A national survey of barriers and facilitators to medications for opioid use disorder among legal-involved veterans in the Veterans Health Administration.

Background: Medications for opioid use disorder (MOUD) are clinically effective at treating OUD among legal-involved populations. However, research shows that legal-involved veterans who receive care through the VHA have lower rates of MOUD use compared to non-legal-involved veterans. Education may be a key factor in intervention strategies to improve MOUD access. This study was a national survey of VHA staff to identify barriers to and facilitators of MOUD, as well as MOUD-related education needs for VHA staff, community partners, criminal justice partners, and legal-involved veterans. Method…

Phenome-wide association of 1809 phenotypes and COVID-19 disease progression in the Veterans Health Administration Million Veteran Program.

BACKGROUND: The risk factors associated with the stages of Coronavirus Disease-2019 (COVID-19) disease progression are not well known. We aim to identify risk factors specific to each state of COVID-19 progression from SARS-CoV-2 infection through death. METHODS AND RESULTS: We included 648,202 participants from the Veteran Affairs Million Veteran Program (2011-). We identified characteristics and 1,809 ICD code-based phenotypes from the electronic health record. We used logistic regression to examine the association of age, sex, body mass index (BMI), race, and prevalent phenotypes to the st

The hidden burden of social anxiety disorder in U.S. military veterans: Results from the National Health and Resilience in Veterans Study.

BACKGROUND: Social anxiety disorder (SAD) is a chronic and disabling psychiatric disorder associated with low levels of help-seeking. To date, however, scarce research has examined the epidemiology of SAD in veterans. This study examined the prevalence, comorbidities and incremental burden of SAD in relation to suicidality and functioning in a representative sample of U.S. military veterans. METHODS: A nationally representative sample of 3,157 U.S. veterans completed a web survey containing measures of SAD symptoms, trauma history, psychiatric history and functioning. Multivariable analyses w…

Characteristics and Health Needs of Veterans in Jails and Prisons: What We Know and Do Not Know about Incarcerated Women Veterans.

PURPOSE: The majority of U.S. veterans in prisons and local jails are men, but incarcerated women veterans remain an important and understudied group. METHODS: This study reported differences in sociodemographic, health, and criminal justice characteristics using Veterans Affairs (VA) administrative data on a national sample of 30,964 incarcerated veterans (30,440 men and 524 women) who received outreach from the VA Health Care for Reentry Veterans program between 2007 and 2011. Descriptive statistics and multivariable logistic regressions determined gender and racial differences in this popu…

A survey of gun collection and use among three groups of veteran patients admitted to veterans affairs hospital treatment programs.

BACKGROUND: An important risk factor for suicide is psychiatric illness, but only a limited amount of work has been directed at assessing the use of firearms and other weapons by select psychiatric populations at high risk for violent acts. METHOD: Patients with combat-related posttraumatic stress disorder (PTSD), patients with schizophrenia, and patients undergoing rehabilitation for substance abuse were asked to complete a weapons-use survey and measures of psychopathology. RESULTS: The PTSD patients surveyed related owning more than four times as many firearms as other subjects and report

Mortality and morbidity of members of the British Nuclear Tests Veterans Association and the New Zealand Nuclear Tests Veterans Association and their families.

More than 20,000 British servicemen--many of them on their National Service, few of them volunteering for the tests, and most in their early … s Island, were not irradiated. The Ministry of Defence still routinely issues a document to nuclear veterans who feel that their illnesses were caused by the radiation they encountered when they were young men which states: The background [radiation] dose received by civilians and members of HM Forces serving at or off Christmas Island in the years 1956 to …

Combat Trauma and Distress Symptoms Among Women Veterans.

BACKGROUND: The experiences of women veterans who served in combat and combat-support roles deserve much attention. The study aims to explore the differences in symptoms of post-traumatic stress disorder between women combatants and women in combat support roles. MATERIALS AND METHODS: This cross-sectional study included the files of 71 veterans' women (mean age 25.81 [SD = 5.243; Median = 24.50]), who served over 20 years, and who filled out questionnaires on admission for evaluation. The outcome variables included 2 measures of post-traumatic stress disorder (PTSD) [PTSD Checklist for DSM-5…

Blood Pressure Control and Mortality Among US Veterans.

