Infective endocarditis with symptomatic cerebral complications: contribution of cerebral magnetic resonance imaging.

BACKGROUND: Cerebral complications are well-identified causes of morbidity and mortality in patients with infective endocarditis (IE). Few s … ncluded ischaemic lesions (n = 25), haemorrhagic lesions (n = 2), subarachnoid haemorrhage (n = 5), brain abscess (n = 6), mycotic aneurysm (n = 7), vascular occlusion (n = 3) and cerebral microbleeds (n = 17). In 19/30 cases, neurological manifestations were observed before the diagnosis of IE. MRI was more sensitive than CT scan in detecting both clinically s …

[When genetics matters: an unusual case of left-sided hemiparesis].

We report the case of a 35-year-old woman who has been admitted to our emergency room because of the sudden onset of a left-sided hemiparesis. The physical examination showed disseminated teleangiectases on the upper and lower lip, on the mucosal surface of the tongue and on the skin. The cerebral CT scan presented a right-sided fronto-parietal lesion, more likely an abscess, confirmed on surgical removal. A meticulous family history showed the presence of similar clinical features among several family members, raising the certainty about the presence of the Rendu-Osler-Weber disease. This cas

[Value of thallium 201-SPECT in typing brain space-occupying lesions].

OBJECTIVE: This study was designed to assess the diagnostic accuracy of single photon emission computed tomography with Tl-201 (SPECT Tl-201) to establish the tumoral or non-tumoral nature of brain space occupying lesions in comparison with usual diagnostic techniques. MATERIAL AND METHODS: The study population consisted of 37 patients, 24 men (64.9 %) and 13 women (35.1 %), mean age 48 +/- 16 years. After establishing the clinical and radiological diagnosis of brain lesion, all patients underwent SPECT Tl-201, evaluating it only by subjective analysis and blinded to neuroestructural techniqu

Granulomatous amebic encephalitis in a patient with AIDS: isolation of acanthamoeba sp. Group II from brain tissue and successful treatment with sulfadiazine and fluconazole.

A patient with AIDS, treated with highly active antiretroviral therapy and trimethoprim-sulfamethoxazole, presented with confusion, a hemifield defect, and a mass lesion in the right occipital lobe. A brain biopsy confirmed granulomatous amebic encephalitis (GAE) due to Acanthamoeba castellanii. The patient was treated with fluconazole and sulfadiazine, and the lesion was surgically excised. This is the first case of AIDS-associated GAE responding favorably to therapy. The existence of a solitary brain lesion, absence of other sites of infection, and intense cellular response in spite of a ver

Meningeal gliomatosis presenting as multiple cerebral infarcts: a case report.

A 70-year-old man presented to the hospital with a history of progressive cognitive decline, and shortly after admission he developed sudden neurologic deterioration leading to coma. CT revealed multiple contrast-enhancing lesions within the cerebral hemispheres and midbrain tectum. Autopsy revealed a left occipital glioblastoma associated with widespread meningeal dissemination of tumor, occlusive vasculopathy, and multifocal infarcts.

Early postoperative seizures after posterior fossa surgery.

A review is reported of the seizure incidence in 726 patients who underwent 740 posterior fossa operations via a suboccipital craniectomy without prophylactic anticonvulsant agents. Thirteen patients (1.8%) experienced seizures within 2 weeks postoperatively. Five of these patients (0.7% of the series) had seizures within 24 hours after operation. The incidence was highest for patients with medulloblastoma (7.2%) followed by those with astrocytoma (2.3%). Also, a higher percentage was found in patients with preoperative ventriculoperitoneal shunt or intraoperative ventriculostomy (2.7%) than i

[Postoperative echographic surveillance].

Real time ultrasonography can be used in the post operative period through a bone defect such as a craniectomy or laminectomy when their surface excess 6 cm2. It permits without moving the patient the study of the normal post-operative evolution of certain lesions such as abscess or ventricule enlargement, and the diagnosis of early post-operative complications. Tardive unfavourable anatomic evolution and/or recurrence can also be studied. In the future, the miniaturization of the probe will allow to extend this technique to smaller bone window.

A challenging presentation of central nervous system involvement in disseminated coccidioidomycosis: Insights from cases.

BACKGROUND: Coccidioidomycosis is a fungal infection endemic to the southwestern United States that rarely involves the central nervous system (CNS) as a brain abscess or subdural empyema. These uncommon manifestations can mimic neoplastic or inflammatory lesions, making diagnosis particularly challenging. CASE DESCRIPTION: We report two cases of disseminated coccidioidomycosis with atypical CNS presentations and imaging findings. In the first case, a patient with pulmonary sarcoidosis presented with a right parietal lesion and subdural empyema initially suspected to represent neurosarcoidosi…

Case Report: Neurological examination, computed tomography, and magnetic resonance imaging in South American camelids and a dromedary.

