Heart beat modulation of spontaneous otoacoustic emissions in guinea pig.

It has been reported that background variation in the frequency of spontaneous otoacoustic emission (SOAEs) may arise from the cardiovascular system and that the side-bands in the spectra of SOAEs may be modulated by heartbeat. For better understanding of the mechanical influence of the cardiac cycle on cochlear functions under physiological and pathophysiological conditions, this study investigated heartbeat-induced modulation of SOAEs in guinea pig. Possible mechanisms of these phenomena are also discussed. The external ear canal acoustic signal, round window electric signal, and the ECG wer

Catheter related bacterial infections mimic reactions to exogenous pyrogens during hemodialysis.

Pyrogenic reactions are characterized by fever, chills, hypotension, or a combination of these developing during or shortly after hemodialysis in a previously asymptomatic patient. The temporal association with treatment implicates exposure of the patient's blood to bacterial pyrogens from contaminated dialysate or a reused dialyzer. Routine body temperature monitoring is recommended to detect these exposures. The current study was prompted by the appearance of several symptomatic febrile episodes in patients who were asymptomatic and afebrile before treatment with high-flux hemodialysis. Duri

Revised technique of cardiopulmonary bypass in one-stage repair of interrupted aortic arch complex.

Twenty infants with interrupted aortic arch associated with various intracardiac anomalies underwent primary complete repair using pulsatile high-flow cardiopulmonary bypass with a short period of circulatory arrest. Age at repair ranged from 11 to 126 days (mean age, 43 days). Weight ranged from 2.2 to 5.5 kg (mean weight, 3.4 kg). Associated cardiac lesions included ventricular septal defect (14 patients), truncus arteriosus (3), transposition of the great arteries (2), and aortopulmonary window (1 patient). Left ventricular outflow tract obstruction was relieved in 2 patients. The aortic ar

Analysis of Pulsatile Vessel Expansion in Healthy, COPD- and PH-Patients Using Dynamic Vessel Segmentation in Free-Breathing Lung MRI.

BACKGROUND: The pulsatile expansion of pulmonary vessels carries dynamic cardiopulmonary information that may reveal disease earlier than structural changes alone. PURPOSE: To test (i) intra- and inter-scan repeatability of dynamic vessel metrics in healthy subjects, and (ii) whether chronic obstructive pulmonary disease (COPD) and postcapillary pulmonary hypertension (PH) exhibit disease-specific differences. STUDY TYPE: Retrospective, multi-cohort feasibility study with repeatability sub-study. SUBJECTS: Healthy: 29 (11 female), COPD: 52 (15 female), PH: 25 (15 female) with isolated postc

Comparison of three Doppler ratio indices for predicting adverse perinatal outcomes in late-onset fetal growth restriction.

OBJECTIVE: To compare the predictive performance of three Doppler ratio indices-cerebroplacental ratio (CPR), cerebral-placental-uterine ratio (CPUR), and uteroplacental-cerebral ratio (UCPR)-for adverse perinatal outcomes in late-onset fetal growth restriction (FGR). METHODS: This retrospective cohort study analyzed singleton pregnancies diagnosed with late-onset FGR (≥32 weeks) between October 2024 and January 2026. Doppler parameters-umbilical artery pulsatility index (UA-PI), middle cerebral artery pulsatility index (MCA-PI), and mean uterine artery pulsatility index (mean UtA-PI)-were re

Assessment of risk for small for gestational age at midgestation to define subsequent care.

BACKGROUND: Previous studies have demonstrated that a competing risks model for the prediction of small-for-gestational-age neonates has a superior performance than traditional risk scoring methods. The Fetal Medicine Foundation fetal and neonatal population weight charts are derived from sonographic estimated fetal weight rather than birthweight because a large proportion of neonates born before term result from pathologic pregnancy. The individualized risk assessment for small for gestational age at midgestation could be the basis of an antenatal plan that aims to improve the management of p

Monitoring and timing of delivery in suspected late fetal growth restriction at term.

BACKGROUND: Infants affected by late fetal growth restriction are at risk of adverse perinatal outcomes, either due to placental insufficiency or the consequences of late iatrogenic prematurity. There is no randomized trial reporting the optimal timing of delivery, and recommending delivery at 37 weeks can cause harm. Antenatal ultrasound parameters can be used to risk-stratify these pregnancies into low-risk cases, which are associated with a lower likelihood of adverse outcomes and are thus suitable for full-term delivery, and high-risk cases, which may benefit from early-term delivery. Validation of the first-trimester machine learning model for predicting pre-eclampsia in an Asian population.

OBJECTIVES: To evaluate the performance of an artificial intelligence (AI) and machine learning (ML) model for first-trimester screening for pre-eclampsia in a large Asian population. METHODS: This was a secondary analysis of a multicenter prospective cohort study in 10 935 participants with singleton pregnancies attending for routine pregnancy care at 11-13+6 weeks of gestation in seven regions in Asia between December 2016 and June 2018. We applied the AI+ML model for the first-trimester prediction of preterm pre-eclampsia (<37 weeks), term pre-eclampsia (≥37 weeks), and any pre-eclampsia,

Preterm preeclampsia screening using biomarkers: combining phenotypic classifiers into robust prediction models.

