BACKGROUND: The prevalence and clinical course of tuberculosis infection have not been well described in patients with systemic lupus erythematosus (SLE) in the United States.
OBJECTIVE: This study documents the demographic, clinical, and laboratory characteristics and outcomes of patients with SLE and latent tuberculosis infection (LTBI) or active TB in an ethnically diverse clinic.
METHODS: We conducted a retrospective review of clinical records of patients with SLE followed during 2005 in a county community hospital rheumatology clinic, with a large immigrant population. Clinical characte

It is recommended to evaluate the presence of latent tuberculosis infection (LTBI) before initiating antitumor necrosis factor alpha (anti-TNF) therapy for rheumatologic diseases. We aimed to present the follow-up results of 192 patients with rheumatologic diseases before anti-TNF therapy for LTBI. We enrolled 192 patients who were given anti-TNF therapy for their rheumatologic diseases between April 2005 and January 2008. The demographic characteristics of the patients were recorded. Chest X-ray was obtained and tuberculin skin test (TST) was performed in all patients before anti-TNF therapy.

In humans, Mycobacterium tuberculosis persists for long periods in a clinically latent state, creating a huge reservoir of 'silent' tuberculosis (TB) (roughly one-third of the global population) from which new cases continually arise. A prognostic marker for active TB would enable targeted treatment of the small fraction of infected individuals who are most at risk of developing contagious TB, contributing greatly to TB control efforts. Here, we propose that TB-specific interferon-gamma release assays might be useful for identifying individuals with progressive infections who are likely to dev

TB is a common and serious global infection that is spread exclusively from person to person. The initial infection in most healthy people leads to LTBI 95% of the time, but untreated individuals have a 5% to 10% lifetime risk for reactivating their infection to develop highly infectious cavitary pulmonary TB or extrapulmonary disease. Following primary infection progressive disease is more likely to develop in children younger than 5 years old or those who are immunocompromised, particularly those with HIV infection. The diagnosis of TB in most of the world depends on the presence of a clinic

Dermal tuberculosis is being rarely diagnosed and its incidence in Poland is less than I % of all registered cases of this disease. A case of the alcohol abusing patient with diagnosed tuberculosis ulcerosa of the anal fissure and pulmonary tuberculosis has been described. Bacteriological confirmation has been achieved using PCR and Bactec 460 methods. Complete healing of the cutaneous lesions and regression of radiological titers in lungs has been achieved as a result of applied typical antituberculotic treatment. Delayed diagnostics of the patient (6 months from occurrence of the cutaneous l

The diagnosis of infectious pulmonary tuberculosis was made in a 53-year-old Macedonian man. A careful examination of all persons in close contact with him according to the guidelines of the Swiss respiratory society was performed. Pulmonary tuberculosis was diagnosed in his 22-year-old son, in his 20-year-old daughter and in his 3-year-old granddaughter. Seven persons had isoniazid preventive treatment for latent tuberculosis. These data shows very clearly that a careful examination of all persons in close contact with contagious tuberculosis is very important.

BACKGROUND & OBJECTIVES: In human immunodeficiency virus (HIV) infected persons with pulmonary tuberculosis (TB), sputum may not always show acid fast bacilli (AFB). Moreover, in most cases of suspected extrapulmonary TB (irrespective of HIV status) mycobacteria-containing material is not readily available for investigation. This study evaluated whether blood culture for Mycobacterium tuberculosis bacteraemia (mycobacteraemia) help in diagnosing TB in such cases.
METHODS: A total of 93 consecutive subjects with a clinical diagnosis of tuberculosis with or without laboratory confirmation, 42 w

To determine the impact of information technology on embedding the latent tuberculosis infection (LTBI) screening guidelines in an electronic medical record (EMR) in a large health care system. The long-term goal of the study is to test clinician adherence to LTBI screening guidelines using an EMR system. However, preliminary results are presented on the potential impact on providers on implementing the alerts on targeted high-risk patients.

