Patient-Ventilator Synchrony.

Patient-ventilator asynchrony develops when the ventilator output does not match the efforts of the patient and contributes to excess work of breathing, lung injury, and mortality. Asynchronies are categorized as trigger (breath initiation), flow (delivery of the breath), and cycle (transition from inspiration to expiration). Clinicians should be skilled at ventilator waveform analysis to detect patient-ventilator asynchronies and make informed ventilator adjustments. Ventilator overdrive suppresses respiratory drive and reduces asynchrony, while other adjustments specific to the asynchrony ar…

Time course evolution of ventilatory responses to inspiratory unloading in patients.

Inspiratory muscle unloading decreases ventilatory drive. In this study, we examined the time course of this effect in patients with chronic obstructive pulmonary disease receiving two modes of ventilatory support: pressure support ventilation (PSV), during which each cycle was assisted, and biphasic positive airway pressure (BIPAP), set up in such a manner that one spontaneous breath took place between two consecutive pressure-assisted breaths. The first breath following the switch from spontaneous breathing to PSV was associated with an increase in tidal volume (VT) and a drop in mean transd

No Evidence for the Efficacy of Slow-Paced Breathing as a Recovery Strategy After Sprint Interval Training.

PURPOSE: Breathing practices are increasingly adopted by athletes due to the potential benefits for recovery, performance, and well-being. Slow-paced breathing is believed to increase cardiac vagal activity and relaxation, potentially supporting adaptive processes. Sprint interval training, by contrast, elicits a strong stress response. Therefore, we hypothesized that adding slow-paced breathing to highly demanding sprint interval training would enhance recovery, as indicated by cardiac autonomic control and sleep metrics, ultimately contributing to greater performance gains. METHODS: Thirty-

Progressive Changes in a Distributed Neural Circuit Underlie Breathing Abnormalities in Mice Lacking MeCP2.

UNLABELLED: Rett syndrome (RTT) is a neurodevelopmental disorder caused by mutations in Methyl-CpG-binding protein 2 (MECP2). Severe breathing abnormalities are common in RTT and are reproduced in mouse models of RTT. Previously, we found that removing MeCP2 from the brainstem and spinal cord in mice caused early lethality and abnormal breathing. To determine whether loss of MeCP2 in functional components of the respiratory network causes specific breathing disorders, we used the Cre/LoxP system to differentially manipulate MeCP2 expression throughout the brainstem respiratory network, specifi

Responses to constant work rate bicycle ergometry exercise in primary pulmonary hypertension: the effect of inhaled nitric oxide.

OBJECTIVES: The purpose of this study was to investigate the responses of patients with primary pulmonary hypertension (PPH) to constant work rate exercise and to examine the effect of nitric oxide (NO) inhalation. BACKGROUND: Maximal exercise tolerance is reduced in PPH, but gas exchange responses to constant work rate exercise have not been defined. We hypothesized that increased pulmonary vascular resistance in PPH would reduce the rate of rise of minute oxygen consumption in response to a given work rate. Because NO may lower pulmonary vascular pressures in PPH, we also postulated that in

The occurrence of sleep-disordered breathing among middle-aged adults.

BACKGROUND: Limited data have suggested that sleep-disordered breathing, a condition of repeated episodes of apnea and hypopnea during sleep, is prevalent among adults. Data from the Wisconsin Sleep Cohort Study, a longitudinal study of the natural history of cardiopulmonary disorders of sleep, were used to estimate the prevalence of undiagnosed sleep-disordered breathing among adults and address its importance to the public health. METHODS: A random sample of 602 employed men and women 30 to 60 years old were studied by overnight polysomnography to determine the frequency of episodes of apne

Severe, Non-apneic Respiratory Dysfunction and Hypoxia following Generalized Convulsive Seizures.

OBJECTIVE: Sudden unexpected death in epilepsy (SUDEP) is a devastating consequence of some generalized convulsive seizures (GCS). Recent work has focused on seizure related apnea as a biomarker of SUDEP risk, frequently without characterizing the adequacy of non-apneic ventilation or identifying other dysfunctional breathing patterns. We hypothesized that GCS frequently induce immediate, severe, non-apneic respiratory dysfunction that can induce critical hypoxia and bradycardia and sought to characterize breathing patterns after GCS. METHODS: Adult patients admitted to an epilepsy monitoring

Breathing matters.

Breathing is a well-described, vital and surprisingly complex behaviour, with behavioural and physiological outputs that are easy to directly measure. Key neural elements for generating breathing pattern are distinct, compact and form a network amenable to detailed interrogation, promising the imminent discovery of molecular, cellular, synaptic and network mechanisms that give rise to the behaviour. Coupled oscillatory microcircuits make up the rhythmic core of the breathing network. Primary among these is the preBötzinger Complex (preBötC), which is composed of excitatory rhythmogenic interne

Experimental dyspnoea interferes with locomotion and cognition: a randomised trial.