INTRODUCTION: South American camelids (SACs) and dromedaries presenting with neurological abnormalities are increasingly common in veterinary practice. Advanced diagnostic imaging techniques, such as computed tomography (CT) and magnetic resonance imaging (MRI), are typically available only in specialized veterinary clinics, and their feasibility is often limited by the animal's size. We aim to describe CT and MRI findings in a cohort of SACs and dromedaries diagnosed with neurological diseases. METHODS: Medical records were retrospectively reviewed for SACs and dromedaries that underwent CT

A Rare Case of Pott's Puffy Tumor in an Adult With Paranasal Osteoma and Pachymeningitis.

Pott's puffy tumor (PPT) is a rare but potentially life-threatening complication of frontal sinusitis, most commonly seen in children and adolescents. We report an unusual case of PPT in a 63-year-old man with a history of intranasal cocaine use, complicated by pachymeningitis. The patient presented with severe right eye pain, periorbital swelling, erythema, and purulent discharge. Initial imaging revealed right orbital cellulitis and erosive paranasal sinus disease, with a suspected frontal subgaleal abscess. Brain MRI confirmed the diagnosis of PPT, demonstrating acute-on-chronic pansinusiti

Emphysematous pyelonephritis: Six case reports and review of literature.

BACKGROUND: Emphysematous pyelonephritis (EPN) is a severe acute necrotizing infection of the renal parenchyma and surrounding tissues that causes the presence of gas in the renal parenchyma, collecting system, or perinephric tissue and has a poor prognosis. EPN occurs primarily in people with diabetes mellitus (DM), but can occur in those without DM when the associated renoureteral unit is obstructed. CASE SUMMARY: We describe our experience with six patients who developed EPN. Five patients had DM, including one with diabetic ketoacidosis, one with multisystem involvements, including eye, l

Cefepime-induced encephalopathy in a COVID-19 patient: a case report.

Prolonged neurological symptoms such as "brain fog" and cognitive impairment have occurred after coronavirus disease 2019 (COVID-19) infection. In this report, we describe impaired consciousness caused by cefepime hydrochloride (CFPM) in a patient with cognitive sequalae of COVID-19. A 56-year-old male patient was diagnosed with penile abscess after COVID-19 infection, and a blood culture detected two drug-resistant Pseudomonas aeruginosa strains. Therefore, CFPM 2 g × twice/day was administered on day 71 after intensive care unit admission. Approximately 48 h after CFPM administration, the pa

Torcular pseudomass in a 14-month-old child: illustrative case.

BACKGROUND: "Torcular pseudomass," or redundant soft tissue in the torcular region, is not an infrequent incidental finding on advanced imaging of the brain in infants and young children. It was recently codified among pediatric neuroradiologists; however, its report in the pediatric neurosurgical community has not previously been elucidated. OBSERVATIONS: The authors present a case of a 14-month-old child who presented with fever and a first-time seizure. Computed tomography of the head suggested an epidural abscess; however, magnetic resonance imaging characteristics of the lesion were cons

Cefepime-Induced Encephalopathy.

Cefepime, a fourth-generation cephalosporin, remains an essential antibiotic targeting a broad spectrum of Gram-positive and Gram-negative organisms. However, it also remains an important, yet often unrecognized, cause of encephalopathy. We are here to discuss a case of a 74-year-old male with a common bile duct low-grade adenoma who presented to the hospital for lethargy. He was placed on intravenous cefepime for a Pseudomonas-infected hepatobiliary abscess. Approximately five days later, the patient's spouse reported acutely worsening cognitive changes. The cefepime level was significantly e

Skull base osteomyelitis with secondary cavernous sinus thrombosis: a rare presentation of an animate foreign body in the ear.

A 42-year-old woman presented with fever, left ear pain, restricted mouth opening, difficulty in swallowing and inability to open her left eyelid for a period of 10 days. She was treated with antibiotics for the same at a local medical facility; however, a sudden decrease in her left eye vision prompted her to visit our tertiary centre. Her history was insignificant except for having multiple left ear syringing for an insect removal 10 days before onset of her current symptoms. On examination, she had ptosis of the left eye with chemosis, dilated pupil with only perception of light and restric

Human disease caused by Streptococcus alactolyticus: a case report of native valve infective endocarditis and review of the literature.

Streptococcus alactolyticus, a member of the Streptococcus bovis/Streptoccus equinus complex, is primarily hosted in the gastrointestinal tract of animals and rarely of humans, with only scarce reports relating to human disease. We herein present a case of subacute infective endocarditis (IE) caused by S. alactolyticus in a 64-year old male with pre-existing mitral prolapse. Despite a 10-month history of low-grade fever and weight loss, the diagnosis of IE was triggered by left quadrant abdominal pain which revealed splenic infarcts on computed tomography. A definitive diagnosis of IE was subs
