A modified triple incision technique for women with locally advanced vulvar cancer: a description of the technique and outcomes.

OBJECTIVE: Women with locally advanced vulvar carcinoma have an excellent chance of a cure by undergoing a radical vulvectomy with an "en bloc" inguinofemoral lymphadenectomy, but the morbidity associated this surgical approach is substantial. To achieve an outcome comparable with the traditional radical method in terms of oncologic safety, and an improved post-operative quality of life, we modified the classic triple-incision technique and suggested it as an alternative for these patients. The aim of this study was to report this new technique. STUDY DESIGN: Between January 2004 and November

Reduced E-cadherin expression correlates with disease progression in Paget's disease of the vulva but not Paget's disease of the breast.

The growth and metastasis of many cancers is due in part to loss of cell-cell adhesion. E-cadherin, plakoglobin and beta-catenin are important in cell adhesion. Our aim was to examine the presence of these molecules in Paget's disease of the vulva and Paget's disease of the breast, and to correlate any differences in their expression with the presence of invasive disease or an underlying carcinoma. Sixty-three archival cases of Paget's disease of the vulva, including eight associated with invasive disease, and 23 archival cases of Paget's disease of breast, which included 10 cases with ductal

The perineal artery axial flap.

We have found the perineal artery axial flap to be useful in reconstruction of moderate defects of the perineum. The experimental surgical technique and clinical applications of the perineal artery axial flap are updated. Our experience with 10 patients over the past 5 years is reviewed. The advantages of this flap are that it is thin and the donor site morbidity is minimal.

Clinical utility of the modified segmental boost technique for treatment of the pelvis and inguinal nodes.

PURPOSE: Low-lying pelvic malignancies often require simultaneous radiation to pelvis and inguinal nodes. We previously reported improved homogeneity with the modified segmental boost technique (MSBT) compared to that with traditional methods, using phantom models. Here we report our institutional clinical experience with MSBT. METHODS AND MATERIALS: MSBT patients from May 2001 to March 2007 were evaluated. Parameters analyzed included isocenter/multileaf collimation shifts, time per fraction (four fields), monitor units (MU)/fraction, femoral doses, maximal dose relative to body mass index,

The importance of the groin node status for the survival of T1 and T2 vulval carcinoma patients.

The purpose of this study was to analyze (1) the prognostic factors for survival of T1 and T2 carcinoma patients and (2) the impact of the initial groin node status for the time to recurrence and site of recurrence. We performed a follow-up study on 190 women with a T1 or T2 squamous cell carcinoma of the vulva. Data were obtained on age and general medical condition, the clinical and histological characteristics of the primary tumor and the inguinofemoral lymph nodes, treatment, recurrences, and survival. The standard treatment was radical vulvectomy with bilateral inguinofemoral lymphadenect

A patient with simultaneous absence of "classical" natural killer cells (CD3-, CD16+, and NKH1+) and expansion of CD3+, CD4-, CD8-, NKH1+ subset.

The clinical manifestations of putative natural killer (NK) cell deficiency are not well-known but theoretically should include recurrent tumors and systemic viral infections. In this article, we discuss a patient with recurrent condylomata, vulvar and cervical carcinoma in situ, pulmonary infiltrates of unknown significance, and a hypercoagulable state. This patient has a dramatic persistent deficiency in her circulating "classic" NK cells (CD3-, CD16+, NKH1+) and a simultaneous persistent expansion of a normally minor lymphocyte cell subset (CD3+, CD4-, CD8-, NKH1+) that does not express the

ROS1 Expression Correlates With Inguinal Lymph Node Affection in Vulvar Cancer Patients: A Retrospective Study.

PURPOSE: Systemic treatment options for vulvar squamous cell carcinoma (VSCC) are limited. ROS1, a tyrosine kinase implicated, for example, in non-small cell lung cancer (NSCLC), has recently shown responsiveness to tyrosine kinase inhibitors. This study investigated immunohistochemical ROS1 expression in VSCC to explore its potential as a future therapeutic target in this rare malignancy. METHODS: In this retrospective study, 48 patients with VSCC undergoing vulvectomy were included. Clinicopathological data were collected in a standardized manner. Immunohistochemistry (IHC) was used to asse

Risk for non-sentinel metastases in the contralateral groin in patients with a unilateral positive sentinel lymph node in primary vulvar cancer- a subgroup analysis of the AGO-VOP.2 QS Vulva Study.

UNLABELLED: The need for contralateral full groin dissection after bilateral sentinel node biopsy (SNB) with only unilateral detection of a macrometastasis is still a matter of debate. GROINSS-VI/II subanalysis has shown an absolute risk <3 % for contralateral groin recurrence or contralateral non-sentinel metastases, however, the individual risk varied widely dependent on the primary tumor characteristics. METHODS: The AGO VOP.2 QS vulva study is a retrospective, multicenter study. Data from 306 patients diagnosed with primary groin node positive VSCC (vulvar squamous cell carcinoma) between

Human epidermal growth factor receptor-2 (HER2) is a potential therapeutic target in extramammary Paget's disease of the vulva.

