Xeroderma pigmentosum and lentigo maligna in identical twins.

Xeroderma pigmentosum (XP) is a rare autosomal recessive genodermatosis. Skin abnormalities result from an inability to repair UV-damaged DNA. Clinically, XP presents with early onset cutaneous changes (severe photosensitivity, actinic keratoses, and telangiectasias) and an increase of developing cutaneous malignancies beginning in early childhood, but lentigo maligna and melanomas are relatively rare. Here we report on homozygote twins in whom there was no positive family history. They showed subnormal physical growth. On ophthalmological examination, both had photophobia and decreased visual…

Convulsive episodes in patients with group A xeroderma pigmentosum.

OBJECTIVES: To clarify the incidence of convulsive episodes in patients with group A xeroderma pigmentosum (XPA). MATERIALS AND METHODS: By investigating the history of convulsive episodes of our 33 XPA patients through either their medical charts or direct interviews with their caretakers. RESULTS: Five patients had several episodes of afebrile convulsion at ages older than 12. With the exception of one patient who began to show convulsive episodes at 13, no other XPA patients exhibited febrile seizures. As far as our 33 XPA patients were concerned, 15% exhibited epilepsy, and 3% experience…

[Anesthesia in patient with Xeroderma Pigmentosum: case report.].

BACKGROUND AND OBJECTIVES: Xeroderma Pigmentosum is a rare, autosomal recessive disease characterized by the premature development of neoplasias due to an exacerbated hypersensitivity to UV radiation. These manifestations are due to DNA excision and repair mechanism damage. As compared to normal individuals, these patients have a 1000-fold increased risk for developing neoplasias on sun-exposed areas. This report aimed at describing the anesthetic management of a patient with Xeroderma Pigmentosum submitted to ophthalmologic surgery. CASE REPORT: Female patient, 7 years of age, with Xeroderma…

Resurfacing the dorsum of the hand in a patient with Xeroderma pigmentosum.

INTRODUCTION: Hepatitis D virus (HDV) infection is a serious health problem leading to cirrhosis and hepatocellular carcinoma (HCC). Despite evidence that zoonotic infections are associated with end-stage liver disease, brucellosis in patients with delta hepatitis related to liver disease has not been well characterized. So, we examined this relationship using recent hospital-based data. MATERIAL AND METHODS: We analyzed data from 96 delta hepatitis patients (mean age: 52.5 ±12.8 years; 50 male; 52 cirrhotics) and 117 (mean age: 50.4 ±7 years; 60 male) control subjects who were selected from

[Xeroderma pigmentosum: report of 6 cases in Dakar].

We report 6 cases of black Senegalese boys with xeroderma pigmentosum. They were between 2 and 16 year-old and presented features of hypersensitivity to UV (keratosis, lentigines, poikilodermia and photophobia). Our cases were remarkable by the early occurrence of squamous cell and basal cell carcinoma located in photoexposed sites causing the death of 5 of them. Xeroderma pigmentosum must be considered as the first preneoplastic genodermatosis.

Facial resurfacing in xeroderma pigmentosum with chemical peeling.

We describe our experience with two patients with xeroderma pigmentosum who underwent multiple trichloroacetic acid chemical peels. Trichloroacetic acid and phenol were used in one case. Until now numerous treatment modalities have been reported. Deep chemical peeling has not been reported before in patients with xeroderma pigmentosum. Chemical peeling is a simple procedure with less associated morbidity.

[Auditory brainstem responses in group A xeroderma pigmentosum].

Xeroderma pigmentosum has been known to result from disturbance in the repair of injured DNA caused by the ultra violet light. According to cell fusion studies, this disease is classified into 8 groups. Among these groups, group A (A-XP) shows the most severe type of neurological disturbance. Slowly progressive diffuse impairment both in the central and the peripheral nervous systems has been reported. Although hearing loss occurs in all the patients, few papers have described the chronological changes of this disability. This study aimed to clarify the development of audiological abnormalitie…

Hypoglycinaemia and psychomotor delay in a child with xeroderma pigmentosum.

Glycine is a nonessential amino acid that serves as both an inhibitory and an excitatory neurotransmitter. Hyperglycinaemia occurs in non-ketotic hyperglycinaemia, a primary defect in the glycine cleavage pathway, and as a secondary feature of several inborn errors of organic acid metabolism. However, specifically low levels of glycine have never been reported. Here we report a child with complementation group C xeroderma pigmentosum (XP) characterized by a splice donor mutation in the XPC gene, multiple skin cancers and specific and persistent hypoglycinaemia. He has cognitive delay, lack of …

hRAD30 mutations in the variant form of xeroderma pigmentosum.

