Clinical and cardiac characteristics of COVID-19 mortalities in a diverse New York City Cohort.

INTRODUCTION: Electrocardiographic characteristics in COVID-19-related mortality have not yet been reported, particularly in racial/ethnic minorities. METHODS AND RESULTS: We reviewed demographics, laboratory and cardiac tests, medications, and cardiac rhythm proximate to death or initiation of comfort care for patients hospitalized with a positive SARS-CoV-2 reverse-transcriptase polymerase chain reaction in three New York City hospitals between March 1 and April 3, 2020 who died. We described clinical characteristics and compared factors contributing toward arrhythmic versus nonarrhythmic d

New-Onset Atrial Flutter Revealing a Large Right Coronary Artery to Superior Vena Cava Fistula.

BACKGROUND: Coronary artery fistulas are rare anomalous communications between a coronary artery and a cardiac chamber or great vessel. Large fistulas can cause chamber dilation, myocardial ischemia, heart failure, and arrhythmias. CASE SUMMARY: A 63-year-old man presented with palpitations, hypotension, and atrial flutter with rapid ventricular response requiring urgent cardioversion. Multimodality imaging with coronary computed tomography angiography and cardiac magnetic resonance revealed a large right coronary artery-to-superior vena cava fistula with right-sided chamber dilation. Cardiac

Cardiac Involvement in Emery-Dreifuss Muscular Dystrophy and Related Management Strategies.

Emery-Dreifuss muscular dystrophy (EDMD) is a group of hereditary muscular dystrophy syndrome caused by deficiency of genes encoding nuclear envelope proteins. Patients having EDMD show the triad of muscle dystrophy, joint contracture, and cardiac disease. In almost all patients, cardiac involvement is prevalent and is the most severe aspect of EDMD. Cardiac disease is predominantly shown by conduction defects, atrial fibrillation/flutter, and atrial standstill. Sudden death and heart failure because of left ventricular dysfunction are important causes of mortality, particularly in those patie

[Effect of chronic stimulation of the negative and positive emotionogenic centers of the hypothalamus on development of cardiac arrhythmias].

Chronic experiments were conducted on 46 rabbits; a study was made of the changes in cardiac activity arising in prolonged (up to 1--2 weeks) electric stimulation of the ventro-median nuclei of the hypothalamus. Disturbances of the cardiac rhythm in the form of tachycardia, atrial flutter and fibrillation, ventricular extrasystole and paroxysmal ventricular tachysystole occurred mostly during the first days of stimulation and ceased after inderal infusion. Functional "weakness" of the cardiac pacemakers was revealed in the animals subjected to 1--2-week stimulation of the ventromedian nuclei.

Atrial flutter presenting as broad complex tachycardia in a patient with right sided pneumonectomy.

A 71-year-old man with no history of coronary artery disease presented with palpitations to the emergency department. The 12-lead ECG showed a regular tachycardia with wide QRS complexes (220 bpm) suggestive of ventricular tachycardia. Instead invasive electrophysiological investigation revealed typical atrial flutter as underlying arrhythmia. The altered QRS morphology resulted from displacement of the heart into the right hemithorax due to right-sided pneumonectomy in combination with bundle branch block.

Leeuwenhoek's disease: diaphragmatic flutter in a cardiac patient.

A 15-year-old girl gave a recent history of dyspnoea and "funny turns". She had congenital aortic stenosis, previous valvotomies, a mechanical valve replacement, permanent pacemaker, atrial tachyarrhythmias, impaired ventricular function, systemic hypotension, pulmonary hypertension, and anxiety. The diagnosis of diaphragmatic flutter was delayed due to all the differential diagnoses and rarity of the condition. It was confirmed by observation, respiratory band monitoring, volume-time spirogram and fluoroscopy during an attack.

[Drug therapy of cardiac arrhythmias 199].

Current antiarrhythmic drug therapy is employed in strict compliance with the proper indication, which is itself limited by the side effects of the available drugs, and modified by the increasing success of such non-drug options as electrical ablation and implantable cardioverters/defibrillators. Drug treatment is restricted to 3 major indications, regular paroxysmal supraventricular tachycardias, atrial flutter and fibrillation, and ventricular tachycardias. Class IA and IC agents are used only to treat supraventricular and ventricular arrhythmias with no structural heart disease. Class III d

Atrial flutter.

