Quantification of humeral head deformity following neonatal brachial plexus palsy.

BACKGROUND: Neonatal brachial plexus palsy frequently leads to glenohumeral dysplasia if neurological recovery is incomplete. Although glenoid retroversion and glenohumeral subluxation have been well characterized, humeral head deformity has not previously been quantified. Nonetheless, humeral head flattening is described as a contraindication to joint contracture release and external rotation tendon transfers. This study describes a novel technique for objectively quantifying humeral head deformity with use of magnetic resonance (MR) imaging and correlates the humeral head deformity with clin…

Nerve allograft transplantation for functional restoration of the upper extremity: case series.

BACKGROUND: Major trauma to the spinal cord or upper extremity often results in severe sensory and motor disturbances from injuries to the brachial plexus and its insertion into the spinal cord. Functional restoration with nerve grafting neurotization and tendon transfers is the mainstay of treatment. Results may be incomplete due to a limited supply of autologous material for nerve grafts. The factors deemed most integral for success are early surgical intervention, reconstruction of all levels of injury, and maximization of the number of axonal conduits per nerve repair. OBJECTIVE: To repor…

The angiosomes of the forearm: anatomic study and clinical implications.

The angiosome concept was introduced in 1987 by Taylor and Palmer. Their anatomic study correlated the blood supply to the skin from the named segmental or distributing "source" arteries with their supply to the underlying muscles, tendons, nerves, and bones. Although this investigation encompassed the body, there were areas where the supply to individual tissues was not examined in detail. The present study, therefore, examines one of these regions where certain voids in our knowledge still exist--the forearm. Ten upper limbs from fresh cadavers were studied over an 18-month period after perf

Consensus in chronic ankle instability: aetiology, assessment, surgical indications and place for arthroscopy.

Ankle sprains are the most common injuries sustained during sports activities. Most ankle sprains recover fully with non-operative treatment but 20-30% develop chronic ankle instability. Predicting which patients who sustain an ankle sprain will develop instability is difficult. This paper summarises a consensus on identifying which patients may require surgery, the optimal surgical intervention along with treatment of concomitant pathology given the evidence available today. It also discusses the role of arthroscopic treatment and the anatomical basis for individual procedures.

A novel type of familial hypercholesterolemia: double heterozygous mutations in LDL receptor and LDL receptor adaptor protein 1 gene.

BACKGROUND: Autosomal recessive hypercholesterolemia (ARH) is an extremely rare inherited hypercholesterolemia, the cause of which is mutations in low-density lipoprotein (LDL) receptor adaptor protein 1 (LDLRAP1) gene. METHODS: A total of 146 heterozygous familial hypercholesterolemic (FH) patients with a mutation in LDLR gene were screened for genes encoding proprotein convertase subtilisin/kexin type 9 (PCSK9) and LDLRAP1. RESULTS: Among the 146 subjects, we identified a 79-year-old Japanese female with double mutations in LDLR gene (c.2431A>T) and LDLRAP1 gene (c.606dup). Two other relat

Reverse Shoulder Arthroplasty with a Glenoid-Sided Lateralized Prosthesis and Concomitant Latissimus Dorsi Transfer Is Associated with Early Proximal Humeral Osseous Erosion Without Superior Functional Outcomes.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) using a lateralized implant design is commonly employed to address deficits in external rotation. In select cases, rTSA is combined with a concomitant latissimus dorsi tendon transfer (rTSA+LT) to augment external rotation. This study compared the clinical and radiographic outcomes of patients undergoing rTSA+LT with those undergoing rTSA alone (rTSA-only). We hypothesized that patients who underwent rTSA+LT would demonstrate (1) improved clinical outcomes especially, active external rotation (ER); and (2) a higher incidence of abnormal ra…

Elbow MRI Findings Do Not Correlate With Future Placement on the Disabled List in Asymptomatic Professional Baseball Pitchers.

BACKGROUND: Injury rates among professional baseball players may reach as high as 5.8 per 1000 encounters, with pitchers being most vulnerable on account of the excessive biomechanical load on the upper extremity during the throwing motion. Anatomically, the shoulder is the most common site of pitching-related injury, accounting for 30.7% of injuries, closely followed by the elbow at 26.3%. Characteristic valgus loading imparts a predictable constellation of stresses on the joint, including medial tension, lateral compression, and posterior medial shearing. The degenerative cohort of tissue ch

A Rare Case of Pontocerebellar Hypoplasia Type 1B With Literature Review.

A full-term female newborn was transferred to our neonatal intensive care unit (NICU) on day two of life for hypotonia. Physical examination was significant for overriding sutures, displaced small anterior fontanelle, axial hypotonia, extremity hypertonia, and slow deep tendon reflexes. She was also noted to have stridor with crying but had unlabored breathing without oxygen requirements and a normal heart examination. A brain magnetic resonance imaging (MRI) showed a large cisterna magna and cerebellar hypoplasia with the majority of the cerebellar vermis present, suggesting a possible Dandy-

Snapping of the medial head of the triceps and recurrent dislocation of the ulnar nerve. Anatomical and dynamic factors.

We describe seventeen patients (twenty-two limbs) who had snapping (dislocation) of both the ulnar nerve and the medial head of the triceps over the medial epicondyle. Two patients (two limbs) were seen because of painless snapping, four patients (five limbs) had snapping and pain in the medial aspect of the elbow, three patients (three limbs) had symptoms related to the ulnar nerve only, and six patients (seven limbs) had snapping and symptoms related to the ulnar nerve. In addition, snapping was identified incidentally on routine screening in five asymptomatic limbs in four patients, one of

[A 65-year-old woman with dysarthria, dysphagia, weakness, and gait disturbance].

We report a 65-year-old woman with progressive dysarthria, dysphagia, weakness, and gait disturbance. The patient was well until 59 years of age (January of 1986) when she noted bilateral ptosis. One year later, she noted a gradual onset of difficulty in speech (articulation). Her speech slowly deteriorated and she noted weakness in chewing power and difficulty in swallowing in addition. In October 1987, she developed emotional incontinence. In January of 1988, she started to drag her left foot. She was admitted to our hospital on June 13 of 1988. On admission, she was alert and general physic

[Physiology and pathophysiology of the metabolism of lipoproteins].

UNLABELLED: PHYSIOLOGY: Lipoproteins (LP) are generally classified according to their density. Triglycerides are mainly transported in chylomicrons and very low density LP (VLDL), cholesterol is mainly transported in low density LP (LDL) and high density LP (HDL). The metabolism of LP is controlled by their apolipoproteins, by specific receptors, enzymes, and transfer proteins. Triglycerides and cholesterol from the diet are transported in chylomicrons. The triglycerides are rapidly hydrolyzed by LP-lipase to yield chylomicron remnants. The released free fatty acids are used either for storage

Case Report: When catatonia-like symptoms are not catatonia: Guillain-Barré syndrome in a patient with schizophrenia.

INTRODUCTION: Schizophrenia is a severe psychiatric disorder. Catatonia is relatively common in schizophrenia; its main manifestations include catatonic stupor, mutism, negativism, and other psychomotor symptom clusters. Patients with schizophrenia are at increased risk of infections, which are also recognized triggers for Guillain-Barré syndrome (GBS). GBS typically presents with limb weakness, paresthesia, facial weakness, respiratory muscle paralysis, and autonomic symptoms, and may similarly result in severe immobility and impaired communication. Consequently, when schizophrenia (especiall