BACKGROUND: Chronic respiratory diseases are associated with cognitive dysfunction, but whether dyspnoea by itself negatively impacts on cognition has not been demonstrated. Cortical networks engaged in subjects experiencing dyspnoea are also activated during other tasks that require cognitive input and this may provoke a negative impact through interference with each other. METHODS: This randomised, crossover trial investigated whether experimentally-induced dyspnoea would negatively impact on locomotion and cognitive function among 40 healthy adults. Crossover conditions were unloaded breat

[Oral respiration. Diagnosis and physiopathology].

The oral breathing can change the functional connections during the development of the facial bones of the children. All this causes an abnormal growth of the facial bones and of the dental structures. This work developed a new discipline to remove this problem.

Numerical simulations to determine the most appropriate welding and ventilation conditions in small enclosed workspace.

In order to improve arc welding work in a small enclosed workspace, numerical simulations were conducted to find the most appropriate welding and ventilation conditions, such as welding currents, hood position and flow rates with no blowhole formation. In the simulations, distributions of airflow vectors and fume concentrations were calculated for two hood opening positions: one faced a welder's breathing zone, the other a contaminant source. As a result it was predicted that a hood opening facing a breathing zone remarkably lowered the fume concentration in the breathing zone compared with th

Therapeutic effect of a cleft lip teat on infants with respiratory and feeding disorders: Two case reports.

RATIONALE: Existing research into the effects of teat application has mainly focused on its negative and positive influence on the development of the oral cavity. Our work demonstrates that apart from changing the setting of the articulatory organs, the teat can also affect the quality of breathing, eating and sleeping. PATIENTS CONCERNS: We described the cases of 2 children: a 19-month-old girl and a 2.5-month-old boy, who had breathing disorders due to withdrawal of the tongue and impaired food intake. INTERVENTION: The babies were bottled fed with a special teat for cleft lip patients to

Impact of structural dynamics on biological functions of flaviviruses.

Flaviviruses comprise a number of mosquito- or tick-transmitted human pathogens of global public health importance. Advances in structural biology techniques have contributed substantially to our current understanding of the life cycle of these small enveloped RNA viruses and led to deep insights into details of virus assembly, maturation and cell entry. In addition to large-scale conformational changes and oligomeric rearrangements of envelope proteins during these processes, there is increasing evidence that smaller-scale protein dynamics (referred to as virus "breathing") can confer extra f

Detection of free radicals in blood by electron spin resonance in a model of respiratory failure in the rat.

Previous work has suggested that a free radical mechanism is involved in some types of muscle fatigue and that there can be free radicals released extracellularly. Because muscle fatigue may be an important factor in respiratory failure, the authors tested the hypothesis that increased concentrations of free radicals could be detected in the blood of animals undergoing severe resistive loading to respiratory failure. An ex vivo spin trapping technique with alpha-phenyl-N-tert-butylnitrone (PBN) was used to investigate the possible formation of free radicals in systemic blood samples by electro

Effect of He-O2 breathing on blood gases and ventilation during exercise in normal man.

Temporal changes in ventilation (VI) and arterial blood gases after substitution of helium (He) for nitrogen were studied in normal man during constant load exercises of 14 min duration (30 and 90 W). An abrupt switch of helium for air breathing (protocol 1; 5 subjects), or vice-versa (protocol 2; 4 subjects), was made at the 7th min. Whatever the work loads, the effect of He appeared rapidly: higher values of VI (protocol 1) were observed throughout the 7 min period of He-O2 breathing, but were only significant (p less than or equal to 0.05) during the first minute after substitution at 90 W.

Advances in Positive Airway Pressure Treatment Modalities for Hypoventilation Syndromes.

RATIONALE: Positive airway pressure therapy for hypoventilation syndromes can significantly improve health-related quality of life (HR-QOL), healthcare costs, and even mortality. The sleep-disordered breathing in such individuals are quite complex and require sophisticated devices with algorithms that are designed to accurately detect and effectively treat respiratory events that includes hypoventilation, upper airway obstruction, lower airway obstruction, central apneas and central hypopneas and reduce the work of breathing while maintaining breathing comfort. OBJECTIVES: The therapeutic phy…

Identifying Degenerate Shear and Layer Breathing Modes in Few-Layer 1T-PtS2 by Circularly Polarized Raman Spectroscopy.

Quasi-covalent interlayer bonding in 1T-PtS2 gives rise to pronounced thickness-dependent optical and electronic properties, yet its impact on vibrational properties remains poorly understood. In particular, low-frequency interlayer vibrations probe directional restoring forces, but the shear and breathing components have not been independently identified in 1T-PtS2. In this work, nearly degenerate in-plane shear modes and out-of-plane layer-breathing modes in few-layer 1T-PtS2 are resolved using circularly polarized Raman spectroscopy. Helicity selection rules separate E-symmetry in-plane vib
