The n- vs. u-serration is a learnable criterion to differentiate pemphigoid from epidermolysis bullosa acquisita in direct immunofluorescence serration pattern analysis.

BACKGROUND: Serration pattern analysis of direct immunofluorescence (DIF) allows the differentiation of epidermolysis bullosa acquisita from other subtypes of pemphigoid. In daily practice its use is limited due to lack of experience and unfamiliarity. OBJECTIVES: To test the learnability of DIF serrated-pattern recognition in groups with various a priori levels of competence. METHODS: An online nversusu-test (www.nversusu.umcg.nl) was created, which contained 26 DIF images of the epidermal basement membrane zone, IgG stained and photographed with a magnification of × 40 and × 63. All images

Psychiatry and emergency medicine: medical student and physician attitudes toward homeless persons.

OBJECTIVE: The purpose of the study was to explore changes in medical students' attitudes toward homeless persons during the Psychiatry and Emergency Medicine clerkships. Simultaneously, this study explored attitudes toward homeless persons held by Psychiatry and Emergency Medicine residents and faculty in an attempt to uncover the "hidden curriculum" in medical education, in which values are communicated from teacher to student outside of the formal instruction. METHOD: A group of 79 students on Psychiatry and 66 on Emergency Medicine clerkships were surveyed at the beginning and end of thei

Patients from across Europe have similar views on patient-centred care: an international multilingual qualitative study in infertility care.

BACKGROUND: International patient centredness concepts were suggested but never conceptualized from the patients' perspective. Previously, a literature review and a monolingual qualitative study defined 'patient-centred infertility care' (PCIC). The present study aimed to test whether patients from across Europe value the same aspects of infertility care. METHODS: An international multilingual focus group (FG) study with 48 European patients from fertility clinics in Austria, Spain, the UK and Belgium, with deductive content analysis. RESULTS: All specific care aspects important to participa

States' experiences with loan repayment programs for health care professionals in a time of state budget cuts and NHSC expansion.

PURPOSE: The landscape of education loan repayment programs for health care professionals has been turbulent in recent years, with doubling of the funding for the National Health Service Corps (NHSC) and cuts in funding for some states' programs. We sought to understand how this turbulence is being felt within the state offices involved in recruiting clinicians to rural and urban underserved communities. METHODS: We conducted key informant telephone interviews with staff of state offices of rural health, primary care organizations, and/or related organizations within 28 diverse states to answ

Disabled children and adolescents may be outsiders in the community.

BACKGROUND: Most children with neurogenic bladder and bowel dysfunctions suffer from myelomeningocele and shunted hydrocephalus. Fewer such births and better treatment have led to more children reaching adulthood. Increased knowledge about their lived experiences can direct support to help them. AIM: The study aims to investigate how children and adolescents aged between 10 and 18 years old with neurogenic bladder and bowel dysfunction live their everyday life. METHODS: Hermeneutic phenomenology was appropriate to investigate the participants' experiences in depth. Thirteen qualitative inter

Living donor kidney transplantation in a global environment.

Live donor kidney transplantation has become a widely sought treatment by patients with end-stage renal failure. As the outcome for the genetically and emotionally related live donor transplants is the same, this review considers live kidney transplantation from the broad scope of current international practice. Unrelated live donor transplantation can now be performed for incompatible donor recipient pairs via a simultaneous paired kidney donation. However, acceptance of the scientific data that an unrelated live donor transplant can now be performed successfully should not be misconstrued as

How does the legal system respond when children with learning difficulties are victimized?

OBJECTIVE: To understand how the Swedish legal system perceives and handles mentally handicapped children who may have been victimized. METHOD: Twenty-two judicial districts in Sweden provided complete files on 39 District Court cases (including the Appeals Court files on 17 of these cases) involving children with learning difficulties or other handicaps as alleged victims of abuse, threat and neglect. The children (25 girls and 14 boys) averaged 11.8 years of age when first allegedly victimized. Sexual abuse was the most frequently alleged crime (33 cases). Court transcripts, court files and

An Interprofessional Entrustment Framework for Midwife-Led Labor Ward Teaching in Oman: A Modified Delphi Study.

Introduction: The labor ward is an important interprofessional setting for undergraduate obstetric education, where medical students work closely with midwives who contribute substantially to their clinical learning. However, this teaching role is often informal. In addition, what students should learn through midwife-led teaching and the level of supervision expected for these activities are often not clearly defined. This study, conducted in the Sultanate of Oman, aimed to establish consensus on intended learning outcomes (ILOs) for midwife-led labor ward teaching and the level of supervisio

From Metrics to Meaning in Neurological Rehabilitation: Clinicians' Perspectives on Digital Metrics of Upper Limb Functioning-A Focus Group Study.

BACKGROUND: Digital assessment technologies, such as optical motion capture and inertial measurement units, enable detailed kinematic analysis and continuous monitoring of upper limb activity in persons with neurological conditions. While such digital metrics of functioning are increasingly recognized in research, their uptake in clinical neurorehabilitation is limited. It remains unclear which digital metrics of functioning clinicians perceive as most meaningful and how these are integrated into patient-centered care. Understanding clinicians' information needs and reasoning processes is a pr

Delivering on person-centred fundamental care: how far have we (really) come? A 10-year retrospective.

The last decade has seen substantial efforts directed towards improving how fundamental care is delivered and experienced. This work was catalysed by landmark inquiries that demonstrated the devastating impacts of poor-quality fundamental care delivery. Much public and academic scrutiny ensued, including our own 2016 discussion paper, which argued that fundamental care was poorly delivered because it was invisible and devalued across healthcare systems. We proposed six strategies for overcoming this issue, ranging from re-conceptualising the value of fundamental care, to enhanced metrics and c

Stakeholder Perspectives on Health System Readiness for Integrating Artificial Intelligence into Public Health in Saudi Arabia.

Background/Objectives: Although artificial intelligence (AI) has the potential to strengthen disease surveillance, predictive analytics, preventive interventions, and population-level decision-making, its successful integration requires health systems to be adequately prepared across technical, workforce, governance, and organizational dimensions. Saudi Arabia's rapid digital health transformation provides a unique national context for exploring these readiness conditions. This study explored stakeholder perceptions of conditions relevant to health system readiness for integrating AI into publ

Together TO-CARE: A Novel Tool for Measuring Caregiver Involvement and Parental Relational Engagement.

Background: Preterm infants and their families face a challenging experience during their stay in the neonatal intensive care unit (NICU). Family-centered care emphasizes the importance of welcoming parents, involving them in their baby's daily care, and supporting the development of parenting skills. NICU staff should support parents in understanding their baby's needs and in strengthening the parent-infant bond. Although many tools outline what parents should learn, there is a limited structured framework to monitor their involvement in the infant's care. Tracking parental participation in d

Roles of dentistry in identifying and supporting individuals who have experienced gender-based violence: a scoping review.

BACKGROUND: Gender-based violence (GBV) is a significant public health concern affecting one in three women globally. GBV is deeply rooted in gender inequality and societal constructs of gender roles, which significantly contribute to its prevalence and impact on women. Injury to the head, face, and neck, including broken teeth and jaws, is common in GBV, as is dental neglect, indicating a role for dental care. However, the impact of maxillofacial injuries or poor oral health and the role dental professionals may play in supporting individuals experiencing GBV are often overlooked. This review