BACKGROUND: Preeclampsia screening is a critical component of antenatal care worldwide. Currently, the most developed screening test for preeclampsia at 11 to 13 weeks' gestation integrates maternal demographic characteristics and medical history with 3 biomarkers-serum placental growth factor, mean arterial pressure, and uterine artery pulsatility index-to identify approximately 75% of women who develop preterm preeclampsia with delivery before 37 weeks of gestation. It is generally accepted that further improvements to preeclampsia screening require the use of additional biomarkers. We recen

Prediction of low birth weight: Comparison of placental T2* estimated by MRI and uterine artery pulsatility index.

OBJECTIVE: Neonates at low birth weight due to placental dysfunction are at high risk of adverse outcomes. These outcomes can be substantially improved by prenatal identification. The Magnetic Resonance Imaging (MRI) constant, placental T2* reflects placental structure and oxygenation and thereby placental function. Therefore, we aimed to evaluate the performance of placental T2* in the prediction of low birth weight using the uterine artery (UtA) pulsatility index (PI) as gold standard. METHODS: This was a prospective observational study of 100 singleton pregnancies included at 20-40 weeks'

Arteries masquerading as varicose veins: A trap for phlebologists.

Ultrasound guided sclerotherapy may be complicated by intra-arterial injections resulting in significant tissue necrosis. Here, we present a 69-year-old man with a history of right small saphenous vein "stripping", presenting for the treatment of symptomatic lower limb varicose veins. Duplex ultrasound of the right lower limb outlined the pathway of venous incompetence. Despite the history of "stripping", the small saphenous vein was present but the sapheno-popliteal junction was ligated at the level of the knee crease. No other unusual findings were reported at the time. During ultrasound gui

A randomised controlled trial evaluating the effect of potassium supplementation on vascular function and the renin-angiotensin-aldosterone system.

There is limited evidence on the effect of potassium supplementation on the vasculature in patients at increased cardiovascular risk. Potassium increases aldosterone and there is a strong association of hyperaldosteronism with poor cardiac outcomes. We aimed to determine whether potassium supplementation has a significant medium-term effect on aldosterone levels and, if so, what the overall effect of this is on vascular function in patients at moderate cardiovascular disease risk. Forty patients at moderate cardiovascular disease risk were included in a randomised placebo-controlled crossover

Fully autonomous preload-sensitive control of implantable rotary blood pumps.

A pulsatility-based control algorithm with a self-adapting pulsatility reference value is proposed for an implantable rotary blood pump and is to be tested in computer simulations. The only input signal is the pressure difference across the pump, which is deduced from measurements of the pump's magnetic bearing. A pulsatility index (PI) is calculated as the mean absolute deviation from the mean pressure difference. As a second characteristic, the gradient of the PI with respect to the pump speed is derived. This pulsatility gradient (GPI) is used as the controlled variable to adjust the operat

Investigation of the arterial stiffness on renal artery Doppler sonograms.

This study aims to identify the effect of stiffness on renal artery Doppler sonograms by using a curve fitting approach. The study includes 20 patients with suspected renovascular hypertension. Each patient underwent recordings during renal artery examinations before and after administration of Captopril 25 mg (Kapril tablet, MN Pharmaceuticals, Istanbul, Turkey). Doppler signals were analyzed with short time Fourier transformation, followed by curve fitting to sonograms using Savitzky-Golay method. Doppler parameters obtained from pre- and post-Captopril curve-fitted sonograms were statistica

Etanercept improves inflammation-associated arterial stiffness in rheumatoid arthritis.

OBJECTIVES: Increased arterial stiffness, an independent risk factor for premature coronary artery disease, has been reported in patients with RA. The objectives of this study were first to assess, in patients with RA, the relationship between disease activity, inflammation and augmentation index, which is a combined measure of arterial stiffness and pulse wave reflection. The second objective was to establish any effect anti-rheumatic treatment may have on augmentation index. METHODS: One hundred and forty-eight RA patients with no previous history of cardiovascular disease (CVD) had their a

Vasoreactive response to maternal hyperoxygenation in the fetus with hypoplastic left heart syndrome.

BACKGROUND: Cardiopulmonary interactions play an important role in the pathophysiology of hypoplastic left heart syndrome (HLHS). Pulmonary vasculopathy has been identified, especially in those with restrictive/intact atrial septum. Responsiveness of the pulmonary vasculature to maternal hyperoxygenation (MH) may provide a tool to assess the degree of pulmonary vasculopathy present before birth. METHODS AND RESULTS: Doppler echocardiography was performed in 27 normal and 43 HLHS fetuses. In HLHS, sampling was repeated after 10 minutes of MH with 60% FiO(2) and after 5 minutes of recovery.
