Autopsy-Proven Immune-Mediated Hepatitis With Rapid Liver Atrophy Despite Minimal Aminotransferase Elevation After Durvalumab Plus Tremelimumab Therapy for Hepatocellular Carcinoma: A Case Report.

Immune-mediated hepatitis caused by immune checkpoint inhibitors (ICIs) is typically suspected based on aminotransferase elevation; however, biochemical signals may be blunted in advanced cirrhosis, potentially delaying recognition. A 71-year-old woman with cirrhosis due to metabolic dysfunction-associated steatohepatitis and Barcelona Clinic Liver Cancer stage B hepatocellular carcinoma refractory to transarterial chemoembolization started durvalumab plus tremelimumab therapy. The patient developed grade 3 immune-mediated colitis requiring high-dose corticosteroids, with sustained clinical re

Challenges in diagnosing and treating distal common bile duct adenocarcinoma: A case report with literature insights.

BACKGROUND: Adenocarcinoma of the distal common bile duct (CBD) is a rare and aggressive malignancy that is often diagnosed at an advanced stage owing to nonspecific symptoms and delayed presentation. This case report details the diagnostic and therapeutic challenges associated with distal CBD adenocarcinoma and highlights the need for an effective multidisciplinary approach. CASE PRESENTATION: A 54-year-old male with a significant smoking history presented with persistent right upper abdominal pain, dark urine, and scleral jaundice. Imaging studies revealed intrahepatic bile duct dilatation,

An Unusual Cause of Hypokalemia to Consider.

Apparent mineralocorticoid excess syndrome is a rare disorder that can be acquired through inhibition of the enzyme 11 β-hydroxysteroid dehydrogenase type 2 by various substances such as bile acids. We report the case of a 61-year-old woman presenting with painless jaundice. Computed tomography demonstrated a pulmonary as well as a pancreatic tumor, with multiple metastases and dilated bile ducts. Laboratory findings showed persistent hypokalemia despite aggressive enteral and parenteral supplementation, as well as hypertension, metabolic alkalosis, and elevated cholestasis enzymes. Urinary po

Acute cholangitis with Achromobacter xylosoxidans bacteremia after endoscopic retrograde cholangiopancreatography in hilar cholangiocarcinoma: A case report.

BACKGROUND: Achromobacter xylosoxidans is a Gram-negative opportunistic aerobe, usually causing nosocomial infections in immunocompromised patients with manifestations including bacteremia, pneumonia, and catheter-related infections. However, A. xylosoxidans have not yet been reported to cause biliary system infections. CASE SUMMARY: A 72-year-old woman presented to the outpatient department of our hospital with a chief complaint of jaundice. Computed tomography of her abdomen revealed the presence of a mass of approximately 2.4 cm in the hilar portion of the common hepatic duct, consistent w

Parasitic Infections in Pulmonary and ICU Patients: Presentation, Diagnosis, and Treatment.

Parasitic infections in the United States are mostly seen in immigrants and travelers. In many cases, pulmonary and intensive care physicians fail to consider parasitic disease, which can result in delayed diagnosis and adverse outcomes. Almost 2,000 cases of imported malaria are diagnosed in the United States each year. Severe cases can be confused with bacterial sepsis (shock, lactic acidosis, pneumonia, renal failure, respiratory failure, and jaundice). In contrast to bacterial sepsis, survival is improved by restrictive fluid therapy. Parenteral artesunate is licensed to treat severe cases

Principles of Palliative and Supportive Care in Pancreatic Cancer: A Review.

Pancreatic adenocarcinoma (PDAC) is well known for its poor survival time. Clinical symptoms are painless jaundice or abdominal or back pain. Less specific symptoms often appear that make diagnosis difficult, e.g., weight loss, loss of appetite, nausea and vomiting, and general weakness. Only 10-20% of patients are diagnosed at an early stage. A cure is practically only possible with a radical surgical operation. In the case of locally advanced findings, neoadjuvant therapy is administered. Among the therapeutic options offered are chemotherapy, radiotherapy (including stereotactic radiotherap

Feasibility of therapeutic endoscopic ultrasound in the bridge-to-surgery scenario: The example of pancreatic adenocarcinoma.

Upfront resection is becoming a rarer indication for pancreatic ductal adenocarcinoma, as biologic behavior and natural history of the disease has boosted indications for neoadjuvant treatments. Jaundice, gastric outlet obstruction and acute cholecystitis can frequently complicate this window of opportunity, resulting in potentially deleterious chemotherapy discontinuation, whose resumption relies on effective, prompt and long-lasting management of these complications. Although therapeutic endoscopic ultrasound (t-EUS) can potentially offer some advantages over comparators, its use in potentia

Pyrrolizidine alkaloids-induced hepatic sinusoidal obstruction syndrome: Pathogenesis, clinical manifestations, diagnosis, treatment, and outcomes.

Hepatic sinusoidal obstruction syndrome (HSOS) can be caused by the intake of pyrrolizidine alkaloids (PAs). To date, PAs-induced HSOS has not been extensively studied. In view of the difference in etiology of HSOS between the West and China, clinical profiles, imaging findings, treatment, and outcomes of HSOS associated with hematopoietic stem cell transplantation or oxaliplatin might be hardly extrapolated to PAs-induced HSOS. Reactive metabolites derived from PAs form pyrrole-protein adducts that result in toxic destruction of hepatic sinusoidal endothelial cells. PAs-induced HSOS typically

[Eruptive disseminated superficial porokeratosis associated with acute hepatitis E].

