Meeting Women Where They Are: Integration of Care As the Foundation of Treatment for At-Risk Pregnant and Postpartum Women.

Purpose In these times of rapidly changing health care policies, those involved in the health care of women, especially during the reproductive years, have a unique and daunting opportunity. There is great potential to positively impact women's health through focus on prevention, attention to addressing disparities, and new focus on the integration of behavioral health care in primary care settings. Description In this report from the field, we suggest that the integration of mental health care into other health services and addressing underlying social needs by partnering with community-based

Debriefing interventions for the prevention of psychological trauma in women following childbirth.

BACKGROUND: Childbirth is a complex life event that can be associated with both positive and negative psychological responses. When giving birth is experienced as particularly traumatic this can have a negative impact on a woman's postnatal emotional well-being. There has been an increasing focus on women's psychological trauma symptoms following childbirth, including the relatively rare phenomenon of post-traumatic stress disorder (PTSD), and the benefit of debriefing interventions to prevent this. In this review we examined the evidence for debriefing as a preventative intervention for psych

Eating disorders during pregnancy.

UNLABELLED: Eating disorders during pregnancy, once thought to be rare, occur in a significant number of women. The incidences of the major eating disorders-anorexia nervosa and bulimia nervosa-are increasing because of cultural pressures on the drive for thinness. Because the age range for these major eating disorders overlaps with the age range for reproductive function, it is not unusual for a clinician to encounter a pregnant patient with a major eating disorder. Eating disorders attributable to the pregnant state include pregnancy sickness, pica, and ptyalism. The diagnostic criteria, eti

A review of postnatal debriefing of mothers following traumatic delivery.

This paper reviews the literature on the effectiveness of the debriefing of mothers after giving birth and considers the recommendations in relation to health visiting practice. Studies suggest a correlation between negative birth experiences and depression in women and a small percentage of women meet the criteria for post-traumatic stress disorder (PTSD). Studies on debriefing were examined, some of which suggested that midwife-led debriefing after an operative birth was ineffective in reducing maternal morbidity and the possibility of contributing to emotional health problems could not be e

Paediatricians' attitudes about discussing maternal depression during a paediatric primary care visit.

OBJECTIVES: To assess paediatricians' beliefs about discussing maternal depressive symptoms during a paediatric visit, and methods paediatricians use to identify mothers with depressive symptoms. METHODS: In-depth telephone interviews were conducted with 23 primary care paediatricians from a practice-based research network. We asked a series of previously developed questions about discussing maternal depressive symptoms during a paediatric visit; methods used to identify mothers at risk; barriers encountered, and potential strategies to improve recognition and treatment of maternal depression

REM sleep and mood state in childbearing women: sleepy or weepy?

STUDY OBJECTIVES: To test the hypotheses that: 1) an increase in endogenous progesterone levels during the luteal phase of the menstrual cycle will alter REM sleep and mood state, and 2) a decrease in endogenous progesterone levels during postpartum will also alter REM sleep and mood state. DESIGN: A longitudinal descriptive study utilizing ambulatory polysomnography for two consecutive nights at seven time points. SETTING: Subject's homes. PARTICIPANTS: The first hypothesis was tested with 34 women studied during both the follicular and luteal phases of their menstrual cycle. The second hy

Unplanned pregnancy and subsequent psychological distress in partnered women: a cross-sectional study of the role of relationship quality and wider social support.

BACKGROUND: Research into the impact of unintended pregnancy on the wellbeing of women tends to focus on pregnancies ending in either termination or lone motherhood. Unintended pregnancy is common in partnered women, but little is known about the association between unintended pregnancy and postpartum affective disorders, such as depression and anxiety in this group. Poor relationship quality and lack of social support are considered risk factors for psychological distress (PD). We examined the association between unplanned motherhood and subsequent PD in partnered women, for whom evidence is

Intrapartum violence during facility-based childbirth and its determinants: A cross-sectional study among postnatal women in Tanzania.

BACKGROUND: Violence during childbirth indirectly contributes to maternal and neonatal morbidity and mortality. It also causes intrapartum health consequences such as prolonged labor, postpartum hemorrhage, and postpartum psychological problems, including postpartum depression, post-traumatic stress disorder, and other negative feelings that lead to a decreased desire for facility delivery and increase the events of home deliveries which reduce the quality of life. In Tanzania, several efforts have been made to promote respectful maternity care. However, violence during childbirth continues to

Factors Associated With Timing of Return to Intercourse After Obstetric Anal Sphincter Injuries.

INTRODUCTION: The impact of obstetric perineal trauma on timing of return to intercourse is unclear, although sexual desire is clearly decreased in these women. In addition, studies examining timing of return to intercourse are cross-sectional and therefore cannot delineate potential reasons that patients might delay return to intercourse. AIM: To identify factors associated with delayed return to intercourse after obstetric anal sphincter injuries. METHODS: This was a planned secondary analysis of a prospective cohort study of women sustaining obstetric anal sphincter injuries during delive

Past wounds, present bonds, future mothers: item-level analysis of perinatal risk profiles.

