Benzylpenicillin Concentrations in Intrapartum Group B Streptococcus Prevention Guidelines; A Systematic Review of the Evidence.

INTRODUCTION: Intrapartum antibiotics are often used to reduce neonatal risk of early-onset Group B Streptococcus (GBS) disease. Protocols aim to achieve a fetal serum and amniotic fluid concentration above the minimum inhibitory concentration (MIC) of GBS at the time of birth. AIMS: We aimed to summarise the evidence for currently recommended doses of benzylpenicillin by comparing intrapartum fetal serum and amniotic fluid concentrations of benzylpenicillin with the associated MIC of GBS at the time of birth. METHODS: We systematically reviewed PubMed, the Cumulative Index to Nursing and Al

A large prospective study of group B Streptococcus carriage in pregnant women in Morocco: prevalence, serotype distribution, antimicrobial resistance and virulence determinants.

BACKGROUND: Group B streptococcus (GBS) is a commensal bacterium that can cause severe infection in neonates. Vaginal colonization with GBS is a risk factor for mother-to-child transmission and, therefore, its monitoring is essential for prevention strategies. Epidemiological data on maternal GBS carriage and the characterization of recovered isolates in North African regions are scarce, mainly due to the lack of systematic GBS screening in pregnant women. Maternal vaccination based on the use of various capsular polysaccharides (CPS) remains a promising strategy for the control of GBS infecti

Early P2X7 receptor activation mitigates neonatal group B Streptococcus infection severity and long-term neurodevelopmental impairment.

Neonatal invasive Group B Streptococcus (GBS) infection remains a leading cause of mortality and long-term neurodevelopmental impairment. Effective treatment for neonatal GBS sepsis is critically hampered by a dysregulated immune response that fails to clear the infection and induces neuroinflammation. The P2X7 receptor (P2X7R) plays a key role in inflammatory responses and neuroinflammation during infection. Here, we investigated the effects of systemic P2X7R modulation on immune and neuronal dysfunction in a neonatal GBS infection model. Neonatal mice born to GBS-colonized dams were treated

A Retrospective Review of Neonatal Sepsis among GBS-Colonized Women Undergoing Planned Cesarean Section after Labor Onset or Rupture of Membranes.

BACKGROUND: Sepsis is a leading cause of mortality and morbidity in neonates, with group B streptococcus (GBS) remaining the most frequent pathogen isolated from term infants. Surveillance data showed that the majority of cases of early-onset GBS disease were neonates born to women who either received no or suboptimal intrapartum antibiotic prophylaxis with a notable portion of those women having a missed opportunity to receive ≥4 hours of chemoprophylaxis. Women planning delivery by cesarean section who present in labor or rupture of membranes prior to their scheduled surgery are unlikely to

A case of infected schwannoma mimicking malignant tumor.

BACKGROUND: Infected schwannoma has been reported, this being one of the four cases published in the literature. Infected schwannoma has proven to be a tough diagnostic challenge to the treating tumor surgeon, mimicking infectious entities and most essentially, a malignant tumor. CASE PRESENTATION: The authors report the case of a 64-year-old male with a soft tissue mass in his right gluteal area that presented initially with right leg pain, then later with signs of inflammation on the tumor area. Magnetic resonance imaging (MRI), computed tomography (CT), and thallium-201 scintigraphy studie

A study on early-onset neonatal group B streptococcal infection, Bulgaria, 2007-2011.

This study examines neonatal group B streptococcal (GBS) colonization and its relation to early-onset GBS disease (EOGBSD), based upon the experience of leading obstetrics and gynecology centers in Bulgaria. The objectives of the study were to update neonatal colonization rates and to assess relationships between clinically differentiated cases (culture-proven GBS newborns) and risk factors inherent to the infant and mother, using a computerized file. The neonatal GBS colonization rate ranged from 5.48 to 12.19 per 1000 live births. Maternal-fetal infection (MFI, a provisional clinical diagnos

Predicting outcomes of neonates born to GBS-positive women who received inadequate intrapartum antimicrobial prophylaxis.

We determined the predicting factors of early-onset group B streptococcal (EOGBS) infection in neonates who were born to GBS carrier mothers with inadequate intrapartum antibiotic prophylaxis (IAP). Medical records of all neonates born from January 1, 2008 to April 1, 2010 were reviewed. Inadequate IAP was defined as delivery less than 4 hours (h) after the first administration of antimicrobial. Of 1910 neonates, 273 were born from mothers colonized with GBS, including 69 who received inadequate IAP. Of 69 neonates, nine showed symptoms, including respiratory distress, fever, tachycardia, vomi

Voided midstream urine culture and acute cystitis in premenopausal women.

