Traumatic vertebral artery dissection is an unusual event in children, but early recognition and treatment are important to prevent neurological deficits. The authors present a case of a 4-year-old boy with a traumatic extracranial vertebral artery dissection, who suffers from left arm monoplegia due to brachial plexus injury.

Vertebral artery dissection is one of the more frequent cerebral-vascular disorders in the young adult. The initial symptoms rarely consist of vertigo with clinical characteristics of Selective Monolateral Acute Vestibular Deficit Syndrome. The case is described of a patient, who arrived with intense rotatory vertigo associated with neurovegetative symptoms and spontaneous nystagmus, which we initially diagnosed as right neuronitis. About 48 hours later, the symptoms of vertigo disappeared spontaneously, and prevalently nuchal cephalea appeared. Since the symptoms were atypical and the otoneur…

We report the successful management of labor and delivery of a parturient with a history of spontaneous bilateral vertebral artery dissection. We also outline the reasons why the obstetric anesthetist should be aware of this condition as well as other cranio-cervical dissections.

A patient with one-and-a-half syndrome due to brainstem ischaemia is presented. The case emphasises the importance of attempting to establish the aetiology of this syndrome--in this case infarction secondary to spontaneous vertebralartery dissection--when making therapeutic decisions.

PURPOSE: Spontaneous dissection of the carotid and vertebral arteries represents a rare pathology. Its pathogenesis, probably multi-factoria … s study was to evaluate MR angiography and MR findings in a population of patients with carotid and vertebral artery dissections.
MATERIALS AND METHODS: We reviewed the MR angiography and MR exams of 15 patients (10 males, 5 females, average age 42.0, age range 30-55) with carotid and/or vertebral artery dissection; the serial MR follow-up studies were also examined.

Aortic artery dissection is a rare but well-recognized complication of Turner's syndrome. Isolated carotid or vertebral artery dissection has not previously been reported. The authors report the clinical and magnetic resonance imaging findings in a 30-year-old woman with Turner's syndrome who developed a high cervical spinal cord infarction with a Brown-Sequard syndrome owing to bilateral vertebral artery dissection. The diagnosis and management of the case is reviewed.

OBJECTIVES: Clinical data and neuroradiological findings of 19 patients with 20 vertebral artery dissections were analysed to describe the features of time of flight magnetic resonance angiography (MRA) for the diagnosis and follow up of this vascular disorder.
METHODS: All patients underwent a combined MRI and MRA protocol with 1.5 T scanners, using a three dimensional flow compensated gradient echo sequence for MRA. Duplex sonography was performed on all patients and selective angiography was available from 17 vertebral artery dissections.
RESULTS: MRI showed ischaemic lesions of the brain…

BACKGROUND AND PURPOSE: Intracranial vertebral artery dissection is an increasingly recognized cause of stroke. However, little is known about its natural history and clinical manifestations, and appropriate management protocol has not yet been established. This study was performed to clarify its clinical course and determine the best management protocol.
METHODS: This study is a retrospective clinical and radiographic review of 11 patients with 13 lesions who presented between 1990 and 1996. Patients with a history of trauma and those who presented with subarachnoid hemorrhage were excluded.…

BACKGROUND AND PURPOSE: We describe the diagnostic potential of duplex color-flow imaging in the evaluation of extracranial vertebral artery dissection.
METHODS: Twenty patients with 24 extracranial vertebral artery dissections (four bilateral) were examined with duplex ultrasonography over a period of 6 years. Color-flow imaging was carried out in 16 of these patients. In 15 patients (75%), the dissection was temporally related to trauma. Angiography was available in 18 patients, confirming the diagnosis.
RESULTS: In 15 patients, the diagnosis was primarily established with ultrasonography.…

The clinical features of headache and neck pain in 14 patients with extracranial vertebral artery dissection proven by angiography or magnetic resonance imaging are reported. Pain was always located on the side of the dissected vertebral artery. Whereas eleven patients had head and posterior neck pain, the others had either only posterior neck pain, no change of a chronic pre-existing headache or no pain at all. Pain started suddenly, was of sharp quality and severe intensity, different from any previously experienced headache. Following acute onset, the time course of pain was monophasic with…

A case of left vertebral artery dissection after neck manipulation associated with an ipsilateral thalamo-peduncular infarct by distal embolism is reported. The oculomotor signs observed are explained for the left eye by an intra-axial third nerve lesion, and for the right eye by a limitation of the horizontal gaze to the right and an upgaze palsy of supranuclear origin. The association of infra and supranuclear lesions without nuclear lesion in thalamo-peduncular infarcts is considered.

PURPOSE: Cervical radiculopathy due to various mechanical causes is commonly seen, however, cervical vertebral artery dissection (cVAD)-related radiculopathy is very rare with poorly characterized clinical outcomes. Thus, we conducted a systematic review of published literature and reported an institutional case to provide a better illustration of this rare entity.
METHODS: We systematically reviewed the PubMed literature and queried the clinical database at our center for cVAD-related radiculopathy. We described the baseline characteristics of patients with cVAD-related radiculopathy, the in…

Spontaneous vertebral artery dissection (VAD) is common after minor neck trauma or abrupt neck manipulation, which may result in stroke. However, violent cough is a less recognised cause of VAD. Only a few cases are reported in the literature. We review the literature and report a case of a 62-year-old man who presented with a headache and neck pain following a period of intermittent coughing, which led to VAD and a cerebellar ischaemic stroke. He responded well to medical treatment and made a full recovery. Therefore, VAD should be considered in the differential diagnosis for patients present…

Cervical spine manipulation is a known risk factor for vertebral artery dissection. In this report, we present a case of fatal vertebral artery dissection that occurred as the result of self-manipulation of the cervical spine. The decedent was a 40-year-old man with no significant past medical history. He was observed to "crack his neck" while at work. Soon after, he began experiencing neck pain, then developed strokelike symptoms and became unresponsive. He was transported to a local medical center, where imaging showed bilateral vertebral artery dissection. His neurological status continued …
