The effects of nutrition on mesenteric oxygenation among neonates with neonatal encephalopathy: a randomized clinical trial.

BACKGROUND: We aimed to evaluate the effects of minimal enteral nutrition (MEN) on mesenteric blood flow and oxygenation with Doppler USG and Near Infrared Spectroscopy (NIRS) during therapeutic hypothermia (TH) in babies with HIE. METHODS: This prospective, randomized-controlled study was composed of infants receiving MEN (study group, n = 30) and infants who were not fed (control group, n = 30) during hypothermia. Infants were monitored continuously with NIRS and mesenteric blood flow velocities were measured with Doppler USG before and after feeding. RESULTS: The mean gestational age and

Mode of delivery and neonatal outcomes in preterm twins less than 32 weeks of gestation or birthweight under 1500 g: a systematic review and meta-analyses.

BACKGROUND: The mode of delivery for twins born before 32 weeks of gestation remains controversial. Our purpose is to conduct a meta-analysis of twin pregnancies less than 32 weeks or twin weight less than 1500 g, so as to find a suitable delivery mode. METHODS: We searched PubMed database, Cochrane Library database, and EMBASE database through December 2022. This protocol was registered with PROSPERO (CRD42023386946) prior to initiation. Studies that compared vaginal delivery to cesarean section for newborns less than 32 weeks of gestation or birthweight under 1500 g were included. The prima

Don't Forget the Bones: Incidence and Risk Factors of Metabolic Bone Disease in a Cohort of Preterm Infants.

Metabolic bone disease of prematurity (MBD) is a condition of reduced bone mineral content (BMC) compared to that expected for gestational age (GA). Preterm birth interrupts the physiological process of calcium (Ca) and phosphorus (P) deposition that occurs mostly in the third trimester of pregnancy, leading to an inadequate bone mineralization during intrauterine life (IUL). After birth, an insufficient intake of Ca and P carries on this alteration, resulting in overt disease. If MBD is often a self-limited condition, in some cases it could hesitate the permanent alteration of bone structures

The Role of Laparoscopy in the Acute Neonatal Abdomen.

IntroductionThe surgical treatment of the acute neonatal abdomen still poses a challenge in pediatric surgery. Various underlying etiologies require different surgical procedures. Until today the role of laparoscopy in the surgical treatment of the acute neonatal abdomen is controversial. The aim of this study was to analyze our experiences with laparoscopy and to perform a review of the literature. Methods Retrospective, single-institution study including all term and preterm neonates initially undergoing laparoscopy due to an acute abdomen. Results Altogether, 17 neonates presenting with an

Copper deficiency presenting as metabolic bone disease in extremely low birth weight, short-gut infants.

Copper deficiency can cause bone lesions in infants, which might be confused with child abuse. Two extremely low birth weight preterm infants had complicated medical courses requiring prolonged parenteral nutrition for short-gut syndrome, which led to the development of cholestasis. Both had spent their entire lives in the hospital. They had been on prolonged ventilator support for chronic lung disease. They developed signs of copper deficiency between 5 and 6 months of age, initially raising child abuse concerns. Musculoskeletal discomfort led to the recognition of radiographic findings of me

Probiotics for prevention of necrotising enterocolitis: an updated meta-analysis.

Several randomized controlled trials (RCTs) have investigated the prophylactic use of probiotics in preterm infants aimed at reducing the rate of necrotising enterocolitis (NEC). There are 4 meta-analyses on this subject. 2 more RCTs have been published since these meta-analyses were completed. Each meta-analysis, as well as the 2 recent RCTs, document reduced rates of NEC and mortality with the use of prophylactic probiotics. We calculated meta-analyses based on 3 approaches: A - RCTs common to all meta-analyses, B - RCTs ever accounted for in a meta-analysis but not common to all, and C - th

Predicting Retinopathy of Prematurity in Thailand: A Practical Model for Risk-Based Screening.

Background/Objectives: Retinopathy of prematurity (ROP) screening must balance timely detection with feasibility, particularly in resource-limited settings. We aimed to develop a pragmatic, locally derived prediction model to support prioritization of ophthalmologic assessment among Thai neonates already eligible for ROP screening. Methods: We conducted a retrospective cohort study of neonates undergoing their first planned ROP screening examination at Chiang Rai Prachanukroh Hospital (2011-2015) and Lampang Hospital (2021-2022). Prespecified predictors included gestational age, birth weight,

Machine learning-based prognostic modeling of early clinical outcomes in very low birth weight infants: insights from a nationwide cohort on delivery room resuscitation, prematurity-related complications, and mortality.

BACKGROUND: Very low birth weight infants (VLBWIs) are at high risk of requiring immediate neonatal resuscitation and developing serious complications or mortality. Early prediction based on prenatal information could improve perinatal preparedness and clinical outcomes. We aimed to develop and evaluate machine learning models that predict the need for delivery room resuscitation, major morbidities, and mortality in VLBWIs using prenatal variables. METHODS: In this population-based cohort study, we included 21,231 VLBWIs from the Korean Neonatal Network (2013–2023). Nineteen prenatal and perin

Development of qPCR-PMAxx™ and ddPCR-PMAxx™ methods for quantitative detection of viable but not culturable Cronobacter sakazakii on stainless steel surfaces after desiccation and exposure to commercial disinfectant.

Cronobacter spp. is an opportunistic foodborne pathogen responsible for life-threatening infections such as meningitis and necrotizing enterocolitis in neonates, but also for various complications in elderly and immunocompromised people. Environmental monitoring in powdered milk manufacturing plants shows that Cronobacter can persist in production environments for long periods of time. The aim of this study is to develop a method to assess the viability of Cronobacter cells after implementation of stress encountered in industries to determine whether this bacterium can persist in processing en
