Microbiology of postoperative infections.

BACKGROUND: Surgical infections, accounting for approximately one quarter of all nosocomial infections, contribute to longer hospital stays and higher medical costs. Clinical failure of antibiotic treatment often is associated with increasing drug resistance. METHODS: Non-duplicated microbial isolates obtained at our institution from surgical patients were identified and tested for antimicrobial susceptibility using the Sceptor system in 2001-2002 and the Phoenix system in 2003-2005. Epidemiologic data were extracted using Epicenter and Powerlab. During selected periods, genetic and biochemic

Frequent expression of P-glycoprotein/MDR1 by nasal T-cell lymphoma cells.

BACKGROUND: Lethal midline granuloma is now considered to be a malignant lymphoma derived from peripheral T cells or from natural killer cells. The therapeutic outcome of nasal T-cell lymphoma (NL) treated by conventional chemotherapy for non-Hodgkin's lymphoma is poor, although some patients have a good response to radiotherapy. To clarify the mechanisms of drug resistance, the expression of P-glycoprotein (P-gp)/MDR1, which is the product of the multidrug resistance (MDR) 1 gene, and MDR3 mRNA in NL cells, were examined. METHODS: Ten Japanese patients with NL were studied. Nine of these pat

Reactivity of P-glycoprotein monoclonal antibodies in childhood cancers.

P-Glycoprotein (P-gp), the product of the mdr-1 gene, is implicated in the development of chemoresistance in a variety of, mostly adult, cancers. Its role in paediatric tumours, most of which are non-epithelial in origin, has yet to be fully elucidated. A study was undertaken to investigate reactivity of two P-gp monoclonal antibodies (MAbs), JBS-1 and MRK16, recognising cytoplasmic and surface epitopes, respectively, of the P-gp molecule, in a variety of newly diagnosed and relapsed childhood cancers. P-gp was not expressed in any of 36 tumours examined (neuroblastoma 13, nephroblastoma 12, r

Petting Zoo Animals as an Emerging Reservoir of Extended-Spectrum β-Lactamase and AmpC-Producing Enterobacteriaceae.

Extended spectrum beta-lactamases and AmpC-producing Enterobacteriaceae (ESBL/AmpC-E) have become a great concern in both human and veterinary medicine. One setting in which this risk could be particularly prominent is petting zoos, in which humans, especially children, directly and indirectly interact with the animals. Yet, while the zoonotic transmission of various Enterobacteriaceae has been reported previously in petting zoos, reports on ESBL/AmpC-E shedding in this setting is currently lacking, despite the high potential risk. To fill this knowledge gap, we conducted a prospective cross-s

Machine learning identifies MiRNA biomarkers and immune mechanisms in active tuberculosis.

Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), remains a major global public health threat. The rising prevalence of HIV/TB co-infection and multidrug-resistant tuberculosis (MDR-TB) has further intensified this challenge. This study aims to explore the role of microRNAs (miRNAs) in the immune response to Mtb infection and to identify potential miRNA biomarkers for active TB diagnosis using machine learning techniques. miRNA expression profiles were retrieved from the Gene Expression Omnibus (GEO) database (accession number: GSE70425). Differential expression analysis between a

Exploring evolutionary perspectives and antibiogram analysis of biofilm-forming Staphylococcus aureus in goat mastitis.

Biofilm formation plays a key role in developing antimicrobial resistance in Staphylococcus aureus (S. aureus) and is thus a major virulence factor responsible for persistent intramammary infections and subclinical mastitis in goats. The current study investigated the prevalence and molecular characterization of biofilm-forming S. aureus isolated from goat mastitis, the associated risk factors, and comparative antibiogram profiling. A total of 768 milk samples were conveniently collected from farms dairy goats in three districts of Pakistan Multan, Bahawalpur, and Rahim Yar Khan. The results r

Genomic heterogeneity underlies multidrug resistance in Pseudomonas aeruginosa: A population-level analysis beyond susceptibility testing.

BACKGROUND: Pseudomonas aeruginosa is a persistent and difficult-to-treat pathogen in many patients, especially those with Cystic Fibrosis (CF). Herein, we describe a longitudinal analysis of a series of multidrug resistant (MDR) P. aeruginosa isolates recovered in a 17-month period, from a young female CF patient who underwent double lung transplantation. Our goal was to understand the genetic basis of the observed resistance phenotypes, establish the genomic population diversity, and define the nature of sequence evolution over time. METHODS: Twenty-two sequential P. aeruginosa isolates wer

Computational Guided Drug Targets Identification against Extended-Spectrum Beta-Lactamase-Producing Multi-Drug Resistant Uropathogenic Escherichia coli.

Urinary tract infections (UTIs) are one of the most frequent bacterial infections in the world, both in the hospital and community settings. Uropathogenic Escherichia coli (UPEC) are the predominant etiological agents causing UTIs. Extended-spectrum beta-lactamase (ESBL) production is a prominent mechanism of resistance that hinders the antimicrobial treatment of UTIs caused by UPEC and poses a substantial danger to the arsenal of antibiotics now in use. As bacteria have several methods to counteract the effects of antibiotics, identifying new potential drug targets may help in the design of n

Phenotypic and genomic analysis reveals Riemerella anatipestifer as the potential reservoir of tet(X) variants.

BACKGROUND: Tigecycline is regarded as one of the last-resort antimicrobials clinically. Emergence of plasmid-mediated tet(X) undermines such an important drug. However, the origins of tet(X) remain largely unexplored. METHODS: Riemerella anatipestifer strains were characterized by PCR, antimicrobial susceptibility testing, WGS and bioinformatics analysis. Functional analysis of tet(X) was verified by cloning experiments. Genomic structures of chromosome- and plasmid-mediated tet(X) were analysed. RESULTS: Thirty-eight R. anatipestifer strains were collected and found to be positive for tet(

Auxin (Indole-3-acetic acid) modulation of quorum sensing enhances phage susceptibility in Klebsiella pneumoniae.

PURPOSE: Klebsiella pneumoniae is a multidrug-resistant (MDR) bacterium that has emerged as a major global public health threat. The decreasing effectiveness of conventional antibiotics has renewed interest in bacteriophage therapy as a promising alternative antibacterial strategy. However, the rapid emergence of phage resistance remains a critical limitation. This study aimed to investigate whether indole-3-acetic acid (IAA) can modulate bacterial quorum sensing (QS) and enhance phage therapy by modulating the QS regulator SdiA. METHODS: Experiments were performed using the lytic bacteriopha