BACKGROUND: Perinatal depression is shaped by interacting psycho-relational risk factors-childhood adversity, current relational functioning, perceived social support, and prenatal attachment. Variable-centered approaches focused on isolated predictors may obscure how these dimensions combine within individuals to generate distinct vulnerability configurations. OBJECTIVE: To identify person-centered profiles of psychosocial functioning during pregnancy and to examine their association with depressive symptoms across the perinatal period. PARTICIPANTS AND SETTING: A sample of 774 pregnant wom

Selective serotonin reuptake inhibitors (SSRIs) in women of reproductive age: a systematic review of local formularies.

BACKGROUND: Depression is the second most common chronic condition affecting women of reproductive age; 23.4% of women enter pregnancy with depression and use of selective serotonin reuptake inhibitors (SSRIs) in pregnancy is often necessary for maternal wellbeing. However, SSRI use during pregnancy can cause congenital malformations, postpartum haemorrhage (PPH), and persistent pulmonary hypertension of the newborn (PPHN). In UK primary care, prescribing formularies are one medium by which prescribers are provided with local medicines advice. AIM: To review all local prescribing formularies

Foreign Nationality, Family Psychiatry History and Pregestational Neoplastic Disease as Predictors of Perinatal Depression in a Cohort of Healthy Pregnant and Puerperal Women during the COVID-19 Pandemic.

Background: Perinatal depression (PND) represents one of the most common mental disorders in the pregnancy and/or postpartum period, with a 5-25% prevalence rate. Our aim was to investigate predictors associated with PND in a cohort of pregnant and puerperal women based in an Italian setting during the COVID-19 pandemic. Methods: We retrospectively recruited 199 (55 pregnant and 144 puerperal) women, afferent to our Perinatal Mental Outpatient Service of Ancona (Italy). Participants were administered an ad hoc case-report form, Whooley Questions (WQ), the General Health Questionnaire-12 (GHQ-1

Allopregnanolone: The missing link to explain the effects of stress on tic exacerbation?

The neurosteroid allopregnanolone (3α-hydroxy-5α-pregnan-20-one; AP) elicits pleiotropic effects in the central nervous system, ranging from neuroprotective and anti-inflammatory functions to the regulation of mood and emotional responses. Several lines of research show that the brain rapidly produces AP in response to acute stress to reduce the allostatic load and enhance coping. These effects not only are likely mediated by GABAA receptor activation but also result from the contributions of other mechanisms, such as the stimulation of membrane progesterone receptors. In keeping with this evi

Neurobiology of peripartum mental illness.

At least one in seven pregnant or recently postpartum women will experience a mental illness such as an anxiety disorder, depressive disorder, or substance use disorder. These mental illnesses have detrimental effects on the health of the mother, child, and family, but little is known about the hypothalamic and other neural correlates of maternal mental health concerns. The transition to parenthood alone is a time of remarkable neural plasticity, so it is perhaps not surprising that current research is showing that maternal mental illness has unique neural profiles. Furthermore, the neural sys

Bipolar disorder differences between genders: special considerations for women.

The objective of this article is to review clinical differences between men and women with bipolar disorder. The secondary objective is to analyze the differences in adherence to medication between genders. Men usually present with manic episodes and have comorbid drug abuse, while women usually present with major depressive episode, the onset is often later, comorbidity of physical pathology is common and adherence to medication is greater than in men. In women who have an earlier onset of the illness and are single, the risk of nonadherence is higher than in other groups of women. There are

Childhood adversity, benevolent experiences, and perceived stress among pregnant women in Mexico: psychosocial determinants of maternal well-being in a middle-income setting.

BACKGROUND: Adverse childhood experiences (ACEs) are strongly associated with poor physical and mental health across the life course. Maternal ACEs have been linked to prenatal stress, hypertensive disorders, gestational diabetes, shorter gestation, lower infant birth weight, and increased risk of prenatal and postpartum depression. Benevolent childhood experiences (BCEs) and social support may buffer these effects; however, ACE-informed perinatal care remains uncommon in low- and middle-income settings. This study aimed to estimate the prevalence of ACEs and BCEs among pregnant women in Mexic

Psychometric properties of the Urdu version of the Postpartum Bonding Questionnaire (PBQ): a validation study among mothers with prenatal anxiety symptoms in Pakistan.

PURPOSE: Mother-infant bonding is crucial for the child's development and the parent's well-being, but research in this area in low- and middle-income countries is rare. We sought to evaluate the psychometric properties of the Urdu version of the Postpartum Bonding Questionnaire (PBQ) among women with prenatal anxiety symptoms in Pakistan. METHODS: This validation study used cross-sectional data within the context of a large randomized clinical trial, called Happy Mother, Healthy Baby (HMHB), conducted from April 2019 to October 2022 among women with at least mild prenatal anxiety symptoms re