BACKGROUND: Intensive blood pressure (BP) control reduces mortality and cardiovascular disease in clinical trials. However, real-world BP measurements often differ from standardized protocols. We evaluated the impact of real-world systolic BP on mortality among US Veterans. METHODS: We conducted a retrospective cohort study of Veterans with hypertension, defined by diagnostic codes, antihypertensive prescriptions, or ≥2 office BP readings ≥130/90 mm Hg in 2016 to 2017, with follow-up through March 2021. Systolic BP was treated as a time-dependent covariate and categorized into 7 groups: <110,…

Trauma, multimorbidity, and functional resilience in U.S. veterans.

BACKGROUND: U.S. military veterans frequently experience exposure to trauma and multiple co-occurring physical health and psychiatric conditions that can impair functioning. Nonetheless, some veterans maintain higher-than-expected levels of functioning despite these adversities, a pattern reflecting functional resilience. Identifying modifiable factors associated with such resilience may inform interventions to support optimal functioning in this population. METHODS: Data were analyzed from 4069 participants in the National Health and Resilience in Veterans Study, a nationally representative …

Immune transcriptomic changes in Australian Gulf War veterans.

BACKGROUND: Gulf War Illness (GWI) is a chronic multisystemic illness found in one-third of Gulf War Veterans. The aetiology of GWI is elusive; however, is strongly associated with exposure to multiple toxic agents, environmental exposures, and prophylactic medications or vaccinations. In the literature, disruption of the immune system and the presence of inflammation have been reported in GWI. In this novel study, we report gene expression-based analysis of a panel of 785 immune function related gene markers in GWI. METHOD: Ribonucleic acid (RNA) was extracted from peripheral blood mononucle…

Alzheimer's Disease Stage Transitions Among United States Veterans.

BACKGROUND: Alzheimer's disease (AD) and related dementias are progressive neurological disorders with stage-specific clinical features and challenges. An important knowledge gap is the "window of time" within which patients transition from mild cognitive impairment or mild AD to moderate or severe AD. Better characterization/establishment of transition times would help clinicians initiating treatments, including anti-amyloid therapy. OBJECTIVE: To describe cognitive test score-based AD stage transitions in Veterans with AD in the US Veterans Affairs Healthcare System (VAHS). METHODS: This r…

Cholangiocarcinoma (CCA) mortality in Vietnam war era veterans.

BACKGROUND: There is concern about the risk of cholangiocarcinoma mortality (CCA) among U.S. veterans who deployed to the Vietnam War theater. A variety of risk factors potentially related to Vietnam deployment may be associated with an increased risk of mortality from CCA. This study assessed the risk of CCA mortality among all Vietnam War era veterans, the first study to do so. METHODS: The Vietnam Era Veterans Mortality Study is a retrospective mortality study of all 2.5 million veterans who served in Vietnam and Southeast Asia (theater) and the 7.3 million veterans who served elsewhere du…

Self-injurious unnatural death among Veterans with HIV.

OBJECTIVE: People with HIV (PWH) are at an increased risk of suicide and death from unintentional causes compared with people living without HIV. Broadening the categorization of death from suicide to self-injurious unnatural death (SIUD) may better identify a more complete set of modifiable risk factors that could be targeted for prevention efforts among PWH. DESIGN: We conducted a nested case-control study using data from the Veterans Aging Cohort Study (VACS), a longitudinal, observational cohort of Veterans from 2006-2015. A total of 5036 Veterans with HIV, of whom 461 died by SIUD, were …

Risk of Dementia in Korean Vietnam War Veterans.

BACKGROUND: The number of cases of all types of dementia is increasing, and a significant increase in prevalence has been noted among veterans. Evidence of an association between dementia and exposure to chemicals such as Agent Orange from the Vietnam War is still limited, and there is a reported lack of awareness. OBJECTIVE: This study aimed to investigate the risk of dementia among Vietnam War veterans in Korea. DESIGN: This retrospective longitudinal study compared the incidence of dementia between Vietnam War veterans and the general population. SETTING: This study used data from the na…

Elective Hand Surgery and Antithrombotic Use in Veterans.

BACKGROUND: Patients undergoing plastic surgery have traditionally been instructed to avoid anticoagulants and antiplatelets during the perioperative period to avoid bleeding that could lead to painful hematomas, skin necrosis, unplanned procedures, and blood transfusions. Many veterans are currently prescribed anticoagulants for prevention of life- and limb-threatening embolic and thrombotic events. In early 2015, the plastic surgery service began to instruct patients undergoing elective hand surgery to stay on their prescription anticoagulant perioperatively. The objective of this study was …