SETTING: Homelessness is an important risk factor for tuberculosis (TB). Health departments often fail to identify contacts for homeless TB cases, but little else is known about the outcome of contact investigations for these cases.
OBJECTIVE: To describe the outcomes of identification, tuberculin skin testing (TST), clinical evaluation and treatment for contacts of infectious homeless TB cases.
DESIGN: Retrospective multicenter review of data of contact investigations conducted in 1996 by five health departments in the United States.
RESULTS: Twenty-seven (8%) of 349 TB cases were homeless

BACKGROUND: Treatment of latent tuberculosis infection (LTBI) with isoniazid is recommended for transplant recipients with positive tuberculin skin test (TST). However, TST could be an imperfect identifier of LTBI in this population. In addition, the risk of isoniazid hepatotoxicity could be high in liver transplant recipients (LTR). A retrospective cohort study was performed to evaluate the diagnosis and treatment of LTBI in LTR.
METHODS: Charts of all 547 patients who received primary liver transplantation at a University Hospital in Spain between 1988 and 1998 were reviewed.
RESULTS: TST

OBJECTIVE: To analyze the usefulness of the Mantoux test in the diagnosis of genital tuberculosis in women of childbearing age.
METHOD: In this report, the investigators studied the ability of a tuberculosis (TB) Mantoux test to diagnose pelvic tuberculosis. A positive TB Mantoux test was clearly defined. The positive control group was of 100 women treated for TB (study group C). The negative control group was of 100 postpartum women (study group B). The study group was 100 infertile women undergoing laparoscopy, in some of whom the diagnosis of TB was made (study group A).

Bacille Calmette-Guérin (BCG) vaccine, a live vaccine developed to prevent tuberculosis (TB), has been given to billions of persons over more than 7 decades. Studies of the efficacy of BCG vaccine have had widely divergent results, underscoring the complexity of the biology and immunology of TB. The long duration of TB infection, the heterogeneity of its clinical expression, and lack of inexpensive, reliable markers of infection and disease have made it difficult to study the impact of a vaccine, especially in resource-poor areas. A meta-analysis of data from trials of BCG vaccine found that s

OBJECTIVE: To summarize relevant parts of the guidelines recommended by the Canadian and American Thoracic Societies for diagnosis and management of asymptomatic tuberculosis (TB) infection.
QUALITY OF EVIDENCE: The latest guidelines published by the Canadian and American Thoracic Societies were reviewed. Unfortunately, neither of these guidelines state explicitly how recommendations were derived. The references accompanying each set of guidelines, however, suggest that they were developed by extensive literature review of the subject and consensus among expert panels.

BACKGROUND: A change has recently been noted in the epidemiology of tuberculosis in the United States. Multiple factors, including human immunodeficiency virus (HIV) infection, increases in the homeless population, and immigration, are cited as causes for an increased prevalence. The population of pregnant women in New Orleans exhibits several of these risk factors and may be compared with a previous description of this group reported in 1983.
METHODS: All patients requesting obstetric care at the Medical Center of Louisiana at New Orleans from January 1994 to April 1995, were offered tubercu

SETTING: Domestic and international air-flights.
OBJECTIVE: To estimate the risk of tuberculosis (TB) transmission aboard aircraft.
DESIGN: A contact investigation of passengers and crew from two flights was conducted following identification of a fellow passenger with pulmonary TB. Immediate post-exposure and follow-up tuberculin skin tests (TSTs) were obtained.
RESULTS: Of 120 contacts, 86 (72%) had a negative TST (< 5 mm); 29 (24%) a positive TST (> or = 5 mm), and 5 (4%) a TST conversion. Of the 29 persons with a positive TST, 27 had other identified risk factors for TB. Risk factors fo

SETTING: Clinical features of human immunodeficiency virus (HIV)-associated tuberculosis depend upon the patients' residual immunity. An immune-dependent presentation has also been described at the histopathological level in many extra-pulmonary sites, but no descriptions have so far been made on the histopathology of HIV-associated pulmonary tuberculosis.
OBJECTIVE: To compare the histopathological features of pulmonary tuberculosis in HIV-infected subjects and seronegative patients.
DESIGN: We carried out a retrospective comparative study on 16 HIV-infected subjects and 16 seronegative pat

The ocular morbidity pattern in 2010 eyes of 1005 patients with active pulmonary and extrapulmonary tuberculosis was studied prospectively. A complete clinical (ocular and systemic) examination, radiologic evaluation, Mantoux test and sputum analysis for acid-fast bacilli were carried out in all patients. The ocular morbidity in patients in this study with active tuberculosis was 1.39%. The most common ocular finding was bilateral healed focal choroiditis (50%). No case of Eales' disease was found in this series. There was no statistically significant correlation between the ocular lesions and

After eight decades of decline, cases of tuberculosis (TB) have increased substantially since 1985. This increase is attributed largely to a reactivation of disease in persons who are coinfected with TB and human immunodeficiency virus (HIV). Multiple-drug resistant tuberculosis (MDR-TB) has a high mortality rate (70 to 90%), mostly occurring in individuals with HIV. Death often results within 4 to 16 weeks of diagnosis. The pathogenesis, screening, chemoprophylaxis, diagnosis, and treatment of persons coinfected TB and HIV is reviewed. Attention is given to infection control and occupational
