[Clinical research on suture anchor fixation for the treatment of avulsion fracture of tibial intercondylar eminence under arthroscopy].

OBJECTIVE: To evaluate the early clinical effects of suture anchor fixation for the treatment of avulsion fracture of tibial intercondylar eminence under arthroscopy. METHODS: From July 2012 to January 2015, 22 patients with the avulsion fracture of tibial intercondylar eminence were treated with arthroscopic suture anchor fixation. There were 12 males and 10 females, with an average age of 22.8 years old(ranged, 18 to 33 years old). Nine patients had the fractures in the right knee and 13 patients had the fractures in the left knee. The mean duration from injury to surgery was 6.8 days(range

Strain analysis of intra- and extra-medullary implants for fixing the subtrochanteric femoral fractures.

BACKGROUND: Intra or extra medullary fixation for subtrochanteric fractures (STF) is still controversial. And the strain on fracture site and implant have not been reported. The aim of our study is to evaluate the differences in strain at the medial and lateral cortex of the subtrochanteric region with PFNA, 'upside-down' LISS and PFLP fixing different types of subtrochanteric femoral fractures under static loads. HYPOTHESIS: Our hypothesis was that the PFNA was superior than LISS and PFLP for fixation of STF. However, PFNA also had some weakness. METHODS: Thirty composite femurs with PFNA,

Post-traumatic elbow osteoarthritis after radial head arthroplasty: Prevalence and risk factors.

BACKGROUND: One of the treatment options for comminuted radial head fractures is radial head arthroplasty (RHA), especially when the elbow is also dislocated. While the clinical outcomes of RHA have been well documented, the incidence of post-traumatic osteoarthritis after RHA and its risk factors are not well known. OBJECTIVE: To determine the incidence of post-traumatic elbow osteoarthritis (humeroulnar and humeroradial joints) after RHA and identify its risk factors. METHODS: All patients who underwent RHA at our hospital between 2008 and 2016 were included retrospectively. The inclusion

Conservative treatment of distal ulna metaphyseal fractures associated with distal radius fractures in elderly people.

INTRODUCTION: Although volar locking plates (VLPs) have been exclusively used for unstable distal radius fractures (DRFs), the optimal management of distal ulna fractures (DUFs) remains controversial. Internal fixation using a plate for DUFs might be challenging because of the presence of osteoporotic bone and fragile bone fragments in elderly patients. HYPOTHESIS: We hypothesized that a strategy including VLP fixation for DRFs and non-intervention for DUFs with early wrist mobilization would provide encouraging results in elderly patients. MATERIALS AND METHODS: Eighteen patients (17 women

The AO Pediatric Comprehensive Classification of Long Bone Fractures (PCCF).

Background and purpose - The AO Pediatric Comprehensive Classification of Long Bone Fractures (PCCF) describes the localization and morphology of fractures, and considers severity in 2 categories: (1) simple, and (2) multifragmentary. We evaluated simple and multifragmentary fractures in a large consecutive cohort of children diagnosed with long bone fractures in Switzerland. Patients and methods - Children and adolescents treated for fractures between 2009 and 2011 at 2 tertiary pediatric surgery hospitals were retrospectively included. Fractures were classified according to the AO PCCF. Seve

Extensor apparatus of the knee: anatomy and injury patterns.

The extensor mechanism of the knee is essential for maintaining normal daily function and for activities such as walking and athletic performance. It comprises the quadriceps muscles and tendon, patella and peripatellar fat pads, patellofemoral joint and retinacula, patellar tendon, and tibial tuberosity. Disruption of any component can result in pain, instability, or loss of active extension. Owing to its superficial location and continuous biomechanical loading, it is particularly vulnerable to acute trauma, repetitive microtrauma, chronic degeneration, and traction apophysitis in the pediat

Biomechanical design of a new proximal humerus fracture plate using alternative materials.

Comminuted proximal humerus fractures are often repaired by metal plates, but potentially still experience bone refracture, bone "stress shielding," screw perforation, delayed healing, and so forth. This "proof of principle" investigation is the initial step towards the design of a new plate using alternative materials to address some of these problems. Finite element modeling was used to create design graphs for bone stress, plate stress, screw stress, and interfragmentary motion via three different fixations (no, 1, or 2 "kickstand" [KS] screws across the fracture) using a wide range of plat

Comparison of Bending Stiffness between String of Pearls Plate-Bone Substitute Constructs with and without Bending Tees in a Fracture Gap Model.

