Fetal gene therapy and pharmacotherapy to treat congenital hearing loss and vestibular dysfunction.

Disabling hearing loss is expected to affect over 900 million people worldwide by 2050. The World Health Organization estimates that the annual economic impact of hearing loss globally is US$ 750 billion. The inability to hear may complicate effective interpersonal communication and negatively impact personal and professional relationships. Recent advances in the genetic diagnosis of inner ear disease have keenly focused attention on strategies to restore hearing and balance in individuals with defined gene mutations. Mouse models of human hearing loss serve as the primary approach to test gen

Residual dizziness after the first BPPV episode: role of otolithic function and of a delayed diagnosis.

Residual dizziness (RD) following the resolution of a benign paroxysmal positional vertigo (BBPV) episode is frequently reported by patients. Possible causes are still under debate in the literature. This study discusses the possible role of otolithic function and of elapsed time from onset of symptoms to diagnosis in the genesis of RD. In total, 116 patients younger than 65 years with their first episode of BPPV and without any other comorbidities were enrolled in the study. Before a bedside examination, subjective visual vertical (SVV) was determined in the case of a history suggestive of BP

Otolith organ function according to subtype of benign paroxysmal positional vertigo.

OBJECTIVES/HYPOTHESIS: The clinical features and treatment outcomes of benign paroxysmal positional vertigo (BPPV) are known to be different depending on the type of and involved canal. This difference could be due to differences in the functional change of the otolith organ. STUDY DESIGN: Case series. METHODS: Forty-nine patients were diagnosed to primary BPPV; 18 were categorized as posterior canal canalolithiasis (PC canalolithiasis), and 31 were categorized as horizontal canal (HC) BPPV with canalolithiasis or cupulolithiasis (HC canalolithiasis or HC cupulolithiasis). The ventrolateral upper cervical cell group in cat projects to all rostrocaudal levels of the periaqueductal gray matter.

The lateral ventral horn of the upper cervical (C(1-3vl)) cord in rat, cat and monkey contains many cells that project to the periaqueductal gray matter (PAG). Until now it was assumed that these cells only project to the ventrolateral part of the caudal PAG. Because the ventrolateral caudal PAG is involved in quiescence and hypotension, it was hypothesized that the C(1-3vl)-PAG projecting cells play a role in immobility behavior, possibly activated by neck muscle afferents. However, in the present anterograde and retrograde study in cat we showed that C(1-3vl) cells do not only target the cau

[Electrophysiological brain-stem diagnosis in patients with a pronounced obstructive sleep apnea syndrome].

BACKGROUND AND OBJECTIVE: The pathophysiological basis of obstructive sleep apnoea syndrome (OSAS) is a loss of tone of the oropharyngeal muscles during sleep, leading to partial and/or total collapse of the oropharyngeal muscle tube. The normal inspiration-synchronous activation (ISA) of the tongue muscles is diminished or lost in patients with OSAS. Observations of OSAS in patients with ischaemic pontomedullary lesions, syringobulbia (syringomyelia), olivo-ponto-cerebellar atrophy or disseminated encephalopathy have indicated that it is due to impairment of ISA. In this study systematic elec

Effect of co-morbid anxiety and depression history on multidomain concussion clinical outcomes and recovery time.

IMPORTANCE: Extant evidence supports a history of psychological health diagnoses, such as anxiety and depression, as risk factors associated with greater symptom burden and prolonged recovery following concussion. However, researchers have yet to examine the combined effect of co-morbid anxiety and depression history on multidomain clinical outcomes and recovery time following concussion. OBJECTIVE: To examine the effect of co-morbid anxiety and depression history on multidomain concussion clinical outcomes including concussion symptoms, cognitive and vestibular/oculomotor functioning, and rec

Saccadic velocity measurements in strabismus.

Traditional evaluation of strabismus has included cover test measurements, evaluation of the range of ocular rotations, and an array of subjective sensory tests. These studies could not always differentiate paresis of an extraocular muscle from restrictions and from various neuro-ophthalmic motility disorders. The measurement of horizontal and vertical saccadic movements can provide an objective test of rectus muscle function. Using EOG, saccades can be recorded easily, inexpensively, and repeatably at any age. In ocular muscle paresis or paralysis, saccadic speed is reduced mildly to markedly

Isolated oculomotor dysfunction with downbeat nystagmus as a rare presentation of anti-GAD autoimmunity.

