Delayed methicillin-resistant Staphylococcus aureus-induced osteomyelitis of the tibia after pin tract infection: two case reports.

BACKGROUND: Pin tract infection is a common complication of external fixation. It usually heals after treatment with debridement, antibiotics, and/or pin removal, only rarely developing into delayed osteomyelitis. We treated two patients with delayed methicillin-resistant Staphylococcus aureus-induced osteomyelitis of the tibia following pin tract infection. CASE PRESENTATION: One patient, a diabetic 60-year-old Japanese man, underwent definitive external fixation using an Ilizarov fixator for postoperative osteomyelitis of an open fracture of his left ankle. One year after removing the exter

Double plate fixation for the management of proximal interphalangeal joint instability in 30 horses (1987-2015).

REASONS FOR PERFORMING STUDY: Double plate fixation has been reported in a small number of horses with comminuted fractures of the middle phalanx. There are no published reports using this technique in the management of other injuries that destabilise the proximal interphalangeal joint (PIJ), including biaxial palmar/plantar eminence fractures and disruption of the palmar/plantar soft tissue support structures of the PIJ. Furthermore, there is a paucity of information on the long-term prognosis for horses undergoing double plate fixation to manage destabilising injuries of the PIJ. OBJECTIVES

Radial head replacement with unipolar and bipolar SBi system: a clinical and radiographic analysis after a 2-year mean follow-up.

Radial head prosthetic replacement is indicated in case of comminuted fracture not amenable to internal fixation, especially when the radial head fracture is part of a pattern of lesions configuring a complex instability of the elbow. Thirty-one SBi radial head prostheses were implanted in 30 patients (one bilateral simultaneous fracture) over a 2 years period. In 10 patients, the mean time from trauma to surgical treatment was 2.4 days, while the remaining 20 patients were treated as "second opinion" cases presenting with elbow stiffness or instability after an average of 19 days from trauma.

Long-term follow-up of partial limb amputation in 13 horses.

Thirteen cases of partial limb amputation in horses were reviewed. Nine horses (69%) survived 18 to 111 months (mean, 41 months). Four unsuccessfully treated horses were euthanatized within 21 days of surgery. The horses' ages at amputation were 4 to 13 years (mean, 7.3 years). There was no predilection for the left or right limb nor for fore or hind limb involvement. Osteomyelitis in six horses (46%) was the most common cause for amputation. Severe, open, comminuted fractures of the third metacarpal bone that were impossible to stabilize by any currently available technique in four horses (31

[The problem of osteosynthesis in peripheral replantation].

In this report, 16 cases of peripheral replantation are described, in which osteosynthesis was carried out by means of AO-mini or small-fragment plates with the obvious advantage of allowing early mobilisation. A disadvantage of this method is that it is necessary to dissect the soft tissue to a greater extent and to elevate the periosteum more than in using Kirschner wires, this dissection to permit sufficient fixation of the plate and screws. As a result, primary technical difficulties in the venous anastomosis may occur and there is a risk of adhesions developing between the extensor appara

[Articular coronal fractures of the distal humerus].

INTRODUCTION: The vague term of capitellar fractures is still frequently used to designate articular coronal fractures of the distal humeral epiphysis. The use of eponyms for their descriptions may cause confusion. Recent publications describe a wide variety of fracture types and recommend new classifications based on the operative findings. We report our results of surgical treatment of 12 cases of these fractures in comparison to recent series of the literature. METHODS: Twelve patients (seven female and five male with a mean age of 31 years and 6 months) have been treated for articular cor

Arthroscopic repair of glenoid rim fractures: a ligamentotaxis surgical technique.

PURPOSE: Glenoid fractures occur as a result of direct impact of the humeral head against the glenoid rim following high-energy trauma. They frequently involve one-third of the glenoid surface with an oblique fracture rim from 2 to 6-7 o'clock, and they must not be confused with bony Bankart lesions. In medium-age patients, they are frequently associated with acute cuff tear while in older patients with chronic cuff tear: These conditions increase the instability of the shoulder if not treated. With this study, we reported the results of the arthroscopic ligamentotaxis technique treatment of a

Clinical effect of distal radius fracture treated with open reduction and internal plate fixation.

BACKGROUND: For some specific comminuted unstable intra-articular fracture, the plaster cast can not maintain the alignment of the articular surface effectively. The aim of this study was to evaluate the clinical effects of distal radius fracture treated with open reduction and internal plate fixation retrospectively. METHODS: From January 2002 to March 2010, 539 cases of distal radius fracture were treated with open reduction and internal fixation, including 184 males and 355 females aging 21 - 72 years (mean 57 years). Fractures were caused by falling to the ground in 459 cases, by traffic

[Evaluation of impaction on bone nail and bone plate of the pertrochanteric fracture].

PURPOSE OF THE STUDY: Impaction in pertrochanteric fracture sites is a well known phenomenon; the screw-plate system is designed to stabilise the fracture. Although easier to use, the risk with the nail-plate system is postoperative penetration of the nail into the joint. The present study was conducted to determine the exact conditions of the impaction, and to identify possible ways to improve the nail-plate system. MATERIAL-METHOD: The study included 129 cases of pertrochanteric fracture, excluding sub-trochanteric fractures. All fractures were fixed with a 130 degrees angulated nail-plate.

Occult ascending pharyngeal artery pseudoaneurysm masked by CTA artifact after penetrating facial trauma: a case report.

Penetrating facial trauma can result in vascular abnormalities that are difficult to identify on initial imaging due to artifact from retained fragments. External carotid artery pseudoaneurysms are uncommon but may carry significant morbidity if not promptly recognized. A 34-year-old female presented after a gunshot wound to the left face. Initial computed tomography angiography (CTA) demonstrated left vertebral artery V3 dissection, severe left internal carotid artery stenosis, and a comminuted mandibular fracture with retained ballistic fragments causing substantial streak artifact. Due to p

Organic monolayers disrupt plastic flow in metals.

Adsorbed films often influence mechanical behavior of surfaces, leading to well-known mechanochemical phenomena such as liquid metal embrittlement and environment-assisted cracking. Here, we demonstrate a mechanochemical phenomenon wherein adsorbed long-chain organic monolayers disrupt large-strain plastic deformation in metals. Using high-speed in situ imaging and post facto analysis, we show that the monolayers induce a ductile-to-brittle transition. Sinuous flow, characteristic of ductile metals, gives way to quasi-periodic fracture, typical of brittle materials, with 85% reduction in defor