OBJECTIVE: The aim of this study was to compare the bending properties of String of Pearls plate-bone substitute constructs with and without bending tees in the nodes over a simulated fracture gap. It is hypothesized that the constructs with tees will have higher bending stiffness. STUDY DESIGN: Acetal polymer tubes and 12-hole, 3.5-mm String of Pearls plates were used to create plate-bone substitute constructs simulating stabilization in a bridging fashion over a 45-mm gap. Twenty-four constructs were made with 12 containing tees in the nodes over the fracture gap. Single-cycle load-to-failu

Stacked Tantalum Cones as a Method for Treating Severe Distal Femoral Bone Deficiency in Total Knee Arthroplasty.

Severe bone loss in distal femur has traditionally been managed with bulk cementing, bone graft, metal augments, trabecular metal cones, and mega prosthesis. The authors present an alternative method using a stacked configuration of two tantalum cones to manage severe distal femoral bone loss. This study aims to evaluate the mid-term outcomes and possible complications in patients treated with this novel technique. We retrospectively analyzed the prospectively maintained records of 16 knees (in 16 patients) presenting with severe distal femoral bone deficiency due to septic loosening (5), gian

Total elbow arthroplasty: a radiographic outcome study.

OBJECTIVES: Total elbow arthroplasty (TEA) is becoming a popular alternative to arthrodesis for patients with end-stage elbow arthrosis and comminuted distal humeral fractures. Prior outcome studies have primarily focused on surgical findings. Our purpose is to determine the radiographic outcome of TEA and to correlate with clinical symptoms such as pain. MATERIALS AND METHODS: This is an IRB-approved retrospective review from 2005 to 2015 of all patients with semiconstrained TEA. All available elbow radiographs and clinical data were reviewed. Data analysis included descriptive statistics an

Predicting alignment after closed reduction and casting of distal radius fractures.

PURPOSE: We sought to independently validate the McQueen equation and LaFontaine's criteria as predictors of instability in a large series of distal radius fractures treated nonsurgically. In addition, we hypothesized that restoring the volar cortical integrity (ie, volar hook) would be another factor that would independently predict the maintenance of a closed reduction in a cast. METHODS: We screened 546 consecutive distal radius fractures with 168 meeting all inclusion criteria. Dorsal tilt, radial height, radial inclination, ulnar variance, and carpal malalignment were measured on initial

High-strength bone polyurethane adhesive with rapid curing for bone tissue injury repair.

Bone tissue adhesives have advantages such as preventing stress shielding and secondary surgical infections, fixing small bone fragments, easing operations, and enhancing wound adaptability. These methods can be applied for the traumatic repair of comminuted fractures. Currently, commercial tissue adhesives fail to meet the biological safety and mechanical strength requirements of bone tissue adhesives. To address this situation, we developed and screened a rapidly cured high-strength polyurethane bone adhesive. Research has shown that polyurethane bone adhesives have shorter curing times (236

A novel model of locking plate and locking spongious screw: a biomechanical in vitro comparison study with classical locking plate.

BACKGROUND: Locking plates are commonly used for the fixation of comminuted, periprosthetic and osteoporotic bone fractures. These plates are secured to the bone with screws, creating a stable connection with fixed angle between the plate and the screws. In this biomechanical in vitro study, our aim is to evaluate and compare the novel locking plate-locking spongious screw model with FDA approved classical locking plate. METHODS: Sawbone PCF-15 osteoporotic bone model was utilized to simulate osteoporotic bone conditions. Two screws were used to attach both the classical locking plate and the

A new technique for reconstruction of the proximal humerus after three- and four-part fractures.

The results of proximal humeral replacement following trauma are substantially worse than for osteoarthritis or rheumatoid arthritis. The stable reattachment of the lesser and greater tuberosity fragments to the rotator cuff and the restoration of shoulder biomechanics are difficult. In 1992 we developed a prosthesis designed to improve fixation of the tuberosity fragments in comminuted fractures of the proximal humerus. The implant enables fixation of the fragments to the shaft of the prosthesis and the diaphyseal fragment using screws, washers and a special toothed plate. Between 1992 and 20

Is Sliding Compression Necessary for Intramedullary Nailing Fixation of AO/OTA Type A3.3 Intertrochanteric Fracture?

OBJECTIVES: The intramedullary nail is considered the gold standard for treating AO/OTA type A3.3 intertrochanteric fractures. However, it still faces a significant rate of failure, mainly due to the critical factor of comminuted lateral wall defects leading to inadequate proximal sliding compression. The primary objective of this study is to investigate the requirement of sliding compression in the treatment of unstable AO/OTA type A3.3 intertrochanteric fractures. To achieve this, we conduct a comparative analysis between two approaches: InterTAN alone and proximal femoral anti-rotation blad
