Handheld cochlear nerve action potential mapping for real-time functional navigation in vestibular schwannoma surgery.

OBJECTIVE: Hearing preservation in vestibular schwannoma (VS) surgery remains challenging due to frequent cochlear nerve displacement and the limited reliability of brainstem auditory evoked potentials (BAEPs) once waveforms deteriorate. This study evaluated the feasibility and clinical utility of bipolar handheld cochlear nerve action potential (CNAP) mapping for intraoperative nerve identification and compared its prognostic performance with BAEP monitoring. METHODS: In this retrospective single-center cohort, 193 patients with histologically confirmed VS underwent intraoperative auditory m

The effects of abnormality of cVEMP and oVEMP on rehabilitation outcomes in patients with idiopathic benign paroxysmal positional vertigo.

The presence of utricular and saccular dysfunction impairs quality of life (QoL) in patients. The aims of the present study were to examine the effect of repositioning maneuvers on QoL of patients with idiopathic benign paroxysmal positional vertigo (BPPV) and to describe the effect of cervical vestibular-evoked myogenic potential (cVEMP) or ocular VEMP (oVEMP) abnormalities on patient recovery after rehabilitation. Thirty idiopathic BPPV patients with/without otolith dysfunctions (n = 15, each group) were included in this clinical trial study. Otolith dysfunction was determined using oVEMP an

Auditory function in women with autoimmune inner ear diseases and their offspring.

OBJECTIVE: The precise cause of congenital sensorineural hearing loss (CSNHL) is unclear in many cases. In a previous study we found that offspring from guinea pigs with autoimmune sensorineural hearing loss (ASNHL) exhibited signs of SNHL. Here we studied women with autoimmune inner ear diseases (AIED) and their offspring. Our aim was to determine if autoimmune damage may be one of the causes of CSNHL. METHODS: Thirty-eight pregnant women with AIED were recruited. Thirty-three had ASNHL; one with autoimmune delayed endolymphatic hydrops (ADEH) and four with autoimmune Meniere's disease (AIMD

Middle fossa approach: indications, technique, and results.

This article discusses the indications, surgical technique, results, and complications of middle fossa craniotomy (MFC) for vestibular schwannoma surgery, focusing on issues such as serviceable hearing, tumor characteristics, and patient-specific factors that help determine options for therapy. MFC is suitable for intracanalicular vestibular schwannomas that extend less than 1 cm into the cerebellopontine angle in patients with good hearing. With the expanding use of modern imaging, many small tumors are being identified in patients with no or minimal symptoms. Patients with these tumors have

Bone availability in the sagittal dimension of the infrazygomatic crest for miniscrew insertion: A retrospective cone-beam computed tomography study.

BACKGROUND: There is limited information available regarding the bone volume in the sagittal dimension of the infrazygomatic crest (IZC) that is safe for the insertion of a temporary skeletal anchorage device. The objective of this study was to assess the sagittal bone availability when the anterior and posterior walls of the IZC reach an inclination of 45°. METHODS: A cross-sectional investigation was undertaken on 100 cone-beam computed tomography scans of 40 males and 60 females. A total of 20 landmarks were digitized for each IZC outline, 1 mm from the vestibular alveolar bone, and their

Protocol for a multicentre randomised controlled trial of STeroid Administration Routes For Idiopathic Sudden sensorineural Hearing loss: The STARFISH trial.

Idiopathic sudden sensorineural hearing loss (ISSNHL) is the rapid onset of reduced hearing due to loss of function of the inner ear or hearing nerve of unknown aetiology. Evidence supports improved hearing recovery with early steroid treatment, via oral, intravenous, intratympanic or a combination of routes. The STARFISH trial aims to identify the most clinically and cost-effective route of administration of steroids as first-line treatment for ISSNHL. STARFISH is a pragmatic, multicentre, assessor-blinded, three-arm intervention, superiority randomised controlled trial (1:1:1) with an intern

Electroencephalography microstate alterations reflect potential double-edged cognitive adaptation in Ménière's disease.

PURPOSE: To explore the microstate characteristics and underlying brain network activity of Ménière's disease (MD) patients based on high-de … tive symptom severity were assessed by neuropsychological questionnaires, pure tone audiometry, and vestibular function tests. Resting-state EEG recordings were obtained using a 256-channel EEG system, and the electric field topographies were clustered into four dominant microstate classes (A, B, C, and D). The dynamic parameters of each microstate were analyzed and utiliz …

Analysis of the thyroid phenotype in 42 patients with Pendred syndrome and nonsyndromic enlargement of the vestibular aqueduct.

BACKGROUND: Pendred syndrome (PS), a recessive disorder caused by mutations in the SLC26A4 (PDS) gene, is associated with deafness and goiter. SLC26A4 mutations have also been identified in patients exhibiting isolated sensorineural hearing loss without apparent thyroid abnormality (nonsyndromic enlargement of the vestibular aqueduct; nonsyndromic EVA). Our aim was to describe systematically the thyroidal phenotypes and the SLC26A4 genotypes of patients presenting with PS or nonsyndromic EVA. METHODS: Nineteen patients with PS and 23 patients with nonsyndromic EVA, aged 5-53 years, were inclu

Pendred syndrome in a large consanguineous Brazilian family caused by a homozygous mutation in the SLC26A4 gene.

