ISSLS prize winner: early predictors of chronic work disability: a prospective, population-based study of workers with back injuries.

STUDY DESIGN: Prospective population-based cohort study. OBJECTIVE: To identify early predictors of chronic work disability after work-related back injury. SUMMARY OF BACKGROUND DATA: Identification of early predictors of prolonged disability after back injury could increase understanding concerning the development of chronic, disabling pain, and aid in secondary prevention. Few studies have examined predictors across multiple domains in a large, population-based sample. METHODS: Workers (N = 1885) were interviewed 3 weeks (average) after submitting a lost work-time claim for a back injury.

Cultural and Social Determinants of Indigenous Peoples' Health: Understanding Oral Health Inequities in Canada, New Zealand/Aotearoa, and Australia.

OBJECTIVE: This cross-sectional study examined the cultural and social determinants of health in Canada, New Zealand, and Australia and their relevance to the oral health of Indigenous pregnant women. METHODS: An Indigenous framework was applied to a social determinants of health (SDH) model, Charlotte Loppie's Tree Metaphor, utilising data from the Baby Teeth Talk Study, a multi-national academic-community partnership, to compare SDHs among Indigenous pregnant women in Canada (n = 544), New Zealand (n = 222), and Australia (n = 446). The same survey instrument, with cultural adaptations, was

Developing a novel typology of unprofessional behaviours between healthcare staff: a best fit framework synthesis.

BACKGROUND: Unprofessional behaviours such as bullying, harassment, and microaggressions negatively affect patient safety and staff psychological wellbeing in healthcare systems globally. These behaviours do so by: (i) inhibiting health care professionals’ abilities to speak up to raise safety concerns; (ii) impairing team communication and individuals’ concentration; and (iii) promoting tolerance of bad practice. Unfortunately, there is little consensus in practice or academia about how these behaviours are defined. This can lead to an underestimation of the prevalence of these behaviours, in

Wearable-derived postdischarge mobility as a digital biomarker for 90-day readmission and mortality in metastatic cancer.

PURPOSE: The postdischarge period after unplanned hospitalization is a vulnerable transition for patients with metastatic cancer, often accompanied by functional decline, symptom burden, psychological distress, early readmission, and death. However, short-term risk assessment during this period still relies largely on episodic clinician-rated measures. We evaluated whether wearable-derived postdischarge step counts are associated with 90-day readmission, mortality, and patient-reported outcomes in patients with metastatic cancer. METHODS: In this prospective, two-center cohort study, adults w

The prevalence of anxiety, depression and the mediating role in leprosy patients: a nationally representative cross-sectional study.

Leprosy is a chronic disease that severely affects the skin and nerves, sometimes leading to disability. The deformities deepen stigma and discrimination, even among patients who have been cured. Consequently, leprosy patients are more likely to suffer from mental illnesses than other diseases. A cross-sectional study was conducted in four Chinese provinces (Fujian, Yunnan, Shandong, and Guizhou), including patients who had completed the full course of multidrug therapy (MDT). We assessed anxiety and depression using the SAS and SDS scales, and functional ability using the ADLs. Influencing fa

The social determinants of Aboriginal and Torres Strait Islander adults who do not smoke in regional Australia.

INTRODUCTION: Commercial tobacco use was systematically embedded as a valuable commodity through colonisation that continues to be exploited for profit by the Tobacco Industry. There have been significant declines in current smoking prevalence among Aboriginal and Torres Strait Islander peoples 18 years and over, from 55% in 1994 to 43% in 2018-2019. This paper seeks to better understand smoke-free behaviours, and to systematically quantify associations between a range of SDOH and non-smoking/never-smoking among Aboriginal and Torres Strait Islander adults (≥18) living in regional Australia.

An analysis from the Quality Outcomes Database, Part 1. Disability, quality of life, and pain outcomes following lumbar spine surgery: predicting likely individual patient outcomes for shared decision-making.

OBJECTIVE Quality and outcomes registry platforms lie at the center of many emerging evidence-driven reform models. Specifically, clinical registry data are progressively informing health care decision-making. In this analysis, the authors used data from a national prospective outcomes registry (the Quality Outcomes Database) to develop a predictive model for 12-month postoperative pain, disability, and quality of life (QOL) in patients undergoing elective lumbar spine surgery. METHODS Included in this analysis were 7618 patients who had completed 12 months of follow-up. The authors prospectiv

Interventions for managing taste disturbances.

