The prion gene complex encoding PrP(C) and Doppel: insights from mutational analysis.

The prion protein gene, Prnp, encodes PrP(Sc), the major structural component of prions, infectious pathogens causing a number of disorders including scrapie and bovine spongiform encephalopathy (or BSE). Missense mutations in the human Prnp gene cause inherited prion diseases such as familial Creutzfeldt-Jakob disease. In uninfected animals Prnp encodes a glycophosphatidylinositol (GPI)-anchored protein denoted PrP(C) and in prion infections PrP(C) is converted to PrP(Sc) by templated refolding. Though Prnp is conserved in mammalian species, attempts to verify interactions of putative PrP bin

Psychological factors associated with pediatric asthma death: a review.

The preceding review of the literature on pediatric asthma death encompasses a broad range of emotional and psychological variables, some of which may ultimately prove to be meaningfully related to asthma death in childhood. However, the essential question of the role of psychological factors in pediatric asthma death remains unanswered. This area of investigation is still in its earliest stages, having relied heavily to date on retrospection, anecdotal recordings, and theoretical speculation. Additionally, conceptualization of the problem has been impeded not only by the remaining medical mys

To Flip or Not to Flip: Introducing the Universal Flippability Index (UFI) for Epigenetic and Damaged Nucleobases.

The catalogue of chemical modifications on DNA and RNA─from 5-methylcytosine (5mC) and its oxidized derivatives (5hmC, 5fC, 5caC), through oxidative damage lesions (8-oxoguanine, thymine glycol, FapyG, 5-hydroxyuracil), to the RNA epitranscriptomic mark N6-methyladenosine (m6A)─has outpaced our mechanistic understanding of how proteins read, write and erase these marks. Structural biology reveals a striking convergence: DNA glycosylases (UDG, hOGG1, TDG, AAG, MutY, NEIL, NTH1), methyltransferases (DNMT1, M.HhaI), TET dioxygenases and the m6A erasers FTO/ALKBH5 all employ a shared base-flipping

Caring, pragmatic and discriminatory: how private paediatric outpatient providers normalise unequal care for Roma-ethnographic findings from affluent Slovakia.

BACKGROUND: Healthcare providers may enact discriminatory practices while perceiving themselves as caring and pragmatic. Whilst anti-Roma discrimination in Central and Eastern European healthcare has been documented in hospital and emergency settings in poorer regions, less is known about routine outpatient care in affluent areas. This ethnographic study examines how private paediatric outpatient providers in Slovakia’s most affluent region normalise unequal care for Roma families through everyday practices rationalised as contextually appropriate responses. METHODS: The study was conducted be

"It's hard to trust an individual, it's easier to trust an image"-patients with low back pain want imaging as a means of coping with uncertainty.

BACKGROUND: Evidence-based guidelines for low back pain recommend against routine imaging. Despite these guidelines, many patients still see … lf-limiting nature of their LBP; the limited discourse about emotional outcomes was often linked to frustrations and fears. The beliefs expressed in interviews were not consistent with participants' questionnaire answers from months previously, suggesting that beliefs may shift over time. Editorial Commentary: The Pelvis is the Lowest Vertebral Level: Diagnostic Approach to Hip-Spine Syndrome.

The human pelvis represents a wonderful example of apparent idealistic simplicity overwhelmed by realistic complexity. Traditionally, the pelvis has been termed a "ring" linking the lower extremity to the spine via the sacroiliac joint. In essence, the pelvis is the lowest vertebral level-"the hip bone's connected to the spine bone." Thus, the law of parsimony seemingly applies in the diagnosis and management of both arthritic and nonarthritic hip and spine disorders in isolation or combination. However, an inverse Occam's razor is much more likely. The layered theory of hip disorders illustra

Should rescue ICSI be re-evaluated considering the deferred transfer of cryopreserved embryos in in-vitro fertilization cycles? A systematic review and meta-analysis.

BACKGROUND: Total fertilization failure represents a particularly frustrating condition for couples undergoing in vitro fertilization. With the aim of reducing the occurrence of total fertilization failure, intracytoplasmic sperm injection (ICSI) has become the first choice over conventional in vitro fertilization (IVF) procedures although evidence of improved results is still debated and its use in couples without male factor infertility is not recommended. Among the strategies potentially useful to promote the use of conventional IVF, we herein call attention to the late rescue ICSI, which c

The Experience of Tinnitus and Its Interaction With Unique Life Histories-Life Events, Trauma and Inner Resources Narrated by Patients With Tinnitus.

Background: The challenges facing people with chronic tinnitus include finding relief and rebuilding quality of life. However, previous traumatic episodes may influence adjustment and prolong suffering. Recovery implies reducing aggravating reactions and improving social roles, relationships and interests. Self-narratives about living with tinnitus have not yet received the attention they deserve in the research literature. Thus, the main goal of the present study was to illustrate how tinnitus suffering interacts with the participants' unique life histories. Method: Four women and one man (ag

BMC family practice integrated GP care for patients with persistent physical symptoms: feasibility cluster randomised trial.

BACKGROUND: Patients continue to suffer from medically unexplained symptoms otherwise referred to as persistent physical symptoms (PPS). General practitioners (GPs) play a key role in the management of PPS and require further training. Patients are often frustrated with the care they receive. This study aims to assess the acceptability of an 'integrated GP care' approach which consists of offering self-help materials to patients with PPS and offering their GPs training on how to utilise cognitive behavioural skills within their consultations, as well as assessing the feasibility of conducting

Evaluating the sterilizing effect of pyriproxyfen treated mosquito nets against Anopheles gambiae at different blood-feeding intervals.

Pyrethroid resistant malaria vectors are widespread throughout sub-Saharan Africa and new insecticides with different modes of action are urgently needed. Pyriproxyfen is a juvenile hormone mimic that reduces fecundity and fertility of adult Anopheles mosquitoes when used as a contact insecticide. A long-lasting insecticidal net incorporating pyriproxyfen is under development. As wild, host-seeking females may succeed in blood-feeding at different intervals after initial contact with mosquito nets the aim of this study was to determine the effect that age and gonotrophic status (nulliparous or

The use of skin grafts in postburn contracture release: a 10-year review.

Postburn scarring and contracture affecting function remain the most frustrating late complications of burn injury. Various techniques are used to release contractures; the choice depends on their location and/or the availability of unaffected skin adjacent to the contracture or elsewhere. A retrospective review was carried out of the case notes of patients who had skin grafting for the release of postburn contracture at the Burns Unit, City Hospital, Nottingham between May of 1984 and August of 1994 to evaluate the experience over this period. Information was obtained about the burn injury, c

Severe contractures of the proximal interphalangeal joint in Dupuytren's disease: combined fasciectomy with capsuloligamentous release versus fasciectomy alone.

Severe proximal interphalangeal joint contracture in Dupuytren's disease presents a frustrating problem for hand surgeon. Some surgeons argue for fasciectomy alone, avoiding violation of the proximal interphalangeal joint, which may prolong morbidity and result in permanent limitation of flexion; this loss of flexion can be more disabling than a mild flexion contracture. Others favor capsulotomy in addition to fasciectomy, especially for severe contractures, to obtain additional release, arguing that one cannot completely correct secondary contracture by fasciectomy alone. We performed a retro