Pendred Syndrome (PS) is an autossomal recessive disorder characterized by sensorineural deafness, goiter and iodide organification defect. The hearing loss is associated with inner ear abnormalities, ranging from an isolated enlarged vestibular aqueduct (EVA) to a typical coclear dysplasia. Mutations in the gene that encodes pendrin (SLC26A4), a chloride/iodide transporter, have been shown to be associated with PS. We describe the clinical and molecular characteristics of a large consanguineous family harboring a mutation in the SLC26A4 gene. The proband was a 26-year-old deaf Brazilian woman

Vestibulo-spinal response modification as determined with the H-reflex during the Spacelab-1 flight.

Our laboratory at the Johnson Space Center has employed the H-reflex recorded from the soleus muscle as a method of monosynaptic reflex testing in conjunction with vertical linear acceleration to assess modification of utriculo-saccular function induced through prolonged exposure to microgravity. It was hypothesized that exposure to free fall would reduce the necessity for postural reflexes in the major leg muscles, and that postural modification would reflect a change, not in the peripheral vestibular organs, but more centrally. This postural adjustment would reflect a sensory motor rearrange

Functional Imaging of Autonomic Regulation: Methods and Key Findings.

Central nervous system processing of autonomic function involves a network of regions throughout the brain which can be visualized and measured with neuroimaging techniques, notably functional magnetic resonance imaging (fMRI). The development of fMRI procedures has both confirmed and extended earlier findings from animal models, and human stroke and lesion studies. Assessments with fMRI can elucidate interactions between different central sites in regulating normal autonomic patterning, and demonstrate how disturbed systems can interact to produce aberrant regulation during autonomic challeng

Reliability and Validity of Computerized Force Platform Measures of Balance Function in Healthy Older Adults.

BACKGROUND AND PURPOSE: Postural control declines with aging and is an independent risk factor for falls in older adults. Objective examination of balance function is warranted to direct fall prevention strategies. Force platform (FP) systems provide quantitative measures of postural control and analysis of different aspects of balance. The purpose of this study was to examine the reliability and validity of FP measures in healthy older adults. METHODS: This study enrolled 46 healthy elderly adults, mean age 67.67 (5.1) years, who had no history of falls. They were assessed on 3 standardized

Consequences of extensile lateral approach to the calcaneal fractures on balance and isokinetic strength of muscle groups crossing the ankle joint.

BACKGROUND: Inherent characteristics of extensile lateral approach (ELA) for fixation of displaced intraarticular calcaneal fractures together with delayed or probably insufficient physical therapy programs may lead to weakness of the muscle groups crossing the ankle joint. Peroneal tendons might be involved more than others because of possible postoperative adhesions. The aim of this study was to evaluate the isokinetic strength of the muscle groups crossing the ankle joint and also to assess balance and functional ability in this group of patients. METHODS: Based on a pilot study, 23 patien

Semicircular canal dysfunction in profound sudden sensorineural hearing loss and its prognostic value: implications for the proper indication of hyperbaric oxygen therapy.

Vestibular involvement in sudden sensorineural hearing loss (SSNHL) is regarded as an indicator of poor prognosis. This study aimed to investigate the prognostic significance of semicircular canal dysfunction in profound SSNHL and evaluate the effectiveness of hyperbaric oxygen therapy (HBOT) accordingly. From October 1, 2020 to August 15, 2025, patients diagnosed with profound SSNHL (≥ 80 dB) and with available video head impulse test results were assessed. A total of 85 patients were included and divided into two groups based on HBOT compliance: the HBOT group (≥ 5 sessions, n = 39) and the

Age-appropriate assessment of concussion in children and adolescents.

Sport-related concussion (SRC) in children and adolescents presents unique diagnostic challenges due to heterogeneous clinical presentations and rapid neurodevelopmental trajectories. This focused review emphasizes that diagnosis fundamentally relies on symptomatic reporting and objective physiological assessments spanning multiple domains, integrating age-specific tools such as the revised Sport Concussion Assessment Tool 6 (SCAT6) and the Sport Concussion Office Assessment Tool 6 (SCOAT6). While the SCAT6 (ages >13) and Child SCAT6 (ages 8-12) are optimized for acute evaluation within the in

RFC1 repeat expansions in downbeat nystagmus syndromes: frequency and phenotypic profile.

OBJECTIVES: The cause of downbeat nystagmus (DBN) remains unknown in a substantial number of patients ("idiopathic"), although intronic GAA expansions in FGF14 have recently been shown to account for almost 50% of yet idiopathic cases. Here, we hypothesized that biallelic RFC1 expansions may also represent a recurrent cause of DBN syndrome. METHODS: We genotyped the RFC1 repeat and performed in-depth phenotyping in 203 patients with DBN, including 65 patients with idiopathic DBN, 102 patients carrying an FGF14 GAA expansion, and 36 patients with presumed secondary DBN. RESULTS: Biallelic RFC

CHD7 variants associated with hearing loss and enlargement of the vestibular aqueduct.

Enlargement of the endolymphatic sac, duct, and vestibular aqueduct (EVA) is the most common inner ear malformation identified in patients with sensorineural hearing loss. EVA is associated with pathogenic variants in SLC26A4. However, in European-Caucasian populations, about 50% of patients with EVA carry no pathogenic alleles of SLC26A4. We tested for the presence of variants in CHD7, a gene known to be associated with CHARGE syndrome, Kallmann syndrome, and hypogonadotropic hypogonadism, in a cohort of 34 families with EVA subjects without pathogenic alleles of SLC26A4. In two families, NM_