Atrial flutter is a supraventricular tachydysrhythmia believed to arise from electrophysiologic disturbances in the atria. It tends to be an unstable rhythm and is usually associated with intrinsic cardiac or pulmonary disease or adverse extrinsic influences on the heart. It is due to either a reentry mechanism or an increased atrial automaticity. Atrial depolarization is regular at a rate of 260 to 340 beats per minute. With a normal atrioventricular (AV) node there is usually a physiologic second-degree block with resultant 2:1 conduction. Higher degrees of AV block can occur in patients wit

Pacing techniques in the treatment of tachycardias.

Pacemakers are playing an increasing role in the management of patients with tachycardia. Pacing to prevent tachycardia must be differentiated from pacing to terminate tachycardia. Pacing is of proven value in preventing ventricular tachycardia that occurs in the setting of complete heart block or the setting of drug-induced QT interval prolongation and may be valuable in some other patients with nonsustained ventricular tachycardia. Pacing termination has found its greatest use as an alternative to cardioversion in patients with atrial flutter, especially those with suspected digitalis toxici

A modified approach for programmed electrical stimulation in mice: Inducibility of ventricular arrhythmias.

BACKGROUND: Electrophysiological studies in mice, the prevailing model organism in the field of basic cardiovascular research, are impeded by the low yield of programmed electrical stimulation (PES). OBJECTIVE: To investigate a modified approach for ventricular arrhythmia (VA) induction and a novel scoring system in mice. METHOD: A systematic review of literature on current methods for PES in mice searching the PubMed database revealed that VA inducibility was low and ranged widely (4.6 ± 10.7%). Based on this literature review, a modified PES protocol with 3 to 10 extrastimuli was developed

Apparent extension of the atrioventricular interval due to sensor-based algorithm against supraventricular tachyarrhythmias.

Rapid ventricular tracking response to supraventricular tachyarrhythmia is one major limitation to DDD pacing. In a DDDR pacemaker, sensor-based algorithms have been used to control these arrhythmias. These include the use of an interim rate limit (conditional ventricular tracking limit) or a separate maximum tracking and sensor rate limits (discrepant upper rate). These algorithms limit inappropriate ventricular pacing rate during tracking of pathological supraventricular tachyarrhythmia and atrial flutter by Wenckebach-like prolongation of the AV interval. We observed that this may cause an

Computerized activation sequence mapping of the human atrial septum.

To delineate the propagation of electrical activation in the atrial septum, atrial epicardial and atrial septal maps were recorded intraoperatively using a 156-channel computerized mapping system in 12 patients during sinus rhythm (n = 10), supraventricular tachycardia associated with septal pathways in Wolff-Parkinson-White syndrome (n = 3), atrioventricular (AV) node reentrant tachycardia (n = 4), and atrial flutter (n = 5). The epicardial and septal data were recorded simultaneously from 156 atrial electrodes, digitized, analyzed, and displayed as isochronous maps on a two-dimensional diagr

Modification of ventricular tachycardia by carotid sinus massage.

A case of ventricular tachycardia (VT) was aggravated by carotid sinus massage in a woman with an old myocardial infarction. The patient presented with a wide QRS complex tachycardia with a rate of 143 beats/min, which fulfilled the classic and newer electrocardiographic criteria for the diagnosis of VT. Carotid sinus massage performed during the tachycardia resulted in its conversion to another, wide QRS complex tachycardia, with different morphology and a faster rate resembling ventricular flutter. This latter tachycardia was converted to sinus rhythm by a thump on the patient's chest. The i…

[Arrhythmias after surgery for congenital heart disease].

The improvement of surgical techniques over the last few years have made postoperative chronotropic insufficiency either by sinus node dysfunction or iatrogenic atrioventricular block less common. However, reentrant tachycardia around lines of incision or a patch may be observed. Persistent dilatation of a cardiac chamber or ventricular dysfunction are predisposing factors. Flutter-like arrhythmias occur mainly after atrial surgery (closure of atrial septal defects, Mustard, Senning or Fontan procedures). Ventricular tachycardias are observed more often after correction of tetralogy of Fallot

Unusual Manifestation of Graves' Disease: Ventricular Fibrillation.

BACKGROUND: It is well known that thyrotoxicosis causes rhythm disorders including sinus tachycardia, atrial fibrillation, and atrial flutter. Atrial fibrillation is the most common arrhythmia in thyrotoxicosis, occurring in 5-15% of patients over 60 years of age, whereas ventricular arrhythmia is an unusual manifestation. CASE REPORT: An 18-year-old Japanese woman was admitted to our emergency department because of loss of consciousness caused by ventricular fibrillation. She had been diagnosed with Graves' disease only 5 days earlier and had no other past medical history. Blood examination
