Right ventricular and pulmonary vascular function indices for risk stratification of patients with pulmonary regurgitation.

BACKGROUND: We hypothesized that echocardiographic indices of right ventricular to pulmonary artery (RV-PA) coupling were comparable to cardiac magnetic resonance imaging (CMRI)-derived RV volumetric indices in predicting disease severity in chronic pulmonary regurgitation (PR). METHODS: Patients with ≥ moderate PR (2003-2015) with and without prior CMRI scans were enrolled into the study cohort and validation cohort, respectively. Endpoint was to determine the association between noninvasive RV-PA coupling indices (tricuspid annular plane systolic excursion/right ventricular systolic pressur

Echocardiographic evaluation of the right ventricular dimension and systolic function in dogs with pulmonary hypertension.

BACKGROUND: Right ventricular (RV) enlargement and dysfunction are associated with prognosis in humans with pulmonary hypertension (PH). HYPOTHESIS/OBJECTIVES: To assess RV size and systolic function in dogs with PH and to determine if they are associated with disease severity and right-sided congestive heart failure (R-CHF). ANIMALS: 89 dogs with PH and 74 healthy dogs. METHODS: Prospective observational study. PH was classified according to the tricuspid regurgitation pressure gradient. RV end-diastolic area (RVEDA) index was calculated as RVEDA divided by body surface area. RV systolic f

Pilot study shows right ventricular diastolic function impairment in young children with obstructive respiratory disease.

AIM: This study determined cardiovascular impairment in young children with obstructive respiratory disease who were assessed using the opening interrupter technique (RINT). METHODS: This pilot study enrolled 41 children who had been referred to pulmonology and allergology specialists at the University of Catania, Italy, from March to July 2017: 23 (mean age 4.13 ± 0.62 years) had chronic coughs and wheezing and 18 controls (mean age 4.27 ± 0.66 years) had obstructive chest disease, but were otherwise healthy. Airway resistance was evaluated using RINT and cardiac function by studying the eje

Right Ventricular Hypertrophy, Systolic Function, and Disease Severity in Anderson-Fabry Disease: An Echocardiographic Study.

BACKGROUND: Right ventricular (RV) involvement has been described in Anderson-Fabry disease (AFD), especially in patients with established Fabry cardiomyopathy (FC). However, few and controversial data on RV systolic function are available, and there are no specific tissue Doppler studies. METHODS: Detailed echocardiographic examinations were performed in 45 patients with AFD. FC, defined as maximal left ventricular wall thickness ≥ 15 mm, was present in 12. The Mainz Severity Score Index was calculated for each patient. Pulsed tissue Doppler was applied to the RV free wall at the tricuspid a

Right ventricular function in dilated cardiomyopathy and ischemic heart disease: assessment with non-invasive imaging.

BACKGROUND: Dilated cardiomyopathy and ischaemic heart disease can both lead to right ventricular (RV) dysfunction. Direct comparisons of the two entities regarding RV size and function using state-of-the-art imaging techniques have not yet been performed. We aimed to determine RV function and volume in dilated cardiomyopathy and ischaemic heart disease in relation to left ventricular (LV) systolic and diastolic function and systolic pulmonary artery pressure. METHODS AND RESULTS: A well-characterised group (cardiac magnetic resonance imaging, echocardiography, coronary angiography and endomy

Impact of transcatheter aortic valve implantation or surgical aortic valve replacement on right ventricular function.

OBJECTIVE: Transcatheter aortic valve implantation (TAVI) has become an alternative to surgical aortic valve replacement (sAVR) in selected high risk patients. While improvement in left ventricular function after TAVI has been demonstrated, little is known about the impact on right ventricular (RV) function. Since postoperative RV dysfunction is linked to adverse outcomes, the authors sought to investigate the effect of TAVI and aortic valve replacement (AVR) on RV function using speckle tracking echocardiography. DESIGN: Cross-sectional study in tertiary healthcare setting. SETTING: 101 pat

Preservation of right ventricular structure and function following transatrial-transpulmonary repair of tetralogy of Fallot.