BACKGROUND: The sense of taste is very much essential to the overall health of an individual. It is a necessary component to enjoy one's food, which in turn provides nutrition to an individual. Any disturbance in taste perception can hamper quality of life in such patients by influencing their appetite, body weight and psychological well-being. Taste disorders have been treated using different modalities of treatment and there is no consensus for the best intervention. Hence this Cochrane Review was undertaken. This is an update of the Cochrane Review first published in November 2014. OBJECTI

3'-Chloro-3 alpha-(diphenylmethoxy)tropane but not 4'-chloro-3 alpha-(diphenylmethoxy)tropane produces a cocaine-like behavioral profile.

A series of 2'- and 3'-substituted and 3',3"-disubstituted 3 alpha-(diphenylmethoxy)tropane analogs were designed and synthesized as novel probes for the dopamine transporter. All the analogs were evaluated for displacement of [3H]WIN 35,428 binding at the dopamine transporter and for inhibition of [3H]dopamine uptake in rat caudate putamen. Compounds were observed to monophasically displace [3H]WIN 35,428 binding to the dopamine transporter with affinities of 21.6-1836 nM (Ki). Generally, meta-substituted compounds were more potent than benztropine and equipotent to or slightly less potent th

Statement of the Independent Forensic Expert Group on Conversion Therapy.

Conversion therapy is a set of practices that aim to change or alter an individual's sexual orientation or gender identity. It is premised on a belief that an individual's sexual orientation or gender identity can be changed and that doing so is a desirable outcome for the individual, family, or community. Other terms used to describe this practice include sexual orientation change effort (SOCE), reparative therapy, reintegrative therapy, reorientation therapy, ex-gay therapy, and gay cure. Conversion therapy is practiced in every region of the world. We have identified sources confirming or i

The contribution of α4β2 and non-α4β2 nicotinic acetylcholine receptors to the discriminative stimulus effects of nicotine and varenicline in mice.

RATIONALE: The extent to which non-α4β2 versus α4β2* nAChRs contribute to the behavioral effects of varenicline and other nAChR agonists is unclear. OBJECTIVES: The purpose of this study was to characterize the discriminative stimulus effects of varenicline and nicotine using various nAChR agonists and antagonists to elucidate possible non-α4β2 nAChR mechanisms. METHODS: Separate groups of male C57BL/6J mice were trained to discriminate varenicline (3.2 mg/kg) or nicotine (1 mg/kg). Test drugs included mecamylamine; the nAChR agonists epibatidine, nicotine, cytisine, varenicline, and RTI-102

Involvement of dopamine D2 receptor signal transduction in the discriminative stimulus effects of the κ-opioid receptor agonist U-50,488H in rats.

We have reported previously that the inhibition of both dopaminergic and psychotomimetic/hallucinogenic components plays a role in the discriminative stimulus effects of U-50,488H. However, the mechanisms that underlie the discriminative stimulus effects of U-50,488H, and especially the component that plays a significant role, have not yet been clarified. The present study was designed to further investigate the mechanism(s) of the discriminative stimulus effects of the κ-opioid receptor agonist U-50,488H in rats that had been trained to discriminate between 3.0 mg/kg U-50,488H and saline. Reappraisal of somatosensory disorders in methylmercury poisoning.

The first well-documented methylmercury (MeHg) poisoning by consumption of fish arose in Minamata, Japan in 1953. MeHg had dispersed from Minamata to the Shiranui Sea. The temporal changes in MeHg in the umbilical cords indicate that residents living around that Sea had been exposed to low-dose MeHg through fish consumption for about 20 years (at least from 1950 to 1968). They have complained of paresthesia at the distal parts of the extremities and around the lip even 30 years after the cessation of exposure to anthropogenic MeHg. The thresholds of touch and two-point discrimination of those

Institutional barriers and professional integration of physicians with an Arab migration background in Germany: a qualitative study.

BACKGROUND: Germany faces a persistent physicians shortage f, particularly in hospitals and rural regions. Migrant physicians, especially from Arab countries, represent a crucial component. However, their professional integration is shaped by complex licensure procedures, demanding working conditions, administrative burdens and sociocultural dynamics. While previous research has examined structural and regulatory barriers, limited empirical evidence exists on integration processes from the perspective of Arab physicians themselves. PURPOSE: This study aimed to explore the challenges and exper

[Mental health service utilization among borderline personality disorder patients inpatient].

BACKGROUND: Borderline personality disorder (BPD) is characterized by a pervasive pattern of instability and impulsivity. Several North American prospective studies support the high level of mental health care utilization in this population. There is little data in other systems of health organization, such as France. Furthermore, little is known on the variables associated with the mental health service utilization among BPD patients. OBJECTIVE: The main objective was to compare the utilization of mental health care among BPD patients, to the general population and patients with another pers