The sauropterygian clade Plesiosauria arose in the Late Triassic and survived to the very end of the Cretaceous. Plesiosauria evolved the greatest species diversity of any marine reptile clade, attaining a global distribution. Plesiosauria consist of two clades, Rhomaleosauridae and Neoplesiosauria. Basal Neoplesiosauria have long necks with at least 30 cervicals, but show qualitative osteological evidence for a stiff neck. Here we quantify neck mobility in lateral, ventral, and dorsal directions based on finite element modeling of neck vertebrae from the Middle Jurassic plesiosaur Cryptoclidu

[Serum concentration of coenzyme Q in xeroderma pigmentosum].

Serum concentrations of CoQ were measured on 22 xeroderma pigmentosum (XP) patients. 10 severely handicapped patients and 32 healthy children and adults. CoQ levels in XP patients (0.5 +/- 0.16 microgram/ml) and handicapped patients (0.57 +/- 0.21 microgram/ml) were lower than those in healthy children and adults (0.73 +/- 0.2 microgram/ml). CoQ levels in XP patients tended to decrease with age and were extremely low in two bed-ridden patients (0.29 and 0.21 microgram/ml), suggesting that CoQ levels decreased with disease progression. We studied the clinical effects of CoQ therapy in 19 XP pat…

A simple method for diagnosing xeroderma pigmentosum variant.

Patients with xeroderma pigmentosum variant have been diagnosed based on a post-replication repair assay using their cells together with their clinical manifestations. We present here an alternative simple method for the diagnosis of xeroderma pigmentosum variant that measures three cellular markers for DNA repair by autoradiography, unscheduled DNA synthesis, recovery of RNA synthesis, and recovery of replicative DNA synthesis after ultraviolet irradiation. Fibroblasts from a patient are plated on three coverslips parallel with normal cells (control). Unscheduled DNA synthesis, recovery of RN…

Xeroderma pigmentosum variant cells are resistant to immortalization.

Xeroderma pigmentosum (XP) is a human repair-deficient disorder that is caused by mutations in any of eight genes (A-G, V). The genes for complementation groups A-G have been cloned fully or in part, but the gene for the XP variant (XPV) has yet to be cloned. The lack of progress with XPV is in large part due to the rarity of stably transformed cell lines. We have attempted to immortalize fibroblasts from several XPV patients to obtain cell lines with which to characterize this disease and clone the appropriate gene. We have found, as have other investigators, that this XP group is very diffic…

Xeroderma pigmentosum and nucleotide excision repair of DNA.

Nucleotide excision repair is a versatile strategy for removing DNA damage from the genome. Tremendous progress in understanding this process has been made in the last few years, and the field continues to develop rapidly. Exciting connections have emerged between nucleotide excision repair, transcription, and DNA replication, but many mysteries remain concerning the biochemical details of the mechanism, the connection with several human inherited syndromes, and the role of DNA repair in preventing cancer.

[Sleep disordered breathing in group A xeroderma pigmentosum].

We studied sleep disordered breathing (SDB) in 12 patients with group A xeroderma pigmentosum (XP) by means of respiratory inductive plethysmography (Respisomnograph:Nims) during polysomnographical examination. The subjects were 6 male and 6 female patients aged from 10 months to 25 years. Four out of the subjects had SDB:3 showed sleep apnea (apnea index ranged from 5.2 to 44.2/h) and 1 presented desaturation during sleep (desaturation time per total sleep time was 4.3%). All these patients were over 12 years. The patients below 14 years had mainly the central type of SDB, and the others aged…

Prenatal diagnosis of xeroderma pigmentosum and Cockayne syndrome.

In a study of fetal cells from a series of 12 pregnancies in ten families at risk for the ultraviolet light-sensitive, DNA repair-deficient diseases xeroderma pigmentosum (XP) and Cockayne syndrome (CS), we detected one XP and two CS homozygote fetuses. The diagnoses were confirmed by analysis of fetal skin fibroblasts or second amniotic samples after termination of the pregnancies. The measurement of ultraviolet light sensitivity and DNA repair depended on properties common to the seven excision repair-deficient XP complementation groups (A-G) and the two CS complementation groups (A, B). No …

Lamellar sclerokeratoplasty of epibulbar tumor in xeroderma pigmentosum.

A 10-year-old boy was followed up from the stage of biopsy-proven dysplastic conjunctival mass to carcinoma in situ. This carcinoma was managed by radical excision and lamellar sclerokeratoplasty. To our knowledge, this case represents the first reported successful management of conjunctival carcinoma in xeroderma pigmentosum.

Do we know the cause of xeroderma pigmentosum?

DNA repair in mammals consists of a large family of genes that encode a variety of mutually interacting gene products. These gene products coordinately locate and prepare damaged sites in chromatin for eventual excision and replacement and interact with transcriptionally active and replicating regions. Subsets of repair genes are represented by the complementation groups of xeroderma pigmentosum (XP), Cockayne syndrome, trichothiodystrophy and the ERCC series, which overlap one another to varying degrees. Cloned DNA sequences or whole chromosomes correct the UV-sensitive phenotype of XP by onl…
