Premature aortic smooth muscle cell differentiation contributes to matrix dysregulation in Marfan Syndrome.

Thoracic aortic aneurysm and dissection are life-threatening complications of Marfan syndrome (MFS). Studies of human and mouse aortic samples from late stage MFS demonstrate increased TGF-β activation/signaling and diffuse matrix changes. However, the role of the aortic smooth muscle cell (SMC) phenotype in early aneurysm formation in MFS has yet to be fully elucidated. As our objective, we investigated whether an altered aortic SMC phenotype plays a role in aneurysm formation in MFS. We describe previously unrecognized concordant findings in the aortas of a murine model of MFS, mgR, during a

The pancreaticoduodenal arteries in human foetal development.

Knowledge of the course of the pancreaticoduodenal arteries is of great importance in pancreatic surgery. Lack of care in the preparation of these vessels may lead to ischaemia or necrosis of the duodenum, the first loop of the jejunum, the head of the pancreas and even the liver, bile ducts and transverse colon. In such events, the surgeon would need to diagnose the course of the vessels and their anastomoses intraoperatively. Anatomical dissection in this special area diminishes the risk of early complications in the form of bleeding and late complications in the form of narrowing of the ana

Epigenetic Induction of Smooth Muscle Cell Phenotypic Alterations in Aortic Aneurysms and Dissections.

BACKGROUND: Smooth muscle cell (SMC) phenotypic switching has been increasingly detected in aortic aneurysm and dissection (AAD) tissues. However, the diverse SMC phenotypes in AAD tissues and the mechanisms driving SMC phenotypic alterations remain to be identified. METHODS: We examined the transcriptomic and epigenomic dynamics of aortic SMC phenotypic changes in mice with angiotensin II-induced AAD by using single-cell RNA sequencing and single-cell sequencing assay for transposase-accessible chromatin. SMC phenotypic alteration in aortas from patients with ascending thoracic AAD was exami

Comparison of Extended Arch Versus Hemiarch Replacement in Elderly Patients With Type A Aortic Dissection: The Shizuoka Kokuho Database.

OBJECTIVES: Extended arch replacement (EAR) and hemiarch replacement (HAR) are surgical options for type A acute aortic dissection (AAD). However, the effectiveness of EAR compared with HAR, particularly in elderly patients, remains unclear owing to its invasiveness and complications. This study aimed to compare the postoperative outcomes of EAR and HAR in elderly patients with type A AAD. METHODS: This retrospective cohort study used data from the Shizuoka Kokuho Database, a prefecture-wide, multi-institutional administrative claims database managed by the Shizuoka Prefectural Government. We

The rationale for a randomized trial for uncomplicated Type B aortic dissection.

Uncomplicated Type B aortic dissection (uTBAD) treatment is changing, and the evidence driving this change is scarce. The decision to offer upfront thoracic endovascular aortic repair (TEVAR) or medical therapy varies significantly among providers, driven more by individual preference than scientific evidence. The IMPROVE-AD trial is a randomized, multidisciplinary, prospective, controlled, superiority trial designed to compare treatment efficacy; functional outcomes; quality of life; and costs in patients with uTBAD undergoing upfront TEVAR or medical therapy with selective TEVAR for deterior

X-linked genetic associations in sporadic thoracic aortic dissection.

The male predominance in sporadic thoracic aortic aneurysm and dissection (TAD) suggests that the X chromosome contributes to TAD, but this has not been tested. We investigated whether X-linked variation-common (minor allele frequency [MAF] ≥0.01) and rare (MAF <0.01)-was associated with sporadic TAD in three cohorts of European descent (Discovery: 364 cases, 874 controls; Replication: 516 cases, 440,131 controls, and ARIC [Atherosclerosis Risk in Communities study]: 753 cases, 2247 controls). For analysis of common variants, we applied a sex-stratified logistic regression model followed by a

Comparison between Zone 2 and Zone 3 distal anastomoses for aortic arch replacement in terms of invasiveness.

OBJECTIVES: Zone 2 anastomosis with total cervical branch reconstruction for acute type A aortic dissection and aortic arch aneurysms became possible after stent-graft introduction. This may be an easier procedure and reduce the risk of recurrent laryngeal nerve palsy. Therefore, this study aimed to compare the outcomes between Zone 2 and Zone 3 distal anastomoses. METHODS: After evaluating the patient data in our institute between April 2016 and April 2022, the patients in whom distal anastomosis was performed at Zone 2 with a stent-graft were defined as the Zone 2 group (n = 70). Evaluation of Aortic Zone 2 Proximal Landing Accuracy During Thoracic Endovascular Aortic Repair Following Carotid-Subclavian Revascularization.

BACKGROUND: Adequate seal for thoracic endovascular aortic repair (TEVAR) commonly requires landing in zone 2, but can prove to be challenging due to the tortuous and angulated anatomy of the region. OBJECTIVES: Our objective was to determine the proximal landing accuracy of zone 2-targeted TEVARs following carotid-subclavian revascularization (CSR) and its impact on clinical outcomes. METHODS: Retrospective review of patients that underwent CSR for zone 2 endograft delivery at a tertiary institute between January 2008 and March 2018 was conducted. Simple Renal Cysts Are Associated With 24-Month Prognosis of Patients With Type B Aortic Dissection and Hypertension.

BACKGROUND: The association of simple renal cyst (SRC) with type B aortic dissection (BAD) has recently been established. However, no studies have examined adverse events after BAD hospitalization among patients with SRC. In this study, we assessed the prognostic value of SRC in BAD patients with hypertension after thoracic endovascular aortic repair (TEVAR). METHODS: We studied all BAD patients with hypertension who were admitted for TEVAR (n = 238; age 56.1 ± 9.8 years, 84.0% male). Aortic-related adverse events (ARAE) were evaluated as outcomes at 3 months and 24 months after TEVAR. RESUL

Surgical outcomes for acute type A aortic dissection with aggressive primary entry resection.

OBJECTIVES: An entry located at aortic arch in acute type A aortic dissection (AAAD) is uncommon. It remains controversial whether or not aggressive primary entry resection should be routinely performed in such patients. We have adopted an aggressive strategy of entry site resection, including total arch replacement (TAR) in patients with arch tears. The purpose of this study was to investigate the efficacy of our surgical management approach, using aggressive primary entry resection. METHODS: Between January 2000 and December 2014, we retrospectively reviewed the records of 334 patients with
