 2 , PEEP, and ventilation adjusted automatically using adaptive support ventilation (ASV) and decision support fluid resuscitation based on urine output in a clinically relevant conscious ovine model of lung injury secondary to combined smoke inhalation and major

Extracorporeal Carbon Dioxide Removal During Continuous Renal Replacement Therapy as Adjunctive Therapy.

Lung-protective ventilation targeting low tidal volumes and plateau pressures is the mainstay of therapy in patients with ARDS. This ventilation strategy limits pulmonary strain, inflammation, and injury, but it may be associated with profound hypercapnic acidosis. In such conditions, extracorporeal CO2 removal can attenuate or normalize hypercapnia and may even facilitate ultraprotective ventilation. Almost half of patients with ARDS develop renal failure. Pathophysiological cross-talk between the injured lung and kidney may aggravate global organ failure and weighs negatively on outcomes. A

[Indications for the insertion of ventilating tubes in the mucotympanum of the child].

The influence of adenoidectomy, myringotomy and lavage of the middle ear was studied in children suffering from secretory otitis. The children were controlled by audiograms and tympanograms over a one-year period of time. 89 percent of all children showed normal values one year following surgery, 11 percent had still high negative middle ear pressures and conductive hearing loss. We think that these children only are candidates for ventilating tubes (grommets).

On the very first, successful, long-term, large-scale use of IPPV. Albert Bower and V Ray Bennett: Los Angeles, 1948-1949.

An "unprecedented respirator patient load at Los Angeles County Hospital [LACH] in 1948 (294 respirator cases)" arose from a seasonal increase in poliomyelitis cases to nearepidemic proportions. A finding by physician Albert Bower and his team that respiratory acidosis was frequent in patients receiving intermittent negative pressure ventilation (INPV), together with their awareness of a previous high mortality rate due to the standard treatment of polio ventilatory failure with Drinker-Collins respirators, led to multiple advances in equipment technology for LACH. Most important was biomedica

Emergency medicine updates: Acute respiratory distress Syndrome.

INTRODUCTION: Acute respiratory distress syndrome (ARDS) is a critical condition associated with severe morbidity and mortality. There are several components of management, and it is important for emergency clinicians to be aware of these as they manage critically ill patients who may face extended time in the emergency department. OBJECTIVE: This paper evaluates evidence-based updates concerning ARDS. DISCUSSION: ARDS is a condition associated with diffuse, severe lung inflammation resulting in hypoxemic respiratory failure. While there have been several definitions, the most recent concept

[Breakthroughs in global critical care medicine 2019].

The following trends emerged in international critical care practice in 2019: increasing analysis to phenotypes of sepsis and acute respiratory distress syndrome (ARDS), increasing evidences of early initiating of vasopressors and antibiotics for septic shock, also including the early furosemide stress test for critically ill patients with acute kidney injury (AKI). In addition, there are many significantly important trials with positive results: high dose vitamin C for septic shock, weaning strategy with pressure support ventilation (PSV) mode, tranexamic acid for patients with acute traumati

[Acute pulmonary oedema associated with laryngospasm after thyroidectomy: a case report].

In this paper, a case report of acute pulmonary oedema following thyroidectomy in a 51-year-old male patient is reported. The aetiological factor was vigorous inspiratory effort against an obstruction of the upper airway occurring immediately after extubation. The patient was transferred to intensive care unit and treated with CPAP ventilation. He recovered completely after 48 hours. The pathogenesis of both laryngospasm and an unexpexted but potentially serious complication such as negative-pressure pulmonary oedema is discussed.

Respiratory dysfunction in ventilated patients: can inspiratory muscle training help?

Respiratory muscle dysfunction is associated with prolonged and difficult weaning from mechanical ventilation. This dysfunction in ventilator-dependent patients is multifactorial: there is evidence that inspiratory muscle weakness is partially explained by disuse atrophy secondary to ventilation, and positive end-expiratory pressure can further reduce muscle strength by negatively shifting the length-tension curve of the diaphragm. Polyneuropathy is also likely to contribute to apparent muscle weakness in critically ill patients, and nutritional and pharmaceutical effects may further compound

A non-invasive method for measuring inspiratory muscle fatigue during progressive isocapnic hyperventilation in man.

Eleven normal adults each performed a ten minute progressive isocapnic hyperventilation (PIHV) test in which ventilatory levels were increased every two minutes. All subjects exhibited mechanical fatigue by failing to maintain the target of 80% of maximum voluntary ventilation (MVV). The mean ventilation at this level was 67.5 +/- 1.4% MVV. This fatigue was accompanied by a fall in transdiaphragmatic pressure. During the test the EMG of the sternomastoid (SM) was monitored by surface electrodes and was analyzed using fast-fourier transform. The centroid frequency (Fc) fell as ventilation incre

Common sleep problems in ICU: heart failure and sleep-disordered breathing syndromes.

Ventilation during sleep is under tight metabolic control, and can be destabilized by upper airway obstruction leading to snoring or obstructive apneas, inadequate respiratory pump muscle activity leading to hypoventilation, and central control instability leading to changes in metabolic feedback and loop gain. These three physiologic disturbances can lead to obstructive sleep apnea hypopnea syndrome (OSAHS), hypoventilation syndromes, and periodic breathing. OSAHS places a strain on the cardiac output by virtue of hypoxemia, large negative intrathoracic pressures, and high swings in systemic