BACKGROUND: Invasive vulvar Paget's disease with over-expression of the human epidermal growth factor receptor 2 (HER2) protein is potentially suitable for targeted therapy, especially in a metastatic setting where no effective treatments are available. METHODS: Four consecutive patients with HER2 positive advanced vulvar Paget's disease, treated with weekly trastuzumab (loading dose 4 mg/kg, then 2 mg/kg) and paclitaxel (80 mg/m2) followed by 3-weekly trastuzumab maintenance (6 mg/kg), are reported. RESULTS: Median age and follow-up of patients were 62.5 years (45-74) and 16 months (6-54),

Vulval hidradenoma papilliferum: a clinical and dermoscopic study.

Hidradenoma papilliferum (HP) is a rare, slow-growing, benign adnexal tumour with apocrine differentiation. It usually affects the anogenital region in adult women and is clinically polymorphous, mimicking other benign or malignant neoformations. The dermoscopic features of vulval HP have not been reported yet. We report the clinical and dermoscopic features in a case series of histopathologically proven vulval HPs. Dermoscopy may be a useful tool for the diagnosis of vulval HP. To our knowledge, our paper represents the first report of dermoscopic findings in a series of vulval HPs in a multi

Molecular alterations in lesions of anogenital mammary-like glands and their mammary counterparts including hidradenoma papilliferum, intraductal papilloma, fibroadenoma and phyllodes tumor.

Lesions affecting anogenital mammary-like glands (AGMLG) are histopathologically very similar to those seen in the breast but whether this morphological similarity is also reflected at the genetic level is unknown. To compare the underlying molecular mechanisms in lesions of AGMLG and their mammary counterparts, we analyzed the mutational profile of 16 anogenital neoplasms including 5 hidradenomas papilliferum (HP), 1 lesion with features of HP and fibroadenoma (FA), 7 FA, 3 phyllodes tumors (PhT)) and 18 analogous breast lesions (6 intraductal papillomas (IDP), 9 FA, and 3 PhT) by high-covera

Repeat sentinel lymph node procedure in patients with recurrent vulvar squamous cell carcinoma is feasible.

OBJECTIVE: Standard treatment of primary T1 squamous cell carcinoma (SCC) of the vulva <4cm consists of wide local excision (WLE) and sentinel lymph node (SLN) procedure of the groin(s). In case of a local recurrence WLE and inguino femoral lymphadenectomy (IFL) is generally recommended. In this study we assessed the feasibility of repeat SLN procedure in patients with recurrent vulvar SCC who were not able or willing to undergo IFL. METHODS: A retrospective study was performed in consecutive patients with recurrent vulvar SCC who underwent a repeat SLN procedure between 2006 and 2014. We pre

The number of removed lymph nodes by inguinofemoral lymphadenectomy: impact on recurrence rates in patients with vulva carcinoma.

OBJECTIVE: Inguinal lymph node (LN) metastasis is a crucial prognostic factor in vulva carcinoma. The aim of this study was to determine the prognostic value of the number of resected LNs in patients with vulvar carcinoma on recurrence rates. METHODS: This retrospective study includes patients with vulvar squamous cell carcinoma who underwent inguinofemoral lymphadenectomy (IFL) between 1998 and 2011. Dissected groins were stratified by the number of removed lymph nodes (<6 LNs versus ≥6 LNs) or inguinal LN metastasis (pN- versus pN+) and analyzed according to groin, local and distance recurr

Primary mammary-like ductal carcinoma of the vulva: a case report and analysis of the literature.

Mammary-like vulvar carcinoma represents an extremely rare disease. Here, the authors report a case of mammary-like ductal carcinoma of the vulva. A 71-year-old woman affected by stage T2N1M0 mammary-like vulvar carcinoma underwent neoadjuvant chemotherapy, radical vulvectomy, and adjuvant antioestrogen hormone therapy. The patient is alive, 24 months after surgery, with no clinical and radiological evidence of disease. To provide a better insight in to the clinical approach for this rare disease, a review of the available literature was performed.

Flap algorithm in vulvar reconstruction after radical, extensive vulvectomy.

The objective of this study was to assess the reconstructive options after radical, extensive vulvectomy; relate them to tumor characteristics; and select a choice of flaps able to correct every remaining defect. This study is a retrospective review of a 4-year experience with 31 flaps in 20 consecutive vulvar reconstructions. Three of the 31 flaps presented nonsignificant delayed healing at their tips and 3 other flaps developed a major breakdown related to an infection or an error in flap planning. According to the authors, the size of the defect is the main issue that must be taken into con
