The spread of tuberculosis is directly related to the processes of globalization and migration. Tuberculosis has also been the main cause of fatality associated with antimicrobial resistance and also the main cause of death in people who have HIV infection. Additionally, tuberculosis smites the lungs in 80% of patients, and in the remaining 20% of patients the tuberculosis may smites other organs, such as the vision/eye. Ocular tuberculosis is a specific infectious disease of bacterial etiology with a chronic and persistent course, the prognosis of which is extremely doubtful. Even effective c

Congenital tuberculosis (CTB) after in vitro fertilization (IVF) is a major new problem in developing countries. Only 16 cases of CTB after IVF have been reported, and no tuberculosis (TB) tests were performed before IVF in these cases. However, on the basis of data in the literature and from the World Health Organization, the incidence of CTB has been substantially underestimated. We describe two cases of CTB after IVF in detail in our center and provide new insight into the important issue of controlling TB vertical transmission in developing countries. Performing an early diagnosis of CTB,

BACKGROUND: Active tuberculosis (TB) infection was diagnosed in two health care workers (HCWs) originally from high-incidence countries at a National Health Service (NHS) hospital trust in Northern England. In response, the trust screened current clinical employees from countries with a high TB prevalence for active and latent TB infection (LTBI).
AIMS: To identify the number of HCWs, within the organization, who are at risk of TB infection.
METHODS: Clinical employees from countries with a high TB prevalence (those described by the World Health Organization as having an incidence of >40 cas

OBJECTIVE: Although the Bacillus Calmette-Guérin vaccine (BCG) protects young children against serious forms of TB, protection against pulmonary TB is variable. We assessed BCG vaccine-induced cellular immune responses and determined for how long they could be detected during childhood in Antananarivo, Madagascar.
METHODS: We assessed BCG vaccine-induced cellular immune responses by TST and IGRA (in-house ELISPOT assay) using BCG and PPD as stimulation antigen, and compared results between vaccinated and non-vaccinated schoolchildren of two age groups, 6-7 and 13-14 years old.
RESULTS: Three

BACKGROUND: The authors aimed to compare TST with QuantiFERON-TB Gold In-Tube (QFT) for detecting latent tuberculosis infection (LTBI) in HIV-infected patients in Iran as a TB-medium endemic country.
METHODS: In a cross-sectional design,130 HIV-positive patients in HIV clinic of Tehran University were studied. Patients screened for LTBI with QFT and TST simultaneously. Concordance between 2 tests results and variables associated with tests discordance were assessed.
RESULTS: Positive results found in 24.6% for TST and 7.7% for QFT. Overall agreement between them was 73.85% (Kappa = 0.083). H

INTRODUCTION: Healthcare workers (HCWs) in low incidence countries with contact to patients with tuberculosis (TB) are considered a high-risk group for latent TB infection (LTBI) and therefore are routinely screened for LTBI. The German Occupational TB Network data is analyzed in order to estimate the prevalence and incidence of LTBI and to evaluate putative risk factors for a positive IGRA and the performance of IGRA in serial testing.
METHODS: 3,823 HCWs were screened with the Quantiferon Gold in Tube (QFT) at least once; a second QFT was performed on 817 HCWs either in the course of contac

The Bacille Calmette-Guérin (BCG) vaccine is the only vaccine currently available for tuberculosis, and it demonstrates variable efficacy against the disease. The assessment of new vaccine strategies is hindered by the small annual probability that an infected individual will develop tuberculosis, and the lack of simple and reliable surrogate markers of protection. The frequency of cytokine-producing T cells as well as the production of IFN-γ have been disputed as surrogate markers of protection. We evaluated the evolution of these immune parameters in a population from a high burden city wher

INTRODUCTION: The diagnosis of latent tuberculosis (LTB) in patients with rheumatoid arthritis (RA) has become important with the introduction of anti-tumor necrosis factor (anti-TNF-α) agents and the appearance of active tuberculosis cases in these patients. The tuberculin skin test (TST) has limited value in patients with RA. Tests based on the release of interferon-gamma (IFN-γ) are being studied, but their role has not been well established for this group of patients.
OBJECTIVES: To compare the diagnosis of LTB in patients with RA by using cellular immune response to the TST and T.SPOT-TB

Children in South Korea are vaccinated with either BCG Pasteur vaccine intradermally (ID), or with BCG Tokyo vaccine given by multipuncture device (MP). Data from a recent national survey indicated that in children under 6 years old, 31.1% had received the ID vaccine and 64.5% the MP vaccine. To compare the T cell responses induced by the two vaccines, children aged 3-7 were recruited and tested for tuberculin skin test reactivity and for in vitro IFN-γ responses to mycobacterial antigens. DTH responses were not significantly different in children vaccinated by either the ID or MP vaccines. PP

SETTING: A rural section of a county in central Florida.
BACKGROUND: Racial disparities in tuberculosis disease (TB) are substantial in the United States.
OBJECTIVE: To determine if TB was attributable to primary infection, reactivation or both.
DESIGN: A population-based survey of latent tuberculosis infection (LTBI), a case-control analysis of TB, and a cluster analysis of TB isolates were performed between 1997 and 2001.
RESULTS: Of 447 survey participants, 135 (30%) had LTBI. Black race was strongly associated with LTBI among US-born (OR 2.6, 95%CI 1.3-5.5) and foreign-born subjects (O

For a number of reasons--but especially the human immunodeficiency virus (HIV) epidemic--tuberculosis (TB) is currently increasing in many parts of the world where it threatens the lives of millions of infants and children. The diagnosis of childhood TB, which has always been difficult, is now even more challenging in infants and children co-infected with HIV and Mycobacterium tuberculosis. Both TB and HIV infection require multiple drugs for effective treatment and to prevent the development of resistance by the infecting agents. Drug-drug interactions are therefore common, and knowledge of t

Tuberculosis (TB) in children is an endemic and sometimes life-threatening disease in Taiwan. This study analyzed the clinical characteristics of a total of 112 children with TB managed in a referral children's hospital in the north of Taiwan between 1998 and 2002. The diagnosis of TB was made by either a positive result of culture, acid-fast stain, histopathology, or polymerase chain reaction (PCR). Tuberculin skin test was positive (indurations > or =10 mm) in 73% of 63 patients tested. The male-to-female ratio was 0.93 (54/58), and the mean age was 10.7 years. Thirty one percent of children

Out of 313 preschool children treated in a specialized children's hospital for local tuberculosis, 23.6% of cases were first diagnosed in phases of reverse development. 12.8% of children cured spontaneously. Local activation as infiltration of intrathoracic lymph nodes and dissemination to the lungs and other organs occurred in 8.1 cases. The activation was seen both in children with spontaneously cured tuberculosis and those after chemoprophylaxis. It is concluded that all the children with newly detected tuberculosis in reverse phase, irrespective of previous chemoprophylaxis, should be trea