BACKGROUND: Porokeratosis (PK) is a rare form of dermatosis characterized by a keratinization disorder of unknown etiology. Herein we describe the first case associated with hepatitis E virus infection. PATIENTS AND METHODS: A 69-year-old patient with colorectal cancer treated with radiation and chemotherapy followed by surgery in April 2017 presented two months later with jaundice associated with annular keratotic lesions of the skin with a raised border. Blood tests revealed elevated liver enzymes and hyperbilirubinemia. Viral hepatitis E was diagnosed based on serology and viral PCR after

Alagille syndrome and a JAG1 mutation: 41 cases of experience at a single center.

PURPOSE: Alagille syndrome is a complex hereditary disorder that is associated with cardiac, hepatic, skeletal, ocular, and facial abnormalities. Mutations in the Notch signaling pathway, such as in JAG1 and NOTCH2, play a key role in embryonic development. A cardiac or hepatic presentation is a critical factor for determining the prognosis. METHODS: We conducted a retrospective study of 41 patients with Alagille syndrome or a JAG1 mutation between 1983 and 2013. RESULTS: The first presentations were jaundice, murmur, cyanosis, and small bowel obstruction at a median age of 1.0 months (range

Diagnosis and management of pancreatic cancer.

Pancreatic cancer remains the fourth leading cause of cancer-related deaths in the United States. Risk factors include family history, smoking, chronic pancreatitis, obesity, diabetes mellitus, heavy alcohol use, and possible dietary factors. Because more than two-thirds of adenocarcinomas occur in the head of the pancreas, abdominal pain, jaundice, pruritus, dark urine, and acholic stools may be presenting symptoms. In symptomatic patients, the serum tumor marker cancer antigen 19-9 can be used to confirm the diagnosis and to predict prognosis and recurrence after resection. Pancreas protocol

Perforation: a rare complication of choledochal cysts in children.

PURPOSE: Spontaneous perforation is a rare complication of choledochal cyst (CDC) which is difficult to diagnose due to its nonspecific clinical presentation. The surgical treatment can be either single-staged cyst excision or two-staged procedure with an initial drainage followed by delayed cyst excision. Both biliary duct obstruction and irritation due to refluxed pancreatic juice have been proposed as possible aetiology. In this report, we describe six cases of CDC perforation in hope to have a better understanding on the clinical features and cause of this complication. METHODS: Medical r

Autoimmune pancreatitis characterized by predominant CD8+ T lymphocyte infiltration.

Autoimmune pancreatitis (AIP) is a rare form of pancreatitis characterized by prominent lymphocyte infiltration and pancreatic fibrosis resulting in organ dysfunction. The pathogenesis and pathology of AIP remain unknown. A 64-year-old Chinese man presented with symptoms and signs of bile duct obstruction diffuse enlargement of the head of pancreas, elevated IgG levels, and negative autoimmune antibody responses. A pylorus-preserving pancreatoduodenectomy was performed and a pancreatic tumor was suspected. However, periductal lymphoplasmacytic infiltration and fibrosis were found in the head o

EUS-assisted rendezvous stenting of the pancreatic duct for chronic calcifying pancreatitis with multiple pseudocysts.

INTRODUCTION: The best choice of endoscopic drainage of pancreatic pseudocysts complicating chronic pancreatitis is currently unknown, with EUS-guided transmural drainage competing with ERCP transpapillary techniques. However, recent studies currently recommend the use of both techniques in complex cases. CASE PRESENTATION: We present the case of a 60-year-old male patient with chronic calcifying pancreatitis, with severe ductal obstruction and multiple communicating pancreatic pseudocysts. The patient presented in the emergency department with weight loss, jaundice, steatorrhea and diabetes.

Cholangiocarcinoma presenting as a solitary epididymal metastasis: a case report and review of the literature.

BACKGROUND: Solid tumor metastasis to the epididymis is a rare occurrence and is mostly discovered incidentally at autopsy or after therapeutic orchidectomy for prostate cancer. Other primary carcinomas that have been demonstrated to metastasize to the paratesticular region include those originating in the stomach, kidney, ileum, and colon. CASE PRESENTATION: A 72-year-old gentleman presented with a firm and tender mass involving the right epididymis. On examination, he was jaundiced. Computed tomography of the abdomen demonstrated an obstructive stricture of the extra-hepatic bile ducts, in

Endoscopic management of bile duct leaks after attempted laparoscopic cholecystectomy.

Fourteen patients with symptomatic bile duct leaks following laparoscopic cholecystectomy were treated using endotherapeutic techniques. Patients presented with abdominal pain, liver test abnormalities, jaundice, leukocytosis, and fever. Twelve leaks originated from cystic duct stumps and two from right posterior hepatic ducts. Distal biliary obstruction, which may have promoted leakage, was present in five patients. Treatment methods included stent insertion with endoscopic sphincterotomy (ES), stent insertion without ES, and nasobiliary tube (NBT) placement without ES. Eleven of 14 patients

The management of fistulas of the biliary tract after injury to the bile duct during cholecystectomy.

Twelve of 123 patients who were treated for benign stricture of the bile duct after cholecystectomy presented with an external fistula of the biliary tract. Nine of these patients had undergone multiple operations prior to referral, six having undergone earlier attempts at repair of the bile duct. Two patients initially required urgent laparotomy for drainage of infected abdominal collections. Distal obstruction of the bile duct below the origin of the fistula was present in five patients. Repair of the bile duct was undertaken after careful investigation and correction of nutritional, fluid a