BACKGROUND: Glutamic acid decarboxylase (GAD) antibodies are associated with a range of autoimmune neurological syndromes, including cerebellar ataxia (CA). Ocular motor disturbances such as nystagmus and diplopia are frequently observed in CA but rarely are seen as isolated manifestation. CASE PRESENTATION: We describe the case of a 60-year-old woman who presented with oculomotor dysfunction consisting of persistent diplopia and vertical nystagmus, in the absence of any other neurological manifestation. Brain MRI and PET-CT scans were unremarkable. Videonystagmography documented downbeat nys

Differing effects on the inner ear of three gentamicin compounds: GM-C1, -C2 and -C1a.

OBJECTIVE: Gentamicin (GM) has been used to ablate the vestibular system function as a form of treatment for Ménière's disease. Generally, clinicians have not recognized that commercial GM comprises three major compounds--GM-C1, -C2 and -C1a--and the effects of the individual compounds on the inner ear have not been elucidated. In this study we investigated differences in cochleo- and vestibulotoxicity between the three compounds MATERIAL AND METHODS: Hartley guinea pigs were treated for 2 weeks with daily subcutaneous injections (100 mg/kg) of the GM complex or the GM-C1, -C2 or -C1a compoun

Clinical and functional characteristics, possible causes, and impact of chronic cough in patients with cerebellar ataxia, neuropathy, and bilateral vestibular areflexia syndrome (CANVAS).

Cerebellar ataxia with neuropathy and bilateral vestibular areflexia syndrome (CANVAS) is an hereditary autosomal recessive disease. Recent studies propose including chronic cough (CC) as a symptom of CANVAS. For 10 patients with CANVAS as genetically confirmed by biallelic expansion of the AAGG repeat motif (AAGGGexp) in intron 2 of replication factor C subunit 1 (RFC1), our aim was, as a multidisciplinary team, to describe clinical and functional characteristics and possible causes of CC following European Respiratory Society (ERS) recommendations, and to evaluate CC impact on quality of lif

Treatment of non-sports related concussion in adolescents following an irritability algorithmic approach: a case series.

BACKGROUND: Concussion management evidence supports the efficacy of gradual active rehabilitation for return to sport in adolescents, however there is paucity of evidence for non-sports-related concussion management. PURPOSE: The purpose of this case series is to describe the feasibility and practicality of an irritability-based step-by-step model to guide rehabilitation. CASE DESCRIPTIONS: Three adolescent patients diagnosed with concussion during a non-sports related event are described. Each adolescent was classified as high, moderate, or low irritability level based on symptom exacerbati

Genetic heterogeneity in patients with enlarged vestibular aqueduct and Pendred syndrome.

BACKGROUND: Pathogenic variants in the SLC26A4 gene, encoding for Cl-/HCO3- and I- anion transporter pendrin, are associated with non-syndromic hearing loss with enlarged vestibular aqueduct (NSEVA) and Pendred syndrome (PDS). In the Caucasian population, up to 75% of patients fail to identify a genetic cause through biallelic mutations in the SLC26A4 gene. The CEVA haplotype could therefore play an important role in the diagnostics of NSEVA. The aim of the study was to determine the genetic etiology of hearing loss with EVA or with fully developed PDS in 37 probands and the functional charact

Oculomotor, vestibular, reaction time, and cognitive tests as objective measures of neural deficits in patients post COVID-19 infection.

OBJECTIVE: An alarming proportion (>30%) of patients affected by SARS-CoV-2 (COVID-19) continue to experience neurological symptoms, including headache, dizziness, smell and/or taste abnormalities, and impaired consciousness (brain fog), after recovery from the acute infection. These symptoms are self-reported and vary from patient to patient, making it difficult to accurately diagnose and initiate a proper treatment course. Objective measures to identify and quantify neural deficits underlying the symptom profiles are lacking. This study tested the hypothesis that oculomotor, vestibular, reac
