Ocular Misalignment in Dizzy Patients-Something's A-Skew.

BACKGROUND AND PURPOSE: Both central (eg, brain stem, cerebellum) and peripheral (eg, vestibular, fourth cranial nerve palsy) etiologies can cause a vertical misalignment between the eyes with a resultant vertical diplopia. A vertical binocular misalignment may be due to a skew deviation, which is a nonparalytic vertical ocular misalignment due to roll plane imbalance in the graviceptive pathways. A skew deviation may be 1 component of the ocular tilt reaction. The purposes of this article are (1) to understand the pathophysiology of a skew deviation/ocular tilt reaction and (2) to be familiar

[Neuro-otology: at the borders of ear and brain].

Vertigo and dizziness are frequent chief complaints in clinical practice. Symptoms may originate from otological, neurological, medical and psychiatric etiologies, which poses an interdisciplinary challenge. Systematic analysis of case history and clinical examination generally allow classification into peripheral-, central- or non-vestibular disorders. The most important criteria for differentiation are the timeline, quality of symptoms, modulating factors and accompanying symptoms. As concerns the clinical examination, the following tests are relevant: head impulse test, test for spontaneous

Urea test and vestibular dysfunction in suspected Ménière's disease.

Twenty-four consecutive patients underwent urea testing as part of a diagnostic work-up for vertigo and/or fluctuating hearing loss. Latent and positional nystagmus were evaluated by electronystagmography before and after urea ingestion. Of 10 patients with no nystagmus pre urea, only one exhibited slight latent nystagmus post urea. Of 14 patients with latent or positional nystagmus prior to the urea test, two lost their nystagmus, four showed marked decrease in slow component velocity, and eight were unchanged. No correlation could be made with cochlear response. Direction changing positional

Response to galvanic vestibular stimulation in patients with unilateral vestibular loss.

OBJECTIVES: This study sought to characterize various responses to galvanic vestibular stimulation (GVS) by comparing GVS-induced eye movements in healthy subjects and patients with vestibular function loss. The study also aimed to estimate the clinical significance of GVS tests. Finally, an effort was made to localize the primary excitation site of stimulation in the vestibular system. MATERIALS AND METHODS: Three parameters of response to GVS, spontaneous nystagmus, galvanic stimulating nystagmus (GSN), and postgalvanic stimulating nystagmus (PGSN), were evaluated in 20 normal subjects and

[Effect of caloric stimulation of the vestibular apparatus on hearing function].

Changes of bone-conduction thresholds for tonal signals (250 and 2000 Hz) in each car due to caloric vestibular tests were studied in 60 healthy test subjects (32 men and 28 women). These changes were found in 48 subjects. In most cases perceptual thresholds decreased (by 7.3 dB on the average) and in few cases increased (by 8.3 dB on the average). The sign of alteration was independent of the tonal vestibular test or the number of examinations. The threshold returned to the norm for a long time (30 min or more). These findings suggest that the interaction may be governed by the principles tha

'PREHAB': Vestibular prehabilitation to ameliorate the effect of a sudden vestibular loss.

A sudden unilateral loss or impairment of vestibular function causes vertigo, dizziness and impaired postural function. In most occasions, everyday activities supported or not by vestibular rehabilitation programs will promote compensation and the symptoms subside. As the compensatory process requires sensory input, matching performed motor activity, both motor learning of exercises and matching to sensory input are required. If there is a simultaneous cerebellar lesion found during surgery of the posterior cranial fossa, there may be a risk of a combined vestibulo-cerebellar lesion, with redu

Effect on vestibular function of cochlear implantation by partial deafness treatment-electro acoustic stimulation (PDT-EAS).

PURPOSE: Although the cochlear implantation procedure does not interfere with vestibular structures directly, both the vestibulum and the cochlea share the same inner ear fluid space, and this fluid may be responsible for transferring possibly damaging forces from one to the other. The purpose of the study is to assess postoperative vestibular function after partial deafness treatment-electro-acoustic stimulation (PDT-EAS) cochlear implantation. METHODS: Fifty-five patients were included in the study (30 females, 25 males, age 11-80, mean 41.8 ± 19.35). cVEMP and oVEMP were performed preopera

The influence of otolith dysfunction on the clinical presentation of people with a peripheral vestibular disorder.

BACKGROUND AND PURPOSE: Traditional vestibular function testing has measured horizontal semicircular canal function only. Otolith function tests have recently been developed, but their clinical significance has not been determined. The purpose of this study was to investigate the influence of otolith dysfunction on the clinical presentation of individuals with a peripheral vestibular disorder. SUBJECTS AND METHODS: Twenty-one subjects with loss of horizontal semicircular canal function only and 37 subjects with combined loss of horizontal semicircular canal and otolith organ function were rec

Evidence of vestibular and balance dysfunction in children with profound sensorineural hearing loss using cochlear implants.

OBJECTIVES/HYPOTHESIS: Similarities between the peripheral auditory and vestibular systems suggest that children with sensorineural hearing loss (SNHL) may demonstrate vestibular and balance impairments. This hypothesis was studied in 40 children with severe to profound SNHL and unilateral cochlear implants. STUDY DESIGN: Prospective cross-sectional study with repeated measures. METHODS: Vestibular function was assessed with caloric, rotational, and vestibular evoked myogenic potential testing; balance was assessed using the balance subset of the Bruininks-Oseretsky Test of Motor Proficiency

Total caloric eye speed in patients with vestibular migraine.

PURPOSE: Vestibular migraine is a common cause of dizziness that lacks a known objective test. This study examined total eye speed on caloric testing as a diagnostic marker for vestibular migraine. MATERIALS AND METHODS: Retrospective chart review of patients seen in a tertiary otologic practice between 2004 and 2016 who had undergone caloric testing with water irrigation and had a diagnosis of vestibular migraine (n=34). A group of patients with benign paroxysmal positional vertigo (n=10) were used as a control group. Patients were grouped into quartiles based on total eye speed. RESULTS: O

Drop attacks in elderly patients secondary to otologic causes with Meniere's syndrome or non-Meniere peripheral vestibulopathy.

Many neurologists are unaware of the drop attack that may occur from an inner ear dysfunction especially in elderly. We studied the clinical features and results of quantitative audiovestibular tests in six elderly patients (> or =65 years of age) who presented with drop attacks attributable to an inner ear pathology. Group was divided into Meniere's syndrome (4) or non-Meniere peripheral vestibulopathy (2). Standard dizziness questionnaire and quantitative audiovestibular function testing were performed. Episodes were described as a sudden push to the ground in four or a violent illusionary m

Vestibulospinal function in otoneurology.

The vestibulospinal level of vestibular function is commonly neglected in the evaluation of dizzy patients. Posturography provides a measuring technique for this function (static balance), and thus becomes a counterpart of electronystagmography, and proves to be an indispensable complement in the evaluation of a number of cases. It enables one to make a better assessment of the patient's condition and to obtain decisive indications for his treatment. Detailed analysis of the posturographic recording indicates several patterns of vestibulospinal dysfunction and allows one to evaluate the sensor
