Left ventricular dysfunction induced by cold exposure in patients with systemic sclerosis.

Raynaud's phenomenon and cardiac abnormalities are frequent in patients with systemic sclerosis. Radionuclide ventriculograms were obtained in 16 patients with Raynaud's phenomenon and systemic sclerosis or the related CREST syndrome and in 11 normal volunteers in order to evaluate changes in left ventricular function that might be induced by exposure to cold. Left ventricular regional wall motion abnormalities developed in nine of 16 patients during cooling compared with only one of 11 control subjects, despite a comparable rise in mean arterial pressure (p less than 0.02). The abnormalities

Association Between MRI-Derived Right-to-Left Ventricular Volume Ratio and Regional Left Ventricular Myocardial Dysfunction in Patients With Pulmonary Arterial Hypertension.

BACKGROUND: Right ventricular (RV) to left ventricular (LV) end-diastolic volume ratio (RV/LV) is a validated marker of RV dilation in pulmonary arterial hypertension (PAH), yet its impact on LV myocardial function and the underlying mediating pathway is incompletely characterized. PURPOSE: To investigate the association between RV/LV volume ratio and LV dysfunction in PAH patients, with a focus on regional strain patterns and mediating pathways. STUDY TYPE: Retrospective. POPULATION: 188 PAH patients were divided into normal (ratio < 1.27, n = 74) and elevated (ratio ≥ 1.27, n = 114) RV/LV

Left ventricular diastolic dysfunction without left ventricular hypertrophy in obese children and adolescents: a Tissue Doppler Imaging and Cardiac Troponin I Study.

BACKGROUND: Obesity increases the risk for various cardiovascular problems. Increase in body mass index is often an independent risk factor for the development of elevated blood pressure and clustering of various cardiovascular risk factors. OBJECTIVE: To determine early markers of left ventricular affection in obese patients before the appearance of left ventricular hypertrophy. METHODS: In this cross-sectional study, we evaluated 42 obese patients and 30 healthy controls. Their ages ranged from 6 to 19 years. Studied children were subjected to anthropometric, lipid profile, and serum Tropo

Patterns of left and right ventricular dysfunction and their clinical outcomes in septic patients.

BACKGROUND: To investigate the various types of left ventricular (LV) and right ventricular (RV) dysfunction and their association with clinical outcomes in septic patients. METHODS: We retrospectively reviewed a cohort of sepsis and septic shock patients admitted to a tertiary hospital intensive care unit (ICU). Left ventricular dysfunction was determined by left ventricular ejection fraction (LVEF). Right ventricular systolic dysfunction (RVSD) was determined by tricuspid annular plane systolic excursion (TAPSE), fractional area change (FAC), or RV S’ velocity. Cardiac function was categoriz

Left Ventricular Dysfunction Following the Ross-Konno Reconstruction is Independent of the Operation.

The Ross-Konno (RK) operation is a well-established surgical treatment for combined left ventricular outflow tract obstruction and aortic valve pathology in children. Prior study has demonstrated that mechanical and electrical dyssynchrony exist post-RK compared to normal controls. The purpose of this study was to evaluate myocardial function pre- and post-RK as defined by echocardiography. Patients undergoing the RK operation (n = 13; median age: 1.3 years; range: 0.5-13.3 years) were evaluated by echocardiography at defined timepoints: pre-RK, post-RK, 1-year post-RK, and 2 years post-RK. An evaluation of the safety of continuing trastuzumab despite overt left ventricular dysfunction.

BACKGROUND: The major limitation in the use of trastuzumab therapy is cardiotoxicity. We evaluated the safety of a strategy of continuing trastuzumab in patients with breast cancer despite mild, asymptomatic left ventricular impairment. METHODS: Charts of consecutive patients referred to a cardio-oncology clinic from January 2015 to March 2017 for decline in left ventricular ejection fraction (lvef), defined as a fall of 10 percentage points or more, or a value of less than 50% during trastuzumab therapy, were reviewed. The primary outcome of interest was change in lvef, measured before and d

Prevalence and risk factors for left ventricular diastolic dysfunction in a scleroderma cohort.