OBJECTIVES: Management strategy of patients with tetralogy of Fallot (TOF)-including timing, as well as surgical technique-remains a controversial topic. We sought to analyse both early and late results of our consistent policy of non-neonatal transatrial/transpulmonary (TA/TP) repair of TOF over almost 14 years, in order to assess preservation or possible deterioration of right ventricular (RV) function. METHODS: All 245 consecutive patients with TOF, referred to our group for repair between September 1997 and December 2010, have been prospectively followed up. Their clinical and echocardiog

Impairment of diastolic function by lack of frequency-dependent myofilament desensitization rabbit right ventricular hypertrophy.

BACKGROUND: Ventricular hypertrophy is a physiological response to pressure overload that, if left untreated, can ultimately result in ventricular dysfunction, including diastolic dysfunction. The aim of this study was to test the hypothesis that frequency-dependent myofilament desensitization, a physiological response of healthy myocardium, is altered in hypertrophied myocardium. METHODS AND RESULTS: New Zealand white rabbits underwent a pulmonary artery banding procedure to induce pressure overload. After 10 weeks, the animals were euthanized, hearts removed, and suitable trabeculae harvest

Standard measures of right ventricular function assessment in adult patients with acute sickle cell crises.

Chronic sickle cell (SC) disease is known to cause pulmonary hypertension (PH) which eventually affects right as well as left ventricular function. However, the acute effects of SC crisis on right ventricular (RV) function in adults have not been well described. Our echocardiography database was queried for patients followed in the outpatient hematology clinic, who were admitted with SC crises, and had an inpatient echocardiogram. Comparisons of RV fractional area change (RVFAC) and tricuspid annular peak systolic excursion (TAPSE) were then made to a group of healthy patients and a group with

Usefulness of the myocardial performance index for assessing right ventricular function in congenital heart disease.

Quantitative assessment of ventricular function in patients with congenital heart disease is often challenging due to distorted ventricular geometry. A myocardial performance index (MPI) has been reported in adults and children that is a Doppler-derived nongeometric measure of ventricular function. The MPI measures the ratio of isovolumic time intervals (isovolumic contraction time and isovolumic relaxation time) to ventricular ejection time. The effects of altered ventricular preload or afterload on the MPI have yet to be determined. This study assesses the impact of altered preload or afterl

Effects of phenylephrine or volume loading on right ventricular function in patients undergoing myocardial revascularization.

The acute effects of phenylephrine (PHE) administration or intravascular volume loading on right ventricular (RV) function were examined in 34 patients undergoing elective coronary artery surgery. After anesthetic induction with sufentanil and midazolam, 20 patients received PHE to treat hypotension and increase systemic arterial pressure 20% above baseline values. PHE effectively restored arterial pressure without changing stroke index (SI), although RV ejection fraction (RVEF) declined (41.3% to 37.6%) with concomitant increases in RV end-diastolic volume index (RVEDVI) (86.3 to 97.5 mL/m2)

[Right ventricular function in patients with rheumatic mitral valve stenosis. Effect of balloon mitral valvuloplasty].

UNLABELLED: Postrheumatic mitral stenosis might cause impairment of right ventricular (RV) function due to both an increase in RV afterload and rheumatic myocardial disease. Therefore, we investigated in 19 patients with postrheumatic mitral stenosis and sinus rhythm right ventricular volumes and hemodynamics by a computerized thermodilution catheter during rest and supine bicycle exercise. In 14 patients the investigation was repeated within 2 days after balloon mitral valvuloplasty. Resting RV ejection fraction was decreased (43 (15-53)%, median (range)) and correlated significantly with RV

Right ventricular dysfunction in patients with symptomatic atrial fibrillation: Prevalence and functional implications.

BACKGROUND: Assessment of left ventricular (LV) function is an essential component of guideline-recommended management of atrial fibrillation (AF). However, the role and implications of right ventricular (RV) dysfunction in AF are poorly understood. OBJECTIVE: This study aimed to determine the prevalence of RV dysfunction and evaluate its association with cardiorespiratory fitness (CRF) and symptom burden in patients with symptomatic AF. METHODS: Consecutive adults with symptomatic paroxysmal or persistent AF and preserved LV ejection fraction scheduled for catheter ablation underwent transt

[Incidence and extent of right ventricular functional disorders following the 1st myocardial infarction].

