Sacral neuromodulation: No more skiing?

Patients with spinal cord injury (SCI) commonly suffer from neurogenic lower urinary tract dysfunction (NLUTD). Sacral neuromodulation (SNM) offers an alternative in the treatment of detrusor overactivity in patients with NLUTD. We report the cases of three patients with NLUTD due to SCI who received SNM. Due to treatment success, all patients could resume skiing. All suffered from skiing accidents, leading to a decreased effectiveness of SNM. Subsequent evaluation revealed a defect of the impulse generator (IPG) and/or dislocation of the electrodes. Reprogramming or replacement of the IPG or

Complete mesh migration into the small bowel after incisional hernia repair: a case report and literature review.

INTRODUCTION: Mesh migration into the intestine is very rare after incisional hernia repair. CASE REPORT: We report the case of transmural mesh migration from the abdominal wall into the small bowel presenting as recurrent small bowel obstruction 18 years after repeated surgical repair of an incisional ventral hernia. At surgery, a mesh was found inside the resected ileal loop. DISCUSSION: Mesh migration into the intestine is a possible, although very rare, complication after incisional hernia repair with nonabsorbable meshes. It tends to occur late with obstructive symptoms, especially if t

Transnasal Penetration of a Ballpoint Pen: Case Report and Review of Literature.

BACKGROUND: Transnasal penetration by a nonmissile foreign body is a rare injury. Consequently, appropriate management remains controversial. We report a case of transnasal penetration by a ballpoint pen and review the literature. To our knowledge, this is the first living patient who sustained carotid artery damage from a transnasal penetrating intracranial injury. CASE DESCRIPTION: A 56-year-old female presented with a ballpoint pen lodged through her left nostril. She exhibited right cranial nerve palsies (III, IV, VI, and V1). A computed tomography (CT) scan of the head revealed a foreign

Spontaneous migration of a portacath into the azygos vein with subsequent development of a tracheo-azygos fistula.

Subcutaneous venous port or 'portacath' devices are widely used, especially in oncology patients. Several potential complications of portacath devices have been well documented. We report a rare case of the spontaneous migration of the tip of a portacath into the azygos vein, with subsequent development of a tracheo-azygos fistula. This resulted in the patient receiving a dose of chemotherapy agent and saline directly into his lungs, which ultimately contributed to his death a few weeks later. To our knowledge, there have been only 10 previously reported cases of inadvertent intrathoracic (med

A novel balloon assisted two-stents telescoping technique for repositioning an embolized stent in the pulmonary conduit.

A 9-year-old male, with history of pulmonary atresia and ventricular septal defect, status post complete repair with a 16 mm pulmonary homograft in the right ventricular outflow tract (RVOT) underwent 3110 Palmaz stent placement for conduit stenosis. Following deployment the stent embolized proximally into the right ventricle (RV). We undertook the choice of repositioning the embolized stent into the conduit with a transcatheter approach. Using a second venous access, the embolized stent was carefully maneuvered into the proximal part of conduit with an inflated Tyshak balloon catheter. A seco

Dental implant migration in grafted maxillary sinus.

BACKGROUND: Migration of oral implants through a previously consolidated graft is an extremely rare event. We reported 2 cases of migration at least 6 months postimplantation in sinus grafted in 2 stages, consequently over 6-month graft maturation period. CASE DESCRIPTION: Sinus elevations and implants placement in 2 patients were performed. Clinical and radiographic follow-up after the prosthetic rehabilitation showed displacement of 1 implant in each patient through the histopathologically verified stable grafts. CLINICAL IMPLICATIONS: Implants migration through composite graft after maxil

Recurrent strokes caused by a malpositioned pacemaker lead.

This case report illustrates the case of a patient who developed recurrent strokes after a pace maker lead was inserted into his left ventricle. It was removed successfully by the cardiothoracic surgeons but he remained very dependent functionally. This case highlights the importance of always reviewing the electrocardiogram and chest radiograph after the insertion of a pacemaker as late diagnosis of this complication can leave the patient with significant morbidity.

Management of recurrent airway strictures in lung transplant recipients using AERO covered stents.

The potential role of AERO tracheobronchial covered stents in the management of recurrent postinfectious strictures of the bronchus intermedius was studied in three lung transplant recipients. Six devices were inserted. Five of the stents migrated immediately on placement. Buildup of thick mucus was observed in all stents remaining in the airway for longer than 1 week. Strictures recurred in all patients 1, 3, and 5 months after stent deployment. Attempts at stent retrieval were successful for three of five devices. The use of AERO stents may not offer a therapeutic advantage versus balloon di

Complications related to percutaneous transarterial embolization of intracranial dural arteriovenous fistulas in 40 patients.

BACKGROUND AND PURPOSE: An increasing number of intracranial dural arteriovenous fistulas (DAVFs) are amenable to endovascular treatment with Onyx. However, reports on complications caused by this technique have been limited. We present the initial Beijing Tiantan Hospital experience with adverse events related to transarterial Onyx embolization for DAVFs. MATERIALS AND METHODS: Between September 2005 and February 2008, a total of 40 patients with DAVFs were treated at our institute with Onyx-18. There were 11 women and 29 men with a mean age of 43.15 years (age range, 23-60 years). We review

Management of pericatheter cerebrospinal fluid leak after intrathecal implantation of a drug delivery system.

Persistent cerebrospinal fluid leak is a known complication of intrathecal pump system insertion for drug delivery. Various treatment modalities, such as an epidural blood patch, use of glue, removal of catheter, and surgical closure of dura and subdural blood patch, have been reported previously. This report presents a 35-year-old woman in whom an intrathecal pump system was inserted for cancer pain management with intrathecal morphine. This was complicated by a persistent cerebrospinal fluid leak that was successfully managed by purse-string sutures over the dura around the catheter, without