OBJECTIVES: Left ventricular diastolic dysfunction (LVDD) is more common in systemic sclerosis (SSc) compared to the general population. Focal myocardial ischaemia and fibrosis may be important in its pathogenesis. LVDD is associated with increased mortality and little is known about the risk factors. Advanced SSc lung complications may accompany LVDD. METHOD: We conducted a cross-sectional study of 300 SSc outpatients with and without LVDD, seen between May 2012 and May 2014, and performed univariate and multivariate regression analyses to determine clinical factors associated with LVDD. LVD

Prevalence and predictors of left ventricular diastolic dysfunction in a Hispanic patient population.

Minimal data exist on attributes of diastolic dysfunction in the Hispanic population. The purpose of this study was to evaluate the prevalence and predictors of diastolic dysfunction in a Hispanic patient population. We performed a retrospective review of 166 consecutive echocardiograms in a southwestern Texas Hospital that caters to a large Hispanic patient population. We identified all echocardiograms that met criteria for diastolic dysfunction and assessed baseline demographics and comorbidities in the cohort of Hispanic patients. A multivariate analysis was performed to identify the indepe

Mineralocorticoid escape by the kidney but not the heart in experimental asymptomatic left ventricular dysfunction.

Unlike healthy subjects, overt congestive heart failure cannot "escape" the sodium- and water-retaining actions of mineralocorticoid excess. It is undefined whether escape occurs in asymptomatic left ventricular dysfunction (ALVD), which is characterized by preserved sodium homeostasis, natriuretic peptide activation, and normal circulating aldosterone. We hypothesized that, in ALVD, mineralocorticoid excess with exogenous deoxycorticosterone acetate (DOCA) would overwhelm renal compensatory mechanisms, resulting in sodium and water retention, and promote renal and cardiac collagen deposition.

Left ventricular dysfunction following ligation of the ductus arteriosus in the preterm baboon.

We evaluated the effect of early closure of the patent ductus arteriosus (PDA) on cardiac performance in a preterm baboon model of hyaline membrane disease. Eleven baboon fetuses were delivered at 75% gestation, resuscitated, and treated with conventional ventilation. The animals were divided into two groups, and underwent left thoracotomy at 2 hr of age. Group 1 (N = 5) had ductal infiltration with formalin to maintain ductal patency. Group 2 (N = 6) had ductal ligation. Hemodynamic parameters were compared between groups before and after surgical intervention. Examination of cardiovascular f

Left ventricular assist device improves survival in children with left ventricular dysfunction after repair of anomalous origin of the left coronary artery from the pulmonary artery.

BACKGROUND: Repair of anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) in infants carries a high operative risk, particularly in infants with myocardial infarction and poor left ventricular function. The marked recovery of left ventricular function reported late after repair, however, suggests that an aggressive approach to repair should be undertaken. METHODS: Of 31 children undergoing primary repair of ALCAPA at our institution from 1987 to 1996, 26 were infants (6 weeks to 9 months old). All but 2 had severe left ventricular dysfunction, and 8 had moderate to

Mitral Transcatheter Edge-to-Edge Repair in Ventricular Functional Mitral Regurgitation and Very Severe Left Ventricular Dysfunction.

BACKGROUND: Patients with functional mitral regurgitation (MR) and very severe left ventricular (LV) dysfunction (LV ejection fraction [LVEF] <20%) have been largely excluded from randomized trials of mitral transcatheter edge-to-edge repair (M-TEER), leaving a substantial knowledge gap regarding the role of M-TEER in this high-risk population. OBJECTIVES: This study aimed to evaluate the safety, feasibility, and 1-year outcomes of M-TEER in patients with functional MR and LVEF <20%. METHODS: The OCEAN (Optimized Catheter Valvular Intervention)-Mitral registry prospectively enrolled patients

Association of left ventricular longitudinal myocardial function with subclinical right ventricular dysfunction in type 2 diabetes mellitus.

BACKGROUND: Left ventricular (LV) involvement in diabetic cardiomyopathy has been reported; however, only limited data exist on right ventricular (RV) involvement. Therefore, our purpose was to investigate RV systolic dysfunction and its association with LV longitudinal myocardial dysfunction in patients with type 2 diabetes mellitus (T2DM) and preserved LV ejection fraction (LVEF). METHODS: We studied 177 T2DM patients with preserved LVEF and 79 age-, sex-, and LVEF-matched healthy volunteers. LV longitudinal myocardial function was assessed as global longitudinal strain (GLS), and RV systol