A series of 63 patients were studied by gated radionuclide angiocardiography 51/2 weeks after their first acute myocardial infarction. Left (LVEF) and right (RVEF) ventricular ejection fraction were assessed in three groups of patients: 12 patients with transmural anterior infarctions, 35 patients with transmural inferior infarctions and 16 patients with non-transmural infarctions. LVEF was depressed only in the group of patients with anterior infarctions (34.4 +/- 16.3%) but remained within normal limits in the other two groups. RVEF was reduced in 64% of patients with inferior infarctions; t

Age-changes in right ventricular function-pulmonary circulation coupling: from pediatric to adult stage in 1899 healthy subjects. The RIGHT Heart International NETwork (RIGHT-NET).

The present study analyzes age-specific changes in RV function and RV-PA coupling in a large cohort of apparently healthy subjects with a wide age-range, to identify reference values and to study the influence of clinical and echocardiographic cofactors. 1899 Consecutive healthy subjects underwent a standardized transthoracic echocardiographic examination. Tricuspid annular plane systolic excursion (TAPSE) and systolic pulmonary artery pressure (SPAP) were measured. Ventriculo-arterial coupling was then inferred from the TAPSE/SPAP ratio. A quantile regression analysis was used to estimate qua

Right ventricular function across different non-invasive respiratory support strategies in severe community-acquired pneumonia.

BACKGROUND: Right ventricular (RV) systolic impairment is clinically relevant in acute respiratory failure, but its relationship with respiratory support modality in severe community-acquired pneumonia (CAP) outside the intensive care unit (ICU) is poorly defined. OBJECTIVES: To compare RV longitudinal systolic performance and hospital length of stay (LOS) among patients receiving non-invasive ventilation (NIV), high-flow nasal cannula (HFNC), or Venturi-mask oxygen therapy. METHODS: In this single-centre prospective observational study, adults with severe CAP (Pneumonia Severity Index class

Deep learning-enabled assessment of right ventricular size and function is associated with cardiovascular outcomes.

Right ventricular (RV) size and function inform prognosis in many cardiovascular diseases, but assessment by two-dimensional transthoracic echocardiography is challenging. Here we show that a deep learning model, Dimensional Reconstruction of Imaging Data-Right Ventricle (DROID-RV), can estimate RV end-diastolic diameter and identify RV systolic dysfunction from transthoracic echocardiography videos. We trained DROID-RV in 17,141 ambulatory cardiology patients (51,188 studies and 383,881 video clips) and validated model predictions in 9,112 primary care patients and 831 patients with RV ejecti

Predicting adverse outcomes after cardiac surgery and structural interventions: the role of right ventricular function.

Right ventricular (RV) function is a major determinant of clinical outcomes in patients undergoing cardiac surgery and transcatheter interventions. Although left ventricular morphology and function have been the traditional focus of preprocedural assessment, RV dysfunction is now recognized as an important predictor of morbidity and mortality. Therefore, inclusion of RV-related parameters in preprocedural risk assessment is on the rise. This review summarizes current evidence on the role of RV function in various cardiac diseases that require surgery or interventions. Conventional two-dimensio

Right Ventricular Function and Mortality in Critically Ill Patients With Heart Failure and Cardiogenic Shock.

BACKGROUND AND OBJECTIVES: Right ventricular dysfunction (RVD) is associated with poor prognosis in patients with acute heart failure (HF), including cardiogenic shock (CS). We evaluated the association of RVD by transthoracic echocardiogram (TTE) with mortality in cardiac intensive care unit (CICU) patients with HF. METHODS: We conducted a retrospective cohort of patients with HF admitted to the Mayo Clinic CICU (2007-2018) who had a TTE within one day of admission. TTE-derived global assessment of right ventricular (RV) function was graded on a semi-quantitative scale. Outcomes included in-