BACKGROUND: The cause of acute uncomplicated cystitis is determined on the basis of cultures of voided midstream urine, but few data guide the interpretation of such results, especially when gram-positive bacteria grow. METHODS: Women from 18 to 49 years of age with symptoms of cystitis provided specimens of midstream urine, after which we collected urine by means of a urethral catheter for culture (catheter urine). We compared microbial species and colony counts in the paired specimens. The primary outcome was a comparison of positive predictive values and negative predictive values of organ

Aortic valve replacement and concomitant coronary artery bypass grafting in a patient with infective endocarditis and anomalous origin of the right coronary artery from the opposite sinus of valsalva.

A 69-yrs-old woman with anomalous origin of the right coronary artery from the opposite sinus of Valsalva (ACAOS) was diagnosed as having infective endocarditis affecting the aortic valve. Transthoracic echocardiography showed severe aortic stenosis and vegetations on the cusp of the aortic valve, which necessitated aortic valve replacement. Before the operation, computed tomography showed a right-ACAOS, with the artery running an interarterial course between the aorta and pulmonary artery. ACAOS running an interarterial course has been reported to be associated with an increased risk of ische

Serotypes and antibiotic resistance in Group B streptococcus isolated from patients at the Maternity Hospital, Kuwait.

A total of 143 group B streptococcus (GBS) isolates collected from mothers at the Maternity Hospital in Kuwait were investigated for their serotypes and antibiotic resistance, and screened by PCR for the carriage of genes for resistance to tetracycline (tetk, tetM, tetL, tetO), erythromycin (ermA, ermB, ermC, ermTR, ermM, mefA, mefE, msrA) and aminoglycosides (aph3, ant4, ant6). All isolates were serotyped using a latex agglutination test. Most of the isolates belonged to serotypes V (38.5 %), III (20.9 %), Ia (7.7 %) and II (11.2 %). Sixteen isolates (11.2 %) were nontypable. All isolates wer

Selection, recombination, and virulence gene diversity among group B streptococcal genotypes.

Transmission of group B Streptococcus (GBS) from mothers to neonates during childbirth is a leading cause of neonatal sepsis and meningitis. Although subtyping tools have identified specific GBS phylogenetic lineages that are important in neonatal disease, little is known about the genetic diversity of these lineages or the roles that recombination and selection play in the generation of emergent genotypes. Here, we examined genetic variation, selection, and recombination in seven multilocus sequence typing (MLST) loci from 94 invasive, colonizing, and bovine strains representing 38 GBS sequen

Intranasal immunization with GBS surface protein Sip and ScpB induces specific mucosal and systemic immune responses in mice.

Sip and ScpB are highly conserved among strains of Group B Streptococcus (GBS). Thus, the two proteins are attractive antigens for inclusion in a vaccine against GBS. In this study, we constructed and expressed the two proteins, and investigated their specific mucosal immune responses against GBS induced by intranasal immunization with cholera toxin (CT) and CpG-oligodeoxynucleotides (CpG-ODNs). Intranasal immunization with different doses of recombinant Sip and ScpB all elicited specific antibodies in serum and vagina of mice. A combination of rSip and rScpB with either CT or CpG-ODN elicited

Adoption of hospital policies for prevention of perinatal group B streptococcal disease--United States, 1997.

Group B streptococcal (GBS) infections are the leading bacterial cause of disease and deaths among newborns in the United States. In 1993, the annual cost of caring for newborns with sepsis caused by group B Streptococcus was an estimated $294 million. A survey of hospital GBS disease prevention practices in 1994 indicated that those hospitals with a prenatal screening policy had fewer neonatal GBS disease cases. In 1996, to promote a coordinated approach to prevention, CDC issued consensus guidelines about GBS disease prevention that were endorsed by the American College of Obstetricians and

Expectant management of rupture of membranes at term.

We conducted a prospective randomized study involving 317 patients with term gestations (greater than 36 weeks) and premature rupture of membranes (PROM). Eighty-five percent of the 167 patients managed conservatively began labor within 48 hours. The cesarean section rate in this group was 7% as opposed to 21% in the group managed by oxytocin induction. There were no neonatal infections, and the maternal intrauterine infection rate was lower in the group managed expectantly, 4% vs 12%. There was no difference in the average time of hospitalization for the two groups. Conservative management of
