Machine learning-based prediction of rotator cuff tears using anatomical parameters: a retrospective cohort study.

BACKGROUND: To apply machine learning to improve risk assessment for rotator cuff tears by developing an integrated predictive model that combines acromial morphology (structural factors) and patient characteristics (systemic factors). METHODS: A total of 342 patients who underwent shoulder radiography and MRI between 2023 and 2025 were included (180 tears, 162 intact). Demographic and radiographic parameters—including the novel acromial angle (NAA), critical shoulder angle, acromion index (AI), and acromial tilt—were collected. Six machine learning classifiers (logistic regression, decision t

Defining the minimal clinically important difference and patient acceptable symptom state following reverse shoulder arthroplasty for glenohumeral arthritis or cuff tear arthropathy at minimum 5-year follow-up.

BACKGROUND: While prior studies have assessed the minimal clinically important difference (MCID) and patient acceptable symptomatic state (PASS) following reverse shoulder arthroplasty (rTSA) at 2-year follow-up, data on clinically significant outcomes at mid-term follow-up remain limited. PURPOSE: (1) To establish the MCID and PASS thresholds for rTSA with a minimum follow-up period of 5 years. (2) To examine factors that are predictive of MCID and PASS achievement for rTSA, including preoperative, demographic, and intraoperative variables. METHODS: All patients who underwent rTSA for rotat

High-Dose Statins Preserve Tendon-Bone Interface Healing Without Adversely Affecting Fatty Infiltration in a Rotator Cuff Repair Rat Model.

PURPOSE: To evaluate the effects of high-dose atorvastatin (hATZ) on histologic and biomechanical tendon-bone interface (TBI) healing and fatty infiltration following rotator cuff (RC) repair in a rat model. METHODS: Twenty Sprague-Dawley rats underwent RC repair surgery on both shoulders, after which hATZ was administered orally for 4 weeks postoperatively. The effects of hATZ on TBI healing were assessed macroscopically, histologically, and biomechanically. Fatty infiltration was evaluated using Oil-Red-O staining and immunohistochemical analysis of gene marker expressions. Expression level

Reduced acromiohumeral distance and increased critical shoulder angle: implications for primary care clinicians.

Objectives: The literature indicates that reduced acromiohumeral distance (AHD) and increased critical shoulder angle (CSA) are associated with large and massive rotator cuff (RC) tears which may not be amenable to a successful repair. The purpose of this study was to examine the overall accuracy of these two radiographic features in diagnosing significant RC pathology. Methods: This was a diagnostic study of patients with shoulder pain. To examine the overall accuracy of the measurements, the area under the Receiver Operating Characteristic curves (AUC) were calculated. The validity indices (

Does Additional Biceps Augmentation Improve Rotator Cuff Healing and Clinical Outcomes in Anterior L-Shaped Rotator Cuff Tears? Clinical Comparisons With Arthroscopic Partial Repair.

BACKGROUND: The repair of anterior L-shaped tears is usually difficult because of the lack of anterior rotator cuff tendon to cover the footprint. The biceps tendon is usually exposed from the retracted anterolateral corner of the torn tendon and can be easily used to augment rotator cuff repair. Hypothesis/Purpose: This study compared the clinical outcomes of the biceps augmentation technique with those of partial tendon repair for the arthroscopic treatment of large anterior L-shaped rotator cuff tears to evaluate the role of additional biceps augmentation in tendon healing. We hypothesized

Immunolocalization of cytokines and growth factors in subacromial bursa of rotator cuff tear patients.

Inflammation in the subacromial bursa causes pain in patients suffering from rotator cuff tear, with this long-lasting inflammation leading to fibrosis and thickening of the subacromial bursa. Both inflammatory cytokines and mechanical stress, and impingement in the subacromial space, might induce and worsen this inflammation. However, little is known of the mechanism of this inflammation. In this study, we used immunohistological staining to demonstrate the expression of Interleukin-1 beta (IL-1 beta), Tumor necrosis factor alpha (TNF-alpha), transforming growth factor beta (TGF-beta), and ba

The results of arthroscopic debridement of glenoid labral tears based on tear location.

A retrospective, average 2-year review of 14 patients with a primary glenoid labral tear treated by arthroscopic debridement was conducted to evaluate the long-term results of labral debridement based on tear location in preoperatively stable shoulders. All patients with concomitant pathology requiring an additional open or arthroscopic procedure were eliminated from this study. Every patient complained of shoulder pain mainly with overhead activities and nine patients (64%) also described "clicking" with shoulder movement. All shoulders were stable to translation in all quadrants during the p

Atypical manifestation of late onset limb girdle muscular dystrophy presenting with recurrent falling and shoulder dysfunction: a case report.

INTRODUCTION: Myopathies can be sub-classified into congenital, hereditary, mitochondrial, and secondary myopathies.Congenital myopathies are usually diagnosed post partal or in early childhood. Manifestation in adolescence is uncommon and most cases occur as sporadic mutations. Therefore, there is a risk of under diagnosing this disease in middle-aged patients showing pain, dysfunction, recurrent trauma or falls, where muscle atrophy is seen as a secondary injury. CASE PRESENTATION: Our report is about a 54 year old Caucasian woman with an extended history of pain, loss of function and weakn

Long-term functional and structural outcome in patients with intact repairs 1 year after open transosseous rotator cuff repair.

HYPOTHESIS: Tendon-to-bone suture and a fatty degeneration index (FDI) <or= 2 are required to achieve an intact cuff in the mid-term. Our primary objective was to determine the structural and functional long-term outcomes of repaired cuffs that were intact after one year. MATERIALS AND METHODS: We studied 30 cuffs with FDI <or= 2 at surgery and no tear one year after tension-free tendon-to-bone suture. Mean follow-up was 9 +/- 0.8 years. Gross tendon-stump lesions were excised prior to repair. RESULTS: Re-tear after one year was rare: only one cuff showed a recurrent tear. In the 29 intact c
